Orthodontics and temporomandibular disorders: a curriculum proposal for postgraduate programs.

In a previous article, we reported the results of a survey of American and Canadian orthodontic postgraduate programs to determine how the t … ion's Commission on Dental Accreditation and the written guide (July 2010) of the American Board of Orthodontics for the its clinical examination. The proposed curriculum, based on the latest scientific evidence in the temporomandibular disorder field, gives program directors a template for covering these subjects thoroughly. At the same time, they can focus o …

Commentary on: periodontally accelerated osteogenic orthodontics (PAOO) - a clinical dilemma.

It is apparent that tooth movement is enhanced by procedures that elevate the remodeling of alveolar bone, and of periodontal and gingival fibrous tissues. The periodontally accelerated osteogenic orthodontics (PAOO) also termed as Wilckodontics, involves full-thickness labial and lingual alveolar flaps accompanied with limited selective labial and lingual surgical scarring of cortical bone (corticotomy). Most of the authors suggest that the RAP is the major stimulus for alveolar bone remodeling, enabling the PAOO. However, we propose that detachment of the bulk of dentogingival and interdenta…

[Contribution of orthodontics in treatment planning patients with reduced periodontium].

Orthodontics is the planned movement of teeth that can be carried thanks to the provoked resorption and apposition of osseous tissue. From the standpoint of the periodontium orthodontists should always be cognizant of the risk of iatrogenically weakening soft tissue status but at the same time they should consider that an improved character of the periodontium is one of the benefits that can be derived from mechano-therapy. These periodontal improvements, in the broadest sense of the term, can result from traditional overall therapy as well as treatment aimed solely at a local condition requir…

Orthodontics in a quantum world II: cranial movement and parafunction.

Maintenance of brain vitality is the most important function of the body. Several oral behaviors are involved in this process. Typical facial characteristics and intra-oral change can result from this. Symptomatically, the response can resemble a temporomandibular joint disorder. An integrative systems approach gives a basis for understanding and treating the condition.

[Orthodontics in general practice 4. Impaction of maxillary front teeth].

Eruption failures in the maxillary frontal region occur during the mixed dentition period. Dislocation and impaction of central incisors are diagnosed during or at the end of the early mixed dentition stage. These failures can be caused by crowding, premature loss of deciduous teeth, trauma or supernumerary teeth. Occasionally, a preventive orthodontic treatment is desirable to release the obstructed eruption path of a canine. Sometimes, a surgical intervention may be indicated to expose an obstructed central incisor and to remove supernumerary teeth. Retention and impaction of canines are gen

Maxillary lateral incisor implants: planning with the aid of orthodontics.

Implants are commonly used to replace congenitally missing lateral incisors in adolescent orthodontic patients. However, these restorations are often challenging for the orthodontist, surgeon, and restorative dentist. In some patients the space across the alveolar crest is too narrow to permit the surgeon to place the implant. Occasionally the root apices of the adjacent central incisor and canine are in close proximity. In other cases the ridge thickness could be inadequate and require augmentation. When the orthodontist opens the space, the papilla heights are adversely affected. Some adoles

Rapid orthodontics with alveolar reshaping: two case reports of decrowding.

Two case reports demonstrate a new orthodontic method that offers short treatment times and the ability to simultaneously reshape and increase the buccolingual thickness of the supporting alveolar bone. A 24-year-old man with a Class I severely crowded malocclusion and an overly constricted maxilla with concomitant posterior crossbites and a 17-year-old female with a Class I moderately to severely crowded malocclusion requested shortened orthodontic treatment times. This new surgery technique included buccal and lingual full-thickness flaps, selective partial decortication of the cortical plat

The feasibility and efficacy of tobacco use prevention in orthodontics.

SMILES PLUS was the first study to extend the clinician-delivered logic model to prevention of tobacco use among adolescents. This multi-site trial with 154 participating offices, based on social learning theory and a behavioral ecological model, was designed to test whether orthodontists can prevent preteens from initiating smoking. The study found that orthodontists do not automatically adhere to anti-tobacco prevention services. Social learning variables can enhance both adherence to counseling guidelines and content of counseling to increase prevention effects. Providing financial incentiv

Orthodontics and orthognathic surgery: adjuncts to restorative and periodontal therapy.

Orthodontic and orthognathic surgical procedures are important adjuncts to the provision of optimum restorative and periodontal care. Dentoalveolar and skeletal abnormalities of either developmental or acquired origin can be corrected primarily, thus eliminating the necessity for compromise in prosthetic design. Dental relationships that are contributory to irreversible periodontal deterioration can be normalized to facilitate stabilization of the periodontal condition and to increase the longevity of the natural dentition. Maxillofacial surgical treatment is technically complex but is more co

Regenerative Orthodontics: GBR and Corticotomy to Stretch the Limits of Orthodontic Treatment.

Orthodontic therapy could lead to marginal bone resorption in cases where the teeth are moved outside the envelope of bone. The purpose of this case series was to test corticotomy with a guided bone regeneration (GBR) procedure to regenerate bone in the direction of movement outside the original bony housing. Ten adult patients (60 anterior teeth), all presenting with severe anterior crowding, were enrolled in the study. Orthodontic therapy in all investigated sites was associated with selective surgical corticotomies and a simultaneous GBR procedure. CBCT examinations were performed before st

Orthodontics and the temporomandibular joint: What orthodontic providers need to know.

Currently, there are three major groups of dentists who provide orthodontic services to the public: postgraduate trained orthodontists, postgraduate trained pediatric dentists, and general dentists who have taken various orthodontic training courses. All of them can expect to encounter a variety of clinical situations that require a proper understanding of how normal temporomandibular joints (TMJs) function, and also how the masticatory system can develop pain-dysfunction problems; those problems are classified and defined as temporomandibular disorders (TMDs). In this paper, six "intersection

Lingual orthodontics as an aesthetic resource in the preparation of orthodontic/surgical treatment.

The lingual technique was introduced in the late '70s and, just like orthodontics, it has developed significantly in the past few years. Adult and young adult have been attracted to such alternative aesthetic treatment. Despite their need of orthodontic treatment, they want to have an aesthetic alternative, as "invisible" as possible--something that can be achieved with the lingual technique. Nevertheless, many treatment or re-treatment cases also demand correction through orthognathic surgery, a process which is nowadays much simpler and considered an effective auxiliary method for the malocc…

Craniofacial Orthodontic Experience in CODA-Accredited Orthodontic Residency Programs.

To evaluate orthodontic care for patients with craniofacial anomalies (CFA) by identifying orthodontic residents' preparedness to treat certain conditions and willingness to receive more training in CFA. A 12-question survey was sent through the American Association of Orthodontics (AAO) organization to orthodontic residents. Questions were primarily designed to obtain information on the frequency with which they dealt with patients with CFA in their training, specific craniofacial conditions that orthodontic residents feel comfortable treating. A total of 150 participants out of 1066 responde…

Orthodontic-periodontal interactions: Orthodontic extrusion in interdisciplinary regenerative treatments.

Orthodontics is a periodontal treatment. "Guided orthodontic regeneration" (GOR) procedures use orthodontic movements in perio-restorative patients. The GOR technique includes a guided orthodontic "soft tissue" regeneration (GOTR) and a guided orthodontic "bone" regeneration (GOBR) with a plastic soft tissue approach and a regenerating reality. The increased amount of soft tissue gained with orthodontic movement can be used for subsequent periodontal regenerative techniques. The increased amount of bone can as well improve primary implant stability and, eventually, simplify a GTR technique to …

Acceleration of tooth movement during orthodontic treatment--a frontier in orthodontics.

Nowadays, there is an increased tendency for researches to focus on accelerating methods for tooth movement due to the huge demand for adults for a shorter orthodontic treatment time. Unfortunately, long orthodontic treatment time poses several disadvantages like higher predisposition to caries, gingival recession, and root resorption. This increases the demand to find the best method to increase tooth movement with the least possible disadvantages. The purpose of this study is to view the successful approaches in tooth movement and to highlight the newest technique in tooth movement. A total
