Audit of stapedectomy results in a teaching hospital.

The number of stapedectomies performed annually has declined since 1963. Also, surgery has become available at an increasing number of centres, resulting in a further fall in the number of such operations performed in teaching hospitals. Several studies have suggested benefit might be gained, both by patients and by otology as a speciality, from limiting training to post-graduates who show an aptitude for otology, and concentrating surgery in a small number of specialized centres. We report a series of 179 patients who had stapedectomy performed for otosclerosis in a teaching hospital. We have

Overlapping otolaryngologic surgery: Safety and efficacy.

BACKGROUND: Surgical procedures scheduled staggered between two operating rooms increase efficiency by eliminating turnover time. However, the practice might increase the surgeon's fatigue. Overlapping surgery has been assumed to be safe because no critical portions of procedures are performed simultaneously in two rooms, but there is little evidence in the literature to support that assumption for otolaryngologic surgery, and there is no evidence comparing non-overlapping and overlapping surgical outcomes for a single surgeon with all confounding factors controlled. METHODS: Retrospective co

Surgical exposure of the facial nerve: contemporary transtemporal techniques.

Contemporary surgical exposure of the facial nerve has as its aims adequate exposure of the facial nerve at any point in its course, with preservation of hearing and vestibular function, and without further injury to the facial nerve and the necessity for producing a mastoid cavity. Today it is surgically possible to expose the facial nerve from the brainstem to its terminal branches in the face. A decade of experience has shown that if the operator is armed with intimate anatomic knowledge of the temporal bone and middle and posterior fossae, and if the principles of facial nerve surgery outl

Surgery of major congenital aural atresia.

Accurate preoperative clinical, audiometric and radiographic evaluation is a prerequisite for surgery of major congenital aural atresia, which is accepted as one of the most difficult otological reconstructive procedures to perform. The main aim is to achieve serviceable hearing. Middle ear and facial nerve anomalies and functional results of eleven consecutive primary operations by one of the authors are presented. Familiarity with various Tympanoplastic and Facial nerve surgical techniques is mandatory.

Clinical Application of the SAMEO-ATO Classification and Analysis of the Correlation Between Individual Subgroups and Surgery Duration.

INTRODUCTION: Tympanomastoid surgery, encompassing intricate middle ear and mastoid procedures, necessitates a standardized classification system to ensure consistent documentation, enhance comparability, and improve outcome analysis across different clinical settings. The SAMEO-ATO classification, established by the International Otology Outcome Group (IOOG), provides a comprehensive framework that categorizes surgical interventions into key components, covering both mastoid operations (SAMEO) and middle ear surgeries (ATO). METHODS: We analyzed correlations between specific subgroups within

Decompression of the geniculate ganglion and labyrinthine segments of the facial nerve through a middle cranial fossa approach using an ultrasonic surgical system: an anatomic study.

PURPOSE: The aim of this study is to evaluate the feasibility and the safety of a novel, alternative method for bone tissue management in facial nerve decompression by a middle cranial fossa approach. Several applications of Piezosurgery technology have been described, and the technique has recently been extended to otologic surgery. The piezoelectric device is a bone dissector which, using micro-vibration, preserves the anatomic integrity of soft tissue thanks to a selective action on mineralized tissue. METHODS: An anatomic dissection study was conducted on fresh-frozen adult cadaveric head

Nuances of otoplasty: a comprehensive review of the past 20 years.

Otoplasty for the correction of the prominent ear is a heavily debated topic in Facial Plastic Surgery. This article presents the past 20 years of literature on the topic in a concise and organized manner. The greatest area of focus is on the finer nuances between cartilage-sparing and cartilage-incising techniques. In addition, some of the latest research on anesthesia techniques, nonoperative approaches, and social issues are discussed.

More Than Meets the Ear: Incidental Cerebrospinal Fluid Leak During Myringotomy in a Child With Common Cavity Deformity.

Cerebrospinal fluid (CSF) otorrhea represents a breach of the temporal bone separating the subarachnoid space from the middle ear and carries a significant risk of meningitis. While most cases are associated with trauma or prior skull base surgery, congenital fistulas may remain occult until unmasked by otologic procedures.  We report the case of a five-year-old male with bilateral profound sensorineural hearing loss and previously unknown labyrinthine anomalies who developed high-flow, pulsatile clear otorrhea during routine left ear myringotomy for recurrent otitis media. Immediate concern f

Transoral Robotic Cleft Palate Surgery: Communication-Related Outcomes and Feasibility.

Treatment of cleft lip alveolus and/or palate includes surgical repair to treat communication-related outcomes such as velopharyngeal insufficiency and otological dysfunction. Robot-assisted surgery has recently evolved into a promising adjunct to conventional surgery, particularly for complex procedures such as transoral (reconstructive) surgery. This structured literature review aims to investigate whether robot-assisted transoral cleft palate repair enhances communication (i.e., speech and otological) outcomes compared to conventional manual cleft palate surgery. A literature search was per

Pig as a large animal model for posterior fossa surgery in oto-neurosurgery: A cadaveric study.

This study proposes a practical model for a new approach to the posterior fossa in common domestic pigs. Several surgical procedures can be simulated in the nonliving pig model, including soft tissue dissection, drilling of temporal bone, dural incision, access to the cerebellopontine angle, exposure of cranial nerves and drilling of the internal auditory canal. The pig model perfectly simulates standard otological and neurosurgical procedures, and we highlight the feasibility of our approach for further experiments in a living pig model with the possibility of reproducing the model for resear

Effects of Tranexamic Acid on Intraoperative Bleeding and Surgical Field Visualization During Middle Ear Surgery: A Narrative Review.

Tranexamic acid is an antifibrinolytic agent widely used in several surgical procedures to reduce intraoperative bleeding. Intraoperative bleeding is a crucial problem for the ear surgeon, as it prevents good visualization of the surgical field. The aim of this work was to analyze the relevant literature about the use of tranexamic acid in ear surgery. A literature search was conducted in agreement with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) 2020 statement, across 3 databases (Medline, Cochrane, and Google Scholar), with the terms "tranexamic acid," and

Postauricular advancement fascio-cutaneo-periosteal flap for closure of mastoid cutaneous fistula.

BACKGROUND: Postauricular cutaneous mastoid fistula (PCMF) is a rare complication of chronic suppurative otitis media, may also complicate ear surgery and, very rarely, has been reported to complicate congenital cholesteatoma. Few authors have given descriptions of techniques of closure, with majority agreeing on the difficulty in managing such fistula due to the necrotic nature of the margin. SETTING: A tertiary care urban referral hospital in a developing economy. METHODOLOGY: A new technique of closure of PCMF is described. This technique utilizes the postauricular fascio-cutaneo-perioste

References in the history of otology: the importance of being earnest.

BACKGROUND: Historical vignettes are often included in scientific otological articles. However, they may contain inaccuracies and errors when authors use secondary or compiled texts rather than primary source material. METHODS: Examples of errors arising from the use of secondary sources are obtained from compiled works in comparison with original publications, with a particular focus on Hildanus' speculum. RESULTS: Different types of inaccuracies may be found in historical vignettes. In Hildanus' works, two different specula are depicted: one was described by himself during his lifetime, an

[Evaluation of surgical techniques used in treating microtia for 35 years].

The author presents an evaluation of the results of surgical treatment in 346 patients with microtia, treated in the previous 35 years, by using different materials to reconstruct the auricle. The reasons for giving-up the application of xenogenic and allogenic cartilage as well as polyester-polypropylene implants to reconstruct the auricular skeleton are explained. A 17-year experience of using autogenic costal-cartilage to reconstruct the auricular skeleton helps to state that early and late complications have been reduced and satisfactory esthetic results have been obtained.
