Terminal Syme Amputation of the Great Toe in the Pediatric Population.

BACKGROUND: In the pediatric population, chronic ingrown toenails (onychocryptosis) can cause infection (paronychia), debilitating pain, and may be unresponsive to conservative treatments. Following multiple failed interventions, a terminal Syme amputation is one option for definitive treatment of chronic onychocryptosis. This procedure involves amputation of the distal aspect of the distal phalanx of the great toe with complete removal of the nail bed and germinal center, preventing further nail growth and recurrence. METHODS: A retrospective review was performed to determine outcomes of a t

Erlotinib in patients with advanced non-small-cell lung cancer: impact of dose reductions and a novel surrogate marker.

Erlotinib is a relatively well-tolerated treatment option for patients with advanced non-small-cell lung cancer (NSCLC). Some patients suffer from severe skin toxicity or diarrhea, making dose reductions or even treatment cessation necessary. Recent clinical trials usually defined a 100 mg daily dose as the lowest acceptable dose, whereas little is known about the efficacy with lower doses. We retrospectively reviewed the files of all patients with advanced non-small-cell lung cancer (NSCLC) treated with erlotinib. We assessed demographic, disease- and treatment-related information. We tried t

How well does the MESTT correlate with CTCAE scale for the grading of dermatological toxicities associated with oral tyrosine kinase inhibitors?

BACKGROUND: Dermatological toxicities associated with tyrosine kinase inhibitors (TKIs) are commonly graded using the National Cancer Institute Common Terminology Criteria for Adverse Events (NCI-CTCAE). A new tool has been proposed by the Multinational Association for Supportive Care in Cancer (MASCC), namely the MASCC EGFRI Skin Toxicity Tool (MESTT), as a class-specific grading system. This study was designed to assess the correlation between the severity grading of the dermatological toxicities associated with TKIs using the NCI-CTCAE v4.0 and the MESTT. PATIENT AND METHODS: One hundred p

Foot abnormalities in Canadian Aboriginal adolescents with Type 2 diabetes.

AIMS: To determine the profile of foot abnormalities in Canadian Aboriginal adolescents with Type 2 diabetes and the risk factors associated with these abnormalities. METHODS: Aboriginal adolescents with Type 2 diabetes underwent an interview, medical record review and foot examination in a tertiary care, paediatric hospital diabetes clinic and two geographically remote outreach clinics. The notes of 110 subjects were reviewed [mean age 15 +/- 3 years; mean duration of diabetes, 30 +/- 20 months; 71 (66%) female and 39 (34%) male] and 77 (70%) of the subjects were examined. RESULTS: Foot abn

Management of cutaneous side-effects of cetuximab therapy in patients with metastatic colorectal cancer.

BACKGROUND: Cetuximab is a chimeric human-murine monoclonal antibody against the epidermal growth factor receptor (EGFR). It has shown activities against multiple malignancies in clinical trials. EGFR-inhibitors (EGFRI) often cause skin toxicity, most frequently acneiform eruption. Xerosis, eczema, fissures, teleangiectases, nail changes and paronychia can be seen in some cases, rarely hyperpigmentation. MATERIALS AND METHODS: We reviewed local practice of skin toxicity management during treatment with cetuximab. From November 2005 to January 2007, 31 patients with metastatic colorectal cance

Fingertip Infections.

The fingertip is the most common site of infections in the hand, which frequently are encountered by surgeons, dermatologists, and emergency and primary providers. Their mismanagement may have serious consequences. This review discusses the unique anatomy of the volar fingertip pulp and perionychium and reviews pathophysiology and treatment of acute and chronic paronychia, including the decision for surgical versus medical management, choice of antibiotics, incisional techniques, and postincisional care. Felons and the evidence regarding their management are reviewed. Several infectious, rheum

Allergic contact dermatitis from acrylates in artificial nails.

Artificial nails are an increasingly popular cosmetic enhancement to the natural nail. Several forms are available, including sculptured nails, photobonded nails, and preformed nails. Reactions to artificial nails have included paronychia, onychodystrophies, and dermatitis at contact areas and at sites distant to the contactant. We present a patient who developed erythema and painful pruritic vesicles of the paronychial tissue several hours after the application of sculptured nails. A strong reaction of erythema and edema occurred at the site of methyl methacrylate and ethylene glycol dimethac

Nail destruction in pemphigus vulgaris.

Involvement of the nails in pemphigus vulgaris (PV) is rare and is usually seen when the disease is severe. The most common clinical manifestations are chronic paronychia and onychomadesis. Finger nails are more frequently involved than toe nails. We report a case of severe and persistent PV in which an exacerbation was preceded by an erosive and destructive lesion of the right hallux nail unit. A nail bed biopsy was performed to rule out a subungual tumour. The image of suprabasal acantholysis yielded the diagnosis of PV. This localisation and clinical manifestation of PV have only been repor

[Green nail syndrome or chloronychia].

"Green nails" or chloronychia is an infection mostly caused by Pseudomonas ueruginosa but also by other bacterial or fungal contamination. The clinical appearance consists in a typical triad: green discoloration of the nail plate associated with proximal chronic paronychia and disto-lateral onycholysis. Exposition to moist environment, microtraumatisms, oaychotillomania and associated nail diseases such as psoriasis may promote infection by Pseudomonas. Treatment consists in cutting of the detached nail plate, brushing the nail bed with a 2% sodium hypochlorite solution twice daily and evictio

[Nail localization of dermatoses].

The nail unit may reflect a dermatological disease by its own. Actually, its involvement may account for the existence of a skin disease that never showed before (i.e. lichen planus). But since the nail apparatus has a limited repertoire of clinical expressions (hyperkeratosis, onycholysis, paronychia...), the diagnosis may be sometimes difficult even for the most trained physician. Nail involvement may be however an additional clue in the diagnosis of an atypical skin disease. In that case, it might help in the final correct diagnosis provided that the physician did not forget that clinical e

[Therapeutic assessment and side-effects of the aromatic retinoid on the nail apparatus].

Therapeutic benefits from aromatic retinoid have been described in the treatment of a variety of dermatological disorders. Stress is given on some of them which coexist with ungual abnormalities and the results are reported. These diseases include psoriasis, acropustulosis, keratosis lichenoides chronica. Drug-induced modifications of the epidermal structures of the nail apparatus are emphasized: fragility with onychorrhexis and onychoschizia is the commonest finding. Onychomadesis and nail shedding can be seen. Onycholysis is rare. Painful paronychia which is sometimes accompanied by granulat

Lipodystrophy associated with protease inhibitors.

Lipodystrophies, characterized by reduction of subcutaneous fat over part or all of the body surface, are uncommon. Their causes are unknown. Recently, lipodystrophy has been reported in human immunodeficiency virus (HIV)-infected patients taking protease inhibitors, which have been recommended since 1996 as standard therapy for HIV disease in combination with nucleoside analogues. In these cases, lipodystrophy consists of an association of peripheral lipoatrophy with central adiposity. We report four HIV-infected men on protease inhibitors who developed a disfiguring lipodystrophy. In three o

Nail changes in pemphigus: a systematic review and meta-analysis.

Pemphigus is a group of autoimmune mucocutaneous bullous disorders characterized by acantholysis resulting from autoantibodies targeting epithelial cell surface antigens. Studies reflect the presence of nail manifestations in some patients and suggest a potential correlation with clinical severity. This study examines the overall prevalence and characterizes the diverse manifestations of nail changes in pemphigus. We searched Cochrane, MEDLINE, EMBASE, and LILACS from 1990 to June 26, 2023 for studies reporting different nail changes in pemphigus patients. Data were collected and pooled to obt

Ungual hyalohyphomycosis caused by Fusarium proliferatum in an immunocompetent patient.

The patient was a 73-year-old healthy female farmer who had been treated with terbinafine for 25.5 months by a primary physician. She exhibited a discoloration and thickening of the right big toenail. She had no concomitant paronychia. Direct microscopy revealed chlamydoconidia and hyphae, and periodic acid-Schiff stained nail specimen showed septate hyphae. On the basis of these morphological features and gene analysis, the final diagnosis was ungual hyalohyphomycosis caused by Fusarium proliferatum. Topical application of 10% efinaconazole solution cured the disease in 10 months.

Hand infections.

Hand infections are very common and must be properly diagnosed and treated to minimize the potentially devastating functional complications. Acute hand infections include felon, paronychia, deep-space infections, septic arthritis, osteomyelitis, and necrotizing fasciitis. Chronic infections are usually caused by atypical mycobacteria and fungi. There are several conditions that mimic acute hand infections, and it is important to recognize them so that the appropriate treatment can be instilled. To prevent all these complications and poor outcome, prompt diagnosis and early institution of appro

[Paroxysmal iatrogenic acne and the epidermal growth factor receptor inhibitors (EGFR)].

Paroxysmal drug-induced acne is a peculiar condition recognized for decades in dermatology. It can be induced by a variety of drugs. In recent years, the epidermal growth factor receptor inhibitors used in oncology have shown this type of adverse effects. The pathogenic mechanism is likely related to the EGF implication in the rupture of the microcomedo wall resulting in the perifollicular inflammatory reaction typical for acne. Other cutaneous adverse effects have been reported with anti-EGFR drugs including paronychia and xerosis. The dermatological treatments of these drug-induced disorders
