Ofuji papuloerythroderma is a distinctive clinical entity of unknown etiology, which may occasionally be associated with B cell and T cell lymphoma or visceral malignancy. We report a case of papuloerythroderma associated with follicular mucinosis in mycosis fungoides (MF) that raises the possibility of papuloerythroderma as a form of prelymphomatous skin eruption. This specific papuloerythroderma responded well to the Re-PUVA treatment, which is a combination of etretinate and PUVA photochemotherapy.

Seventy-five liver biopsies were performed in 30 psoriatics and 5 patients with mycosis fungoides undergoing treatment with photochemotherapy with 8-methoxypsoralen and UVA light. All patients had pre- and post-PUVA biopsies, the latter after one year. Five psoriatics had a further biopsy after 2 1/2 years of treatment. No statistically significant differences were found between biopsies taken before and after treatment.

Therapy with oral psoralen photochemotherapy (PUVA) and oral beta-carotene was compared in 29 patients with polymorphous light eruption. Complete remission occurred in 90% (9/10) of those treated with PUVA and in 32% (6/19) treated with beta-carotene. In the six patients who were treated in consecutive years with both modalities all had remissions with PUVA while only two had remissions with beta-carotene.

In this review of current therapy for mycosis fungoides, the analytical data have been extrapolated from the literature. We have considered the various therapeutic modalities such as topical therapy, photochemotherapy, electron beam radiation therapy, systemic chemotherapy, the combined modalities, as well as the use of interferon and other alternative biological approaches. We conclude with a final section on comments and recommendations that may prove useful in the design of appropriate therapeutic strategies.

Systemic PUVA photochemotherapy is widely used for a variety of skin diseases. Administration of 8-methoxypsoralen in a dilute bathwater solution may be an effective alternative therapy with no systemic side-effects and requiring smaller amounts of UVA to induce the required phototoxic response. PUVA baths of the palms and soles are an interesting alternative for dermatoses confined to these sites. This article reviews the authors' experience with PUVA bath therapy. Indications for and applications of this technique are discussed.

An 83-year-old Caucasian man with cutaneous T-cell lymphoma developed an aggressive squamous cell carcinoma of the left forearm, which recurred and metastasized after Mohs micrographic surgery and systemic chemotherapy with cis-platin and 5-fluorouracil. He was treated with extracorporeal photopheresis, radiation therapy, PUVA photochemotherapy, and interferon therapy for cutaneous T-cell lymphoma. Aggressive squamous cell carcinoma can occur in the setting of extracorporeal photopheresis.

Report about a 44-year-old psoriatic female who developed recurrent bullous eruptions on her extremities during oral photochemotherapy. Ultrastructural analysis revealed an epidermolytic blister formation due to cytolytic alteration of infranuclear parts of basal cells. Pathogenetically, an initial subclinical phototoxic alteration of the epidermodermal coherence is assumed which may lead to a visible bulla by an additional mechanical trauma. There was no evidence for a subclinical bullous pemphigoid.

A 56-year-old female patient with generalized granuloma anulare was treated with bath-PUVA therapy with 8-methoxypsoralen. Her diabetes mellitus renal insufficiency excluded oral photochemotherapy. A series of 30 treatments resulted in complete disappearance of the lesions on the trunk and neck and significant improvement of those on the limbs. No side-effects were observed. Bath-PUVA therapy seems to have positive effects not only on epidermal, but also on dermal processes.

Eight patients with polymorphous light eruption (PLE) were treated with the 8-methoxypsoralen (0.6 mg/kg) and UV-A given in an ordinary PUVA cabin. Excellent or good results were observed in 4 and moderate in others remained patients. It is concluded that PUVA treatment is a safe form of therapy which exercises its beneficial effect in the patients with PLE. Various theories concerning the mode of action of photochemotherapy in PLE are discussed in this paper.

In a comparative study, 60 patients of psoriasis vulgaris were subjected to PUVASOL and PUVASOL with trimcinolone acetonide in two different groups of 30 patients each. Patients given photochemotherapy with 8 MOP and local application of trimacinolone acetonide showed marked imorovement (66.6% and 60%) after 4 and 8 weeks of therapy in comparison to 26.6% and 16.6% in the group where no topical corticosteroids were applied. None of the patients showed any adverse reaction to the treatment.

Polymorphonuclear leukocyte (PMN) and mononuclear cell (MNC) locomotion was investigated in patients with psoriasis vulgaris before and after treatment with oral photochemotherapy (PUVA) and was found to be significantly increased (PMN: p < 0.001; MNC: p < 0.002) when compared to healthy controls. No significant difference was observed in the locomotion of PMN or MNC before and after PUVA treatment. These results indicate that PMN and MNC locomotion is increased in psoriasis vulgaris and remains unaltered by PUVA treatment.

Although its etiology remains unknown, evidence has accumulated to support an autoimmune pathogenesis for alopecia areata. Our review summarizes the immunologic data and also examines the role of genetics, atopy, and psychologic stress in this disorder. Until etiology is better understood, treatments for alopecia areata are likely to remain palliative. Nevertheless, newer therapies such as photochemotherapy, topical immunotherapy, and perhaps systemic immunotherapy (e.g., inosiplex) offer new hope for patients with extensive disease.

A patient with acute generalized pustular psoriasis was successfully treated with a combination of oral cyclosporin (6 mg/kg per day) and photochemotherapy (PUVA). Although early inpatient treatment with weak topical steroids and PUVA produced initial improvement, the patient's clinical condition fluctuated, with the subsequent development of erythroderma. The addition of oral cyclosporin produced dramatic improvement within 1 week of its commencement. The patient remained in remission 12 months following cessation of therapy.

Drugs with side effects affecting vision are often used in the treatment of skin disorders. The study evaluated principal groups of medicines which may negatively influence vision and the eye itself. Antimalaric drugs may cause a number of disorders of vision and a diagnosed retinophaty is an absolute contraindication. Retinoids often cause a dry eye condition, which results in intolerance to wearing contact lenses. They can also be a cause of poor nightly vision. Psoralens, used in photochemotherapy, can penetrate to the frontal part of the eye, inducing clouding of the lens and cataract. Glu…

BACKGROUND: The use of an adequate initial dose of ultraviolet A (UVA) radiation for photochemotherapy is important to prevent burns secondary to overdosage, meanwhile avoiding a reduced clinical improvement and long-term risks secondary to underdosage. The ideal initial dose of UVA can be achieved based on the phototype and the minimal phototoxic dose (MPD). The objective measurement of constitutive skin color (colorimetry) is another method commonly used to quantify the erythematous skin reaction to ultraviolet radiation exposure. The aim of this study was to determine variations in MPD and …

Kaposi's varicelliform eruption, caused by herpes simplex type 1, developed in a patient with mycosis fungoides four days after initiation of photochemotherapy with methoxsalen plus long-wave ultraviolet light (PUVA). He had a history of recurrent localized herpes infection. Vidarabine (adenine arabinoside) was used as treatment. Rechallenge with PUVA did not provoke another herpes infection. Kaposi's varicelliform eruption is rare in mycosis fungoides. Vidarabine may prove to be useful in the treatment of widespread herpes simplex.

We present a case of a 17-year-old woman with vitiligo who had no known previous liver diseases nor any exposure to concurrent hepatotoxic drugs and who developed a liver injury while on topical methoxypsoralen photochemotherapy (PUVA). The reaction, manifested by elevated liver enzymes, was provoked on 3 consecutive occasions, and on the last one, with 1% methoxypsoralen lotion only. Although the exact mechanism of liver injury in our patient is unknown, it seems to be an idiosyncratic hypersensitivity reaction to topically applied methoxypsoralen.

Both topical nitrogen mustard and psoralen photochemotherapy may induce benign and malignant alterations in the skin. We describe the explosive appearance of multiple epidermal cysts and squamous cell carcinomas in a patient whose cutaneous T-cell lymphoma was treated sequentially with these two types of therapy. This is the first report of both processes in the same patient with cutaneous T-cell lymphoma. It strongly supports the concept of lesion induction, while raising the question of an additive or even synergistic effect of these types of therapy.

Treatment with 8-methoxypsoralen plus ultraviolet A radiation and extracorporeal photochemotherapy (photopheresis) are widely used for the treatment of psoriasis and other inflammatory skin diseases, graft-versus-host disease, and mycosis fungoides. As the ratio of Th1 and Th2 cells appears to be critical for pathogenesis and progression of these disorders the effect of psoralen plus ultraviolet A on Th1 and Th2 cytokine production by CD4+ lymphocytes was investigated. Human peripheral blood lymphocytes were incubated in the presence of anti-CD3, rh-IL2, and rh-IL4 for 48 h. After subsequent s…
