A 53-year-old man with treated, biopsy-proven Whipple's disease is described during the exacerbation of the disease, and several new findings not previously reported in association with this disease are presented. The unique combination of ocular signs included uveitis, glaucoma, epiphora, superficial punctate keratitis, and an unusual fibrovascular pannus involving the anterior chamber angles and corneal periphery of both eyes. These ocular abnormalities emphasize the need for thorough ophthalmologic evaluation of all patients with Whipple's disease.

This article focuses on hand and wrist deformities arising from rheumatoid arthritis. The pathogenesis is synovitis and pannus formation, leading to direct tissue invasion and mass effect resulting in stretching and attenuation of ligaments. The typical deformity of a rheumatoid wrist is a dorsal subluxation of the ulna head (or more aptly, volar translation of the radius given that the ulna is fixed in position), with volar subluxation, ulnar translocation and supination of the carpus, eventually leading to carpal collapse. For each of these deformities, the pathogenesis is discussed, along w…

A 19-year-old primigravida, with a history of mechanical mitral prosthesis at the age of six years following previous repair of an atrioventricular septal defect, presented at 26 weeks' gestation with cardiogenic shock secondary to obstruction of the mitral prosthesis. She underwent successful emergency redo mitral valve replacement and caesarean section. At operation the prosthesis was found to be virtually completely obstructed with pannus. Both mother and baby had an excellent outcome following surgery. The case report is presented with a brief review of the literature.

Incontinentia pigmenti achromians is a multisystem hereditary disorder characterized specifically by a whorled or streaked cutaneous hypopigmentation and frequently characterized by numerous neurologic, musculoskeletal, and ocular abnormalities. We present a patient with incontinentia pigmenti achromians in whom the ocular abnormalities included the commonly reported exotropia, myopia, small optic nerve, and hypopigmentation of the fundus, as well as rarely reported corneal asymmetry, pannus, and atropic irides with irregular pupillary margins. The patient also had a cataract in the right eye …

A 29-year-old man with rheumatic stenosis and insufficiency of the aortic, mitral, and tricuspid valves underwent urgent triple-valve replacement on January 12, 1966, because of uncontrollable congestive heart failure. Twenty-two years and 8 months later (September 13, 1988) reoperation was necessary to replace the Kay-Shiley tricuspid prosthesis because of progressive stenosis of the valve due to pannus formation. The aortic and mitral prostheses continue to function normally. The patient has again returned to full activity.

We studied the clinical and histopathologic ocular findings in four related males with a newly recognized syndrome consisting of microphthalmos, microencephaly, mental retardation, agenesis of the corpus callosum, hypospadius, and cryptorchidism with X-linked recessive inheritance. The ocular abnormalities include microphthalmos, corneal pannus and hypoplasia, cataracts, uveal hypoplasia, retinal dysplasia, optic nerve hypoplasia, and congenital blepharoptosis. In case 4, a female twin who died in utero (at 15 weeks' gestation) showed none of the ocular abnormalities.

INTRODUCTION AND IMPORTANCE: Morbid obesity can present with life-limiting panniculus morbidus, a severe excess of abdominal tissue. We describe the importance of a multidisciplinary, tailored surgical approach involving the resection of 30 kg of tissue to achieve both functional and psychological recovery, despite high perioperative risks.
CASE PRESENTATION: A 47-year-old man [body mass index (BMI) 61] presented with a pendulous pannus extending to the knees, leading to urinary and sexual dysfunction, recurrent skin infections, ulcerations, and inability to perform daily activities. He under…

Rheumatoid arthritis (RA) is an autoimmune disease primarily characterized by chronic, erosive polyarthritis. It is associated with a high rate of disability, and its pathogenesis remains incompletely understood. Uncontrolled chronic inflammation, synovial hyperplasia, Pannus formation, and bone destruction in RA patients remain the core challenges facing current clinical treatment, and the inflammatory response is generally considered the initiating factor for this series of pathological processes. In an inflammatory environment, the body's metabolic rate accelerates, leading to increased loc…

Objective: Our aim was to present a rare case of a middle-aged male patient, diagnosed with Graves' orbitopathy, which had an atypical rapid unilateral onset. Initially, the left eye presented exophthalmos, eyelid retraction, corneal ulceration, and pannus formation with an important vascular component due to corneal exposure. The same symptoms developed in the right eye within a short period of time. Methods: A 52-year-old man presented in our department with bilateral proptosis, decrease in visual acuity, and orbital pain, which developed initially in the left eye seven months before the rig…

BACKGROUND: Porcine valves are used for mitral valve replacement, but the limited long-term durability has restricted the application in younger patients. Degenerated porcine mitral valves were explanted to analyze the failure modes and damage characteristics.
METHODS: Twelve porcine valves were explanted via secondary mitral valve replacement surgery. Microcomputed tomography scanning, morphological and pathological examinations were performed to classify the cusp tears, calcification, and pannus formation. The causes of valve deterioration were subsequently analyzed.
RESULTS: The mean age …

BACKGROUND: Atopic keratoconjunctivitis (AKC) is defined as a chronic keratoconjunctivitis associated with atopic dermatitis, but severe progressive keratoconjunctivitis as a sole manifestation of atopy also may occur. The authors report three patients with a longstanding history of chronic conjunctivitis with bilateral pannus formation. These patients denied any history of major atopy at the time of presentation.
METHODS: All patients were evaluated for collagen vascular disease and for evidence of atopy. Conjunctival biopsy was obtained in each case and was analyzed by light microscopy and …

The wrist is frequently involved in rheumatoid arthritis. The areas where the synovial membrane is best developed are the best areas for the development of the pannus as well. Proliferation of the synovial membrane lining the joint capsules leads to loss of ligamentary support of the wrist. This is the basis for further deformation. The direction of the carpal deformation is determined by normal anatomical conditions. Destruction of the elements that are essential for the stabilization of the wrist is responsible for these deformations; these elements are described. The flexor and extensor ten…

We report a patient with a congenital ichthyosiform eruption, sensorineural deafness, vascularizing keratitis and pannus formation, and hypotrichosis, who developed recalcitrant fungating candidal plaques on the skin. There was no family history of similar disease, or of consanguinity. The steroid sulphatase level in the keratin was within normal limits, and this finding excluded a diagnosis of X-linked recessive ichthyosis. Treatment with oral fluconazole for 14 weeks resulted in complete resolution of the fungating lesions, and there has been no evidence of recurrence during a 12-month follo…

An 11-year-old girl developed peripheral corneal thinning and superficial pannus formation with no other ocular or systemic signs. During the subsequent two years she developed inflammation of both ears, progressive collapse of the nasal cartilage, and polyarthritis. A clinical diagnosis of relapsing polychondritis was made. A spontaneous perforation of her right eye secondary to progression of peripheral ulcerative keratitis was successfully repaired. Treatment with dapsone, a nonsteroidal anti-inflammatory agent resulted in remission of her ocular and systemic disease.
