Myopathy induced by statin-ezetimibe combination: Evaluation of potential risk factors.

Although both atorvastatin and ezetimibe may cause myopathy, statin-induced myopathy is less likely at low doses, and ezetimibe is only rarely reported to induce myopathy. Also, ezetimibe is not usually known to potentiate statin-induced myopathy. We report a case of myalgia with elevated serum creatinine phosphokinase in a patient after 2 months of therapy with fixed dose combination of atorvastatin and ezetimibe (10 mg each). At the time of the event, patient was undertaking moderate physical exertion in the form of brisk walking for 30-40 min a day and was detected to have low serum Vitamin

Physiological responses that account for the increased power output in speed skating using klapskates.

The present study investigates which physiological sources support the increase in mechanical power output (W out) that can be obtained using klapskates in speed skating. It was hypothesized that the increase in W out could be achieved through an increase in gross efficiency or an increase in aerobic power (W aer). Six speed skaters performed a submaximal and maximal 1,600-m skating test with both klapskates and conventional skates, to measure gross efficiency and maximal W aer during speed skating. The rate of oxygen uptake (VO2) and post-exercise blood lactate concentrations ([La]) were meas

Self-perception of the ability to work in the cold.

Accumulating evidence of beneficial effects from physical exertion must be balanced against increased risk of cardiac arrest during performance. There is evidence that, by using such cues as heart rate, individuals can monitor their level of exertion perceptually. We undertook experiments to discover whether temperature and heart rate interact to affect self-perception when the effective temperature is moved downard from the comfort zone. In the first pilot study, 36 males practiced a new game, SwedeBall, for a period of 20 minutes. Twelve were randomly assigned to play at a temperature of 22

Training effects on blood zinc levels in humans.

Effects of physical training on fasting erythrocyte and plasma zinc distributions were studied on seven previously sedentary male students. The training consisted of running over 5 km, 6 times/week for 10 weeks. Maximum aerobic capacity (VO2max) and 12 min walk-run performance increased significantly (p less than 0.01) after training. The erythrocyte concentrations of total zinc and of zinc derived from carbonic anhydrase I (CA-I) rose significantly (p less than 0.05) after training, whereas no such effects were noted in CA-II-derived zinc, Cu2Zn2 superoxide dismutase-derived zinc, and other z

The effects of strenuous exercise on infection with Francisella tularensis in rats.

To investigate the effects of strenuous forced exercise on the course and complications of a bacterial infection and on myocardial responses and performance capacity, rats with tularemia (characterized by pyogranulomatous hepatic and splenic lesions) were exercised by swimming on days 0-6 of infection. Levels of glutamic oxaloacetic and pyruvic transaminases in plasma, densities of pyogranulomatous lesions, and bacterial counts in blood, liver, and spleen were similar in exercising and resting rats. Although a few exercising rats showed an unusual dissemination of infection, the antibody respo

Pacing, the missing piece of the puzzle to high-intensity interval training.

This study examined physiological and perceptual responses to matched work high-intensity interval training using all-out and 2 even-paced methodologies. 15 trained male cyclists performed 3 interval sessions of three 3-min efforts with 3 min of active recovery between efforts. The initial interval session was completed using all-out pacing, with the following 2 sessions being completed with computer- and athlete-controlled pacing in a randomised and semi-counterbalanced manner. Computer- and athlete-controlled intervals were completed at the mean power from the corresponding interval during t

Effect of 6-month athletic training on motor abilities in seven-year-old schoolgirls.

The effects of six-month athletic training on improving motor abilities in 7-year-old schoolgirls were assessed. Analysis of the results of 12 motor tests showed significant improvement in the study group (n = 38) in comparison with control group (n = 140) subjected to conventional physical education classes only. The improvement referred to the variables of aerobic endurance (3-min run), flexibility (forward bow), explosive strength (ball throwing and 20-m run), keeping balance (bench standing), static strength (bent arm hang), and repetitive strength (sit-ups). These are probably adaptive ch

Examining the effects of home modifications on perceptions of exertion and safety among people with mobility disabilities.

BACKGROUND: People with mobility disabilities often have reduced stamina and limited energy, making daily activities physically demanding. Home modifications, such as installing grab bars and optimizing the environment, have the potential to reduce exertion and enhance safety in the home, enabling individuals to participate more in other activities. OBJECTIVE: The purpose of this study was to evaluate the effects of a home modification intervention on perceptions of exertion and safety among people with mobility disabilities. METHODS: The study utilized the Home Usability Program (HUP), whic

Physical Activity After Breast Cancer Surgery: Does Depression Make Exercise Feel More Effortful than It Actually Is?

BACKGROUND: Prior to treatment, breast cancer patients are less physically fit compared to peers; during cancer treatment, their fitness typically declines. Depressive symptoms are associated with reduced activity up to 5 years post-treatment, but research has not identified mechanisms linking depression and lower activity. The current study assessed relationships among breast cancer patients' depression and perceived exertion during exercise as well as heart rate, an objective indicator of exertion. METHODS: Participants were 106 breast cancer patients, stages I-IIIA, who completed surgery b

Progression through return-to-sport and return-to-academics guidelines for concussion management and recovery in collegiate student athletes: findings from the Ivy League-Big Ten Epidemiology of Concussion Study.

OBJECTIVE: To examine the progression of collegiate student athletes through five stages of a return-to-activity protocol following sport-related concussion (SRC). METHODS: In a multisite prospective cohort study, we identified the frequency of initial 24-48 hours physical and cognitive rest, and the sequence of (1) symptom resolution and return to (2) exertion activity, (3) limited sport, (4) full sport and (5) full academics. In resulting profiles we estimated the likelihood of return to full sport ≤14 days or prolonged >28 days and tested for variability based on timing of the stages. RES

Exertional dyspnea in older adults: the effects of mechanical unloading of the thorax.

BACKGROUND: Age-related changes in respiratory function that constrain ventilatory capacity could contribute to dyspnea on exertion (DOE) in older adults. However, whether mechanically unloading the thorax using an external cuirass device could reduce ventilatory constraints during exercise and decrease DOE in older adults is unknown. METHODS: All participants (n = 39) performed two 6 min constant-load cycling tests: one without the cuirass (CTL) and one with -20cmH2O of continuous negative pressure (CNP-20). We measured ratings of perceived breathlessness (RPB), the exercise ventilatory resp

What every intensivist should know about exertional heat stroke.

Exertional heat stroke (EHS) is a life-threatening condition caused by strenuous physical activity, leading to severe hyperthermia and central nervous system dysfunction. Unlike classic heat stroke, EHS can occur in young, healthy individuals and in temperate climates, when heat production exceeds the body's ability to dissipate heat. Rising global temperatures and increased participation in endurance events are contributing to a growing incidence of EHS. Early recognition and rapid cooling are essential to prevent multi-organ failure and death. Whole-body cold-water immersion is the gold stan

Sickle cell trait does not cause "sickle cell crisis" leading to exertion-related death: a systematic review.

Globally, an estimated 300 million individuals have sickle cell trait (SCT), the carrier state for sickle cell disease (SCD). Although SCD is associated with increased morbidity and shortened life span, SCT has a life span comparable with that of the general population. However, "sickle cell crisis" has been used as a cause of death for decedents with SCT in reports of exertion-related death in athletes, military personnel, and individuals in police custody. To appraise this practice, the American Society of Hematology convened an expert panel of hematologists and forensic pathologists to cond

Core temperature and mental status of two runners experiencing exertional heat stroke after a road race.

Prehospital management of exertional heat stroke (EHS) consists of monitoring rectal temperature (Tre) while aggressively cooling via cold water immersion. Recent recommendations suggest using central nervous system (CNS) dysfunction to determine cessation of cooling when Tre is not available. We examined cognitive responses of two runners with EHS after a road race. This comparison illustrates the need to use caution with this recommendation, as the manifestation of CNS responses in EHS patients can be unpredictable. The variables in both cases highlight the importance of avoiding using menta

Atypical Presentations of Exertional Collapse.

Exertional collapse associated with sickle trait (ECAST) is an underrecognized cause of exertional collapse triggered by complex mechanisms involving acidosis, rhabdomyolysis, and arrhythmias, all of which create sickling, vaso-occlusive problems, and ultimately end organ failure. Three cases are described in young athletes, as well as 12 other examples of ECAST noted in case reports and news sources. Exertional collapse associated with sickle trait can be differentiated from other common causes of collapse (exertional heat syndrome, acute cardiac events, and asthma) because it is a conscious
