Falling perinatal mortality in twins in the UK: organisational success or chance?

In June 2018, Mothers and Babies Reducing Risks through Audits and Confidential Enquiries across the UK (MBRRACE-UK) published a Perinatal Surveillance report of an audit between 2013-2016. This noted that the stillbirth rate for twins nearly halved between 2014-2016; whereas the stillbirth rate for singletons remained static. There was a statistically significant reduction in the rate of stillbirth in twins over this period from 11.07 (95% CI, 9.78-12.47) to 6.16 (95% CI, 5.20-7.24) per 1000 total births. This commentary discusses these observations, the effects of twin chorionicity, and the

Time trends and epidemiological patterns of perinatal lamb mortality in Norway.

BACKGROUND: Perinatal mortality is a major cause of loss in the sheep industry. Our aim was to explore time trends in crude population stillbirth and neonatal mortality rates in Norway. We used data on 6,435,715 lambs from flocks enrolled in the Norwegian Sheep Recording System (NSRS) from 2000 through 2010 for descriptive analysis of trends. Longitudinal patterns of mortality rates were compared for lambs within different levels of variables suspected to be associated with perinatal loss. RESULTS: There was an approximately linear increase in the annual proportion of stillborn lambs during t

Institutional deliveries and perinatal and neonatal mortality in Southern and Central India.

BACKGROUND: Skilled birth attendance and institutional delivery have been advocated for reducing maternal, perinatal and neonatal mortality (PMR and NMR). India has successfully implemented various strategies to promote skilled attendance and incentivize institutional deliveries in the last 5 years. OBJECTIVES: The study evaluates the trends in institutional delivery, PMR, NMR, and their risk factors in two Eunice Kennedy Shriver NICHD Global Network for Women's and Children's Health Research sites, in Belgaum and Nagpur, India, between January 2010 and December 2013. Perinatal mortality in urban and rural areas in Poland in 2002-2012.

AIM: The aim of this study was to analyze the level and trends of perinatal mortality by mother's place of residence (urban vs rural area) in Poland in 2002-2012. MATERIAL AND METHOD: This study was based on the data of the Central Statistical Office on the number of live births, infant deaths (0-6 days) and stillbirths by mother's place of residence (urban vs rural area), reported in 2002-2012 in 16 provinces and Poland in general. Joinpoint model was used to analyze perinatal mortality rate trends over time and average annual percent change (APC). Urban/rural ratio was employed to demonstra

A meta-analysis of selected maternal and fetal factors for perinatal mortality.

BACKGROUND: In several developing countries, achieving Millennium Development Goal 4 is still off track. Multiple maternal and fetal risk factors were inconsistently attributed to the high perinatal mortality in developing countries. However, there was no meta-analysis that assessed the pooled effect of these factors on perinatal mortality. The purpose of this meta-analysis was to identify maternal and fetal factors predicting perinatal mortality. METHODS: In this meta-analysis, we included 23 studies that assessed perinatal mortality in relation to antenatal care, parity, mode of delivery, g

A prospective study of twinning and perinatal mortality in urban Guinea-Bissau.

BACKGROUND: Despite twinning being common in Africa, few prospective twin studies have been conducted. We studied twinning rate, perinatal mortality and the clinical characteristics of newborn twins in urban Guinea-Bissau. METHODS: The study was conducted at the Bandim Health Project (BHP), a health and demographic surveillance site in Bissau, the capital of Guinea-Bissau. The cohort included all newborn twins delivered at the National Hospital Simão Mendes and in the BHP study area during the period September 2009 to August 2011 as well as singleton controls from the BHP study area. [Perinatal mortality of fetuses and newborns in Poland in the years 1999-2008].

BACKGROUND: Procreation and perinatal health which includes maternal health, fetal growth and well being of newborns are a challenge for modern medicine. One of the indices of evaluation of perinatal health is perinatal mortality of fetuses and newborns. This has been emphasised in the National Health Programme in Poland for 2007-2015. AIM: Analysis of perinatal mortality in the period 1999-2008 of fetuses, newborn and stillborn, with MATERIAL AND METHODS: The analysis was based on data from the Central Statistical Office, from the Ministry of Health dokument MZ-29 and on data obtained from

[Impact of maternal antecedents on neonatal mortality in a regional perinatal hospital].

OBJECTIVE: Determine the prevalence of maternal risk factors and evaluate their impact on neonatal mortality in a regional perinatal center. MATERIALS AND METHODS: A cohort of 25,365 live newborns was studied between January 1st 2000 and December 31st 2004. Maternal antecedents were registered in a data base: sociodemographic; medical history; obstetric antecedents of previous pregnancies; as well as evolution of current pregnancy and birth. Newborn birth weight, gestational age and condition at discharge were registered too. Neonates who died were considered cases and controls those discharg

Determinants of perinatal mortality and serious neonatal morbidity in the second twin.

OBJECTIVE: To identify potential determinants of perinatal mortality and neonatal morbidity among second twins relative to first twins. METHODS: A retrospective cohort design was used to study twin deliveries in Nova Scotia from 1988 to 2002. Monoamniotic or conjoined twins and twin pairs with major congenital anomaly or antepartum fetal death of either twin were excluded. The primary outcome was a composite measure of perinatal mortality and neonatal morbidity, including birth asphyxia, respiratory distress, neonatal trauma, and infection. Risk of adverse outcome of second twins relative to

Perinatal mortality in first- and second-born twins in the United States.

OBJECTIVE: To evaluate the prevailing mortality paradox that second-born twins are at higher risk of perinatal mortality than first-born twins. METHODS: We used the 1995-1997 United States "matched multiple birth" data files assembled by the National Center for Health Statistics, for analysis of risk of perinatal mortality in first- and second-born twins (293788 fetuses). Perinatal mortality was defined to include stillbirths after 20 weeks of gestation and neonatal deaths (deaths within the first 28 days). Gestational age-specific risk of perinatal mortality (per 1000 total births), stillbir

Clinico-pathological study of causes of perinatal mortality in a developing country.

Perinatal mortality rates are an important indicator of the overall obstetric and neonatal services available, and the socio-economic status of the community. These rates are also used to identify shortcomings in services provided. The aim of this study was to identify clinical causative factors of perinatal deaths (supported by post mortem evidence where possible). This was a prospective descriptive study. The total number of deliveries and perinatal deaths over a 6-month period were recorded and clinico-demographic data noted. Post mortem examination information, if available, was also recor

[Maternal and perinatal morbidity-mortality in pregnant women with renal transplant].

The outcome of 32 pregnancies in renal allograft recipients is reported. The mean age at the time of conception was 27.3 years (range, 20 to 37) with an average interval of 47 months from the time of transplantation to conception (range, 2 to 163). Twenty-nine patients received the graft from a living related donor, one from a living no related donor an 2 from cadaver donors. All patients continued their immunosuppressive regimen during pregnancy and only 6 patients were taking cyclosporine A. Hypertension during pregnancy was observed in 10 patients (31%), superimposed preeclampsia in 4 (14%)

[Obstetric care and perinatal mortality in a rural area of northeastern Brazil].

This paper examines deliveries in a rural community of northeastern Brazil served by midwives who had received training that included the referral of pregnancies with complications; such training is essential to improve obstetrical care in rural areas of less developed countries. Data were collected on 1,661 women, of whom 62% gave birth in their homes and 38% in hospitals. The women referred to hospital were more likely to be primigravidas, to experience complications during labor, or to have had a stillbirth in the past. The parturients who were sent straight to hospital were more likely to

The influence of in-utero transfer on perinatal mortality in a tertiary care centre.

From March 1, 1986 through February 28, 1989 inclusive there was a total of 8,319 births with a birth-weight of 500 g or more at Royal North Shore Hospital (RNS). Three hundred and sixty one births (4.3%) resulted from in-utero transfer of high risk pregnancies (IUT); the remainder were booked at RNS. There were 141 perinatal deaths of which 55 (39%) occurred in infants transferred in-utero. For the whole population delivered at RNS the perinatal mortality rate was 17.0/1,000 births (10.8/1,000 for booked patients versus 152.4/1,000 for IUT births), the stillbirth rate was 7.1/1,000 births (5.

Trends in perinatal, neonatal and postneonatal mortality in Italy, 1955-84.

Trends in stillbirth rates, perinatal, neonatal and postneonatal mortality in Italy over the period 1955-84 were analyzed. There was a 75% reduction (from 28.4 to 7.1/1000 births) in stillbirth rates, and a nearly 70% fall (from 46.2 to 14.5/1000 births) in overall perinatal mortality (from the 180th day of pregnancy to the first week of life). Further, mortality rates from the 8th to the 28th day of life dropped from 7.4 to 1.6/1000 livebirths, and mortality from the second month to the first year of life from 25.1 to 2.2/1000 livebirths. The fall in stillbirth rates was similarly evident acr

Meningeal haemorrhage and congestion associated with the perinatal mortality of beef calves.

Thirty Angus, Hereford and Shorthorn term calves dying before, during or within 7 days of birth in a large beef herd were autopsied. Selected specimens were examined histologically. Two carcasses were classified in the category of ante-parturient death, 24 as parturient, and 4 were classified in the post-parturient death category. Three of the latter showed evidence of starvation. Eight of the calves in the parturient death category were assisted during delivery, and 16 were not assisted. The incidence of nulliparous dams among assisted parturient deaths was significantly higher than among the
