Hidradenitis suppurativa is a chronic relapsing infection of the apocrine sweat glands. Its association with penoscrotal lymphedema is not well recognized. A case of massive scrotal elephantiasis associated with chronic hidradenitis of the perineum and scrotum is described. A wide resection of the scrotal mass and perineum was performed, with reconstruction of the perineum and penis carried out using local skin flaps and split-thickness skin grafts. This one-stage treatment yielded an excellent cosmetic and functional outcome.

Since the first case of the disease reported by John Hunter in 1786, more than 113 cases have been reported in the English literature, and 13 cases have been reported in the Japanese literature. Straddle injury is the most common cause of genital injuries in children and is defined as an injury resulting from a blunt impact to the perineum due to a fall onto a hard object. This type of injury tends to affect the anterior perineum, and the perianal area is usually spared. Herein, we report a unique case of straddle injury that resulted in concomitant anorectal and posterior urethral injuries due to an atypical impact to the posterior perineum.

The management of Fournier's gangrene in pregnancy in a woman with chronic idiopathic neutropenia is described. We report here a case of malignant mesothelioma presenting as a perineal and intrascrotal mass. A 42-year-old Japanese male presented with an enlarging mass in the perineum and intrascrotum.
