Diastolic Dyssynchrony Differences in Patients with Single Right Ventricles vs. Control Patients.

AIMS: Diastolic dyssynchrony is increasingly being recognized as another marker for possible adverse cardiac events. Minimal data exist in hypoplastic left heart syndrome (HLHS) patients. The goal of this study was to determine if there were differences in diastolic dyssynchrony in patients with HLHS vs. control patients. METHODS/RESULTS: Tissue Doppler imaging (TDI) and strain rate (SR) analysis of the right ventricle in HLHS and control patients were performed. Time interval from onset of QRS complex on electrocardiography to peak TDI e' wave was obtained. Differences in intervals were calc

The right measure for the right question: A framework for domain-matched patient-reported outcome measure selection in knee arthroplasty research.

Patient-reported outcome measures (PROMs) are one of the primary tools for evaluating outcomes in knee arthroplasty research. Yet, they are frequently selected by convention rather than scientific rationale. The available instruments differ substantially in their measurement domains, spanning joint-specific symptoms and perception, physical function and activities of daily living, sport-specific and high-demand function, health-related quality of life, and patient satisfaction and expectation. When the domain of the chosen PROM does not correspond to the domain addressed by the research questi

The New Minimally Invasive Techniques for Pilonidal Sinus Disease Surgery: Choosing the Right Technique for the Right Patient-A Narrative Review.

Background and Objectives: Pilonidal sinus disease (PSD) is a common condition affecting predominantly young adults and it is associated with a substantial impact on quality of life. Despite the wide range of available surgical options, optimal management remains controversial due to heterogeneous recurrence rates, wound complications and prolonged recovery. Over the last decade, a paradigm shift has occurred from midline excision toward off-midline closure techniques and minimally invasive procedures. This narrative review aims to critically appraise contemporary surgical strategies for PSD,

Electrocardiographic characteristics in right ventricular vs biventricular pacing in patients with paced right bundle-branch block QRS pattern.

BACKGROUND: A paced right bundle-branch block (RBBB) QRS morphology is present in most patients with biventricular pacing (BiVP) but is also present in some patients with right ventricular pacing (RVP). The aim of this study was to determine if there are electrocardiographic characteristics that distinguish BiVP from RVP in patients with a paced RBBB QRS pattern. METHODS: Twelve-lead-paced electrocardiograms (ECGs) were analyzed from 356 consecutive patients (302 RVP and 54 BiVP). Further analyses were performed on those ECGs with a paced RBBB morphology, which included QRS pattern, axis, amp

The Right Atrium and Atrialised Right Ventricle are Associated with Cardiac Output During Exercise in Patients with Ebstein Anomaly of the Tricuspid Valve.

The right atrium (RA) and atrialised RV (aRV) may be important in determining exercise capacity in Ebstein anomaly (EA). We performed exercise testing and resting/exercise cardiac magnetic resonance imaging (CMRI) to examine associations between RA and aRV and left heart volumes, cardiac index and exercise capacity (%VO2max). In 13 un-operated EA patients (median age 13.5 years), indexed end-diastolic volumes were calculated from resting CMRI; right atrium (RAi), atrialised RV (aRVi), functional RV (fRVEDVi), left atrium (LAi) and left ventricle (LVEDVi). Systolic (SCRsys) and diastolic (SCRdi

Parameters of speckle tracking echocardiography in right atrial fibrosis of patients with rheumatic mitral stenosis: Focusing on right atrial stiffness.

BACKGROUND: In rheumatic mitral stenosis (RMS), left atrial pressure elevation can lead to right heart dysfunction and fibrosis. The relationship between speckle-tracking echocardiography (STE) right-heart parameters, right atrial fibrosis, and its histological confirmation, remains unclear. METHODS: 46 RMS patients (mean age of 53.0 ± 8.9 years, 67 % females) were enrolled. The exclusion criteria were the presence of moderate or severe aortic valve diseases, rheumatic tricuspid lesions and poor acoustic windows. RA tissue specimens were collected during the operation in RMS patients to analy

Double-Outlet Right Ventricle With Intact Ventricular Septum and Left Atrioventricular Valve Regurgitation in a Patient With Right Atrial Isomerism.

We present a very rare case of right atrial isomerism, double-outlet right ventricle, and incomplete atrioventricular septal defect (intact ventricular septum). In the neonatal period, the right ventricle was compressed by a "blind-ended" left ventricle with mild-to-moderate left atrioventricular valve regurgitation. The regurgitation gradually decreased from mild-to-moderate to mild with body weight gain. The patient underwent systemic-to-pulmonary shunt at three months of age and bilateral bidirectional Glenn at eight months of age. Although the echocardiogram demonstrated that the right ven

Successful ablation of a right epicardial accessory pathway via the right ventricular diverticulum in a patient with Wolff-Parkinson-White syndrome.

INTRODUCTION: We describe one rare case of successful ablation of a right epicardial accessory pathway (AP) via the right ventricular diverticulum in a patient with Wolff-Parkinson-White syndrome. METHODS: A 42-year-old woman was referred to the hospital for a catheter ablation of Wolf-Parkinson-White syndrome. The earliest activation was shown to be present in the region of the tricuspid annulus. However, ablation had no effect on the AP. RESULTS: We decided to do a selected angiography, in which a big diverticulum near the right tricuspid annulus was shown to be present. Ablation in this r

Pulmonary hypertension and right ventricular dysfunction in patients with left to right shunt coronary artery fistula: evaluation with cardiac CT.

In this study, we aimed to evaluate whether patients with left to right shunt coronary artery fistula (LRSCAF) are predisposed to developing pulmonary hypertension and right ventricular dysfunction compared with healthy individuals. The value of cardiac CT findings in determining the necessity of intervention for these patients was investigated. We retrospectively studied 19 patients with LRSCAF and 19 healthy patients. Several parameters were observed on cardiac CT by two radiologists, including pulmonary trunk diameter (PA diameter), right ventricular diameter (RVD), left ventricular diamete

Effect of right ventricular outflow tract obstruction on right ventricular volumes and exercise capacity in patients with repaired tetralogy of fallot.

Patients with tetralogy of Fallot and combined right ventricular outflow tract obstruction (RVOTO) and pulmonary regurgitation (PR) have a less dilated right ventricular (RV) and better RV function compared with patients without RVOTO. It is not known whether RVOTO is associated with improved exercise capacity. We compared cardiac magnetic resonance imaging, echocardiography, and exercise tests in 12 patients with RVOTO (Doppler peak RVOT gradient ≥30 mm Hg) and 30 patients without RVOTO. RV end-systolic and end-diastolic volumes were smaller in patients with RVOTO compared with patients witho

Abnormal right atrial and right ventricular diastolic function relate to impaired clinical condition in patients operated for tetralogy of Fallot.

BACKGROUND: Atrial enlargement may reflect ventricular diastolic dysfunction. Although patients with tetralogy of Fallot (TOF) have been studied extensively, little is known about atrial size and function. We assessed bi-atrial size and function in patients after TOF repair, and related them to biventricular systolic and diastolic function, and clinical parameters. METHODS: 51 Patients (21 ± 8 years) and 30 healthy controls (31 ± 7 years) were included and underwent magnetic resonance imaging to assess bi-atrial and biventricular size, systolic and diastolic function. Patients also underwent

Right ventricular peak systolic longitudinal strain is a sensitive marker for right ventricular deterioration in adult patients with tetralogy of Fallot.

The aim of this study was to evaluate the feasibility of right ventricular (RV) longitudinal peak systolic strain (LPSS) assessment for the follow-up of adult patients with corrected tetralogy of Fallot (TOF). Adult patients (n = 18) with corrected TOF underwent echocardiography and CMR twice with a time interval of 4.2 +/- 1.7 years. RV performance was derived from CMR, and included RV volumes and ejection fraction (EF). LPSS was calculated globally (GLPSS) and in the RV free wall (LPSS FW), with echocardiographic speckle-tracking strain-analysis. Baseline (G)LPSS values were compared between
