External auditory canal cholesteatoma presenting with facial nerve palsy: rare clinical entity.

INTRODUCTION AND IMPORTANCE: External auditory canal cholesteatoma (EACC) is a rare, non-neoplastic lesion representing 0.1%-0.5% of otologic cases. Despite its rarity, EACC can be highly destructive, similar to middle ear cholesteatoma. Owing to its proximity to vital structures such as the facial nerve, severe complications, including facial paralysis, may occur, although such presentations remain exceptionally uncommon and scarcely reported. CASE PRESENTATION: We report the case of a 45-year-old male patient who presented to our ENT department with right-sided facial paralysis. Otoscopic e

Treatment of acute otitis media in children under 2 years of age.

BACKGROUND: Recommendations vary regarding immediate antimicrobial treatment versus watchful waiting for children younger than 2 years of age with acute otitis media. METHODS: We randomly assigned 291 children 6 to 23 months of age, with acute otitis media diagnosed with the use of stringent criteria, to receive amoxicillin-clavulanate or placebo for 10 days. We measured symptomatic response and rates of clinical failure. RESULTS: Among the children who received amoxicillin-clavulanate, 35% had initial resolution of symptoms by day 2, 61% by day 4, and 80% by day 7; among children who receiv

Reliability and validity of industrial audiometry: implications for hearing conservation program design.

Hearing conservation in industry relies heavily on monitoring audiometry to detect early noise-induced hearing loss in workers who are exposed to potentially damaging noise, with or without hearing protectors. The "real-world" reliability and validity of these measurements, as well as otoscopic observations in industry, have not been extensively investigated. In addition, there is considerable controversy over the selection of a definition of "significant threshold shift" in industrial audiometry. These and related issues were considered in a series of three studies utilizing data from an acti

Traumatic Acute Brain Herniation through the Ear in a Child: Concealed compound fracture.

A seven-year-old girl presented to Sultan Qaboos University Hospital, Oman, with a history of having been hit by a motor vehicle. After this, she had right-sided cerebrospinal fluid otorrhoea, and a week later, brain matter extruded through the right ear. A computed tomography scan of the brain demonstrated a tegmen fracture communicating with the external auditory canal. There was no hearing or facial nerve impairment and an otoscopic examination showed an intact tympanic membrane. She underwent a transcranial repair of the middle cranial fossa base, which revealed a wide dural and bony defec

Carcinoid tumor of the middle ear.

The patient was a 32 year-old man who was first seen in 2003 for 2 years hearing loss. On otoscopic examination, a whitish mass could be observed through the intact tympanic membrane. High resolution computed tomography showed a tumor like lesion in the middle ear with no evidence of bone destruction. A tympanomastoidectomy was performed. Light microscopy showed fragments of cellular tissue in which both glandular and trabecular growth pattern could be identified Immunohistochemical examinations showed positive staining of the tumor cells for cytokeratin and chromogranin. The diagnosis of carc

[Sudden hearing loss].

Sudden hearing loss is a reduction of sensoneurinal sound perception with a sudden onset. It is usually occurring only in one ear and without a recognizable cause. If a hearing loss is suspicioned due to otoscopic and tuning fork test findings, the patient should rapidly be sent to an ENT-specialist because of the possible consequences. After the first examinations by means of ear microscopy and audiogram further diagnostics may be necessary. If no cause is found, the idiopathic sensorineural hearing loss remains as the exclusion diagnosis. Therapy follows a polypragmatic approach based on the

A Rare Ectopic Localization of Hidradenoma Papilliferum in the External Auditory Canal.

We describe the case of a 57-year-old man with a 2-year history of intermittent left-sided ear fullness and recurrent cerumen impaction. Otoscopic examination revealed a soft, flesh-colored pedunculated mass on the posterior wall of the left external auditory canal (EAC). Temporal bone computed tomography demonstrated a 6 mm pseudonodular lesion with minimal underlying osteolysis. The patient underwent surgical excision under local anesthesia, and histopathological evaluation confirmed the diagnosis of hidradenoma papilliferum (HP) with no evidence of malignancy. This case highlights the impor

Chronic Meningitis Due to Otitis Media With Cholesteatoma in a Five-Year-Old Child With a Cochlear Implant.

Cochlear implants (CIs) restore hearing in patients with severe-to-profound deafness. Post-CI meningitis is a rare but redoubted complication. We present the case of a five-year-old CI recipient who experienced an episode of chronic meningitis caused by chronic otitis media with cholesteatoma encasing the electrode lead. We hypothesize that the cholesteatoma led to an ascending infection to the cochlea, passing through the labyrinths, resulting in chronic meningitis. Although positive neural responses were initially noted on cochlear electrical stimulation, these responses resolved a few weeks

An Uncommon Presentation of Human Otoacariasis by Haemaphysalis longicornis.

Ticks are obligate parasites on animals and sometimes humans. They usually suck the blood of the hosts and can carry various infectious diseases as a vector. Otoacariasis is the presence of ticks and mites within the ear canal and relatively common in domestic and wild animals. However, tick infestations of human ear canals are rarely reported in the scientific literature and hardly occur in developed countries. Herein, we report a rare case of otoaracariasis involving Haemaphysalis longicornis . A 9-year-old girl living in a suburb presented with otalgia of left ear for 1 day. Otoscopic exami

A potentially catastrophic anatomical variation: aberrant internal carotid artery in the middle ear cavity.

Aberrant internal carotid artery (ICA) is a rare but a very important vascular anomaly of temporal bone. Misdiagnosis of the anomaly may lead to massive hemorrhage and severe complications during otologic procedures. It is essential to keep this anomaly in mind for any otologic surgeon to prevent catastrophic complications. We present a case of aberrant ICA appeared as a nonpulsatile middle ear mass. The patient had a complaint of hearing loss, and the otoscopic examination of the patient revealed a tympanic membrane perforation and a blue-reddish retrotympanic mass. Multidetector computed tom

An unusual dark pigmentation on the tympanic membrane.

OBJECTIVE: To report an extremely rare case of dark pigmentation on the tympanic membrane due to alkaptonuria, and to discuss the probable association between this condition and hearing loss. CASE REPORT: A 58-year-old man with alkaptonuria was admitted with tinnitus and hearing loss in both ears. Physical examination showed bluish-black pigmentation on the helixes of both ears and both sclerae. Otoscopic examination revealed dark discolouration of both tympanic membranes. Audiological evaluation revealed mixed high frequency hearing loss in both ears. Tympanometric examination revealed type

Otomycosis: Diagnosis and treatment.

Aspergillus and Candida spp are the most frequently isolated fungi in patients with otomycosis. The diagnosis of otitis externa relies on the patient's history, otoscopic examination under microscopic control, and imaging studies. Direct preparation of the specimens, particularly with optical brighteners, mycologic culture, and histologic examination, is very important and strongly recommended for the correct diagnosis. Patients with noninvasive fungal otitis externa should be treated with intense débridement and cleansing, and topical antifungals. Topical antifungals, such as clotrimazole, mi

Cochlear implant infection due to cholesteatoma.

There are an increasing number of young children undergoing cochlear implantation. Cholesteatoma is a serious, although rare, long-term complication. We present the only documented case in which cholesteatoma has presented with late abscess formation in the bed of a cochlear implant package of a child. Cholesteatoma may be an iatrogenic complication of implantation surgery. Damage to the bony part of the external auditory canal should be avoided. The successful two-staged surgical treatment using explantation, debridement, gentamycin bead insertion, cholesteatoma excision and modified radical

Limits of normal for pressure sensitivity in the fistula test.

In patients with perilymphatic fistula (PLF), nystagmus may sometimes be elicited by application of pressure to the external ear canal. The extent to which the normal population also exhibits such 'pressure sensitivity' is presently unknown. Our goal was to determine the limits of normal pressure sensitivity and to quantify the performance of the fistula test. Our subjects consisted of 13 normal controls and 7 patients with a history of pressure sensitivity who later underwent exploratory tympanotomy. We measured nystagmus prior to and following pressurization of the external ear canal. Pressu

Traumatic separation of the annular cartilage from the external auditory meatus in a cat.

A 13-year-old cat was evaluated because of anorexia and otic discharge of 1 week's duration. A left head tilt and left facial nerve deficit were evident. Pink fleshy tissue was seen in the area where the tympanic membrane was expected during otoscopic examination. The left ear canal was laterally displaced from the skull on a rostrocaudal (open mouth) radiographic view, suggesting ear canal separation. During surgery, the annular cartilage was found to be separated from the external auditory meatus, and total ear canal ablation was performed. Complications did not develop, and facial nerve fun

Effect of naproxen, a prostaglandin inhibitor, on acute otitis media and persistence of middle ear effusion in children.

Prostaglandins are thought to be of importance in the pathophysiology of otitis media with effusion (OME), and the possibility of reducing the frequency and persistence of this condition by using prostaglandin inhibitors has been suggested. In a double blind manner, naproxen was administered to children with acute otitis media, in addition to amoxicillin, and its influence on the subsequent occurrence and persistence of middle ear effusion was evaluated. Eighty-one children participated in the study. No significant difference was found in the number of patients with tympanograms consistent wit

Septal deviation in newborn infants.

Authors report an experiment performed in a randomized group of newborns to evaluate the frequency of post-partum septal deviations. A number of 423 subjects out of 1142 have been examined in a 17-month period. Newborns have been examined with an otoscope and two different kinds of nasal lesions have been considered: (1) pyramid deformation with septum dislocation and columella deviation; (2) deviation and subluxation of the septum. A number of 315 newborns were naturally delivered and 108 by cesarean section. In the former group, 10 pyramid deformations and 12 septal deviations have been foun

Extended high-frequency hearing loss among Afro-Colombian adolescents from a rural area in Colombia: a cross-sectional study.

OBJECTIVES: Research trends concerning hearing loss within teen rural populations are limited and current evidence suggests that extended high-frequency audiometry can be a sensitive tool to detect subclinical hearing loss. Moreover, current research emphasises the importance of representing different ethnic populations in science. This study aimed to determine the prevalence of acquired hearing loss through conventional pure-tone (0.25-8 kHz) and extended high frequency (EHF) (9-20 kHz) audiometry in Afro-Colombian adolescents from a rural area in Colombia. DESIGN: Observational, cross-secti
