The impact on postpartum care by telehealth: a retrospective cohort study.

BACKGROUND: During the COVID-19 pandemic, our institution turned to telehealth as the primary method of postpartum care delivery. OBJECTIVE: We aimed to determine the impact of telehealth on completion of postpartum care goals. STUDY DESIGN: In a single-center retrospective cohort study, we compared a 14-week period, March to June 2019, before implementation of telehealth, with the same calendar months after implementation during 2020. Patients with a postpartum visit scheduled at our institution during the study period were included. To demonstrate a 10% difference in attendance to the post

National Initiatives to Improve Systems for Postpartum Care.

Purpose To showcase several current national initiatives that focus on reducing maternal mortality and severe maternal morbidity and promote postpartum health and wellness for all women. Description Maternal injuries and deaths are a serious public health concern with tremendous impact on families, communities, and healthcare providers. Over the past two decades, it has become apparent that the timing of serious maternal complications has shifted, with more than half of deaths occurring in the immediate postpartum period up to 1 year following birth. Many of these reported deaths could have be

Factors affecting the duration of postnatal visits.

24 community midwives recorded details of 783 postnatal visits over a 2 week period. The findings provide a description of postnatal visiting practice in Lothian Region, Scotland. Analysis revealed that 10 factors were significantly associated with the duration of visits. These could be grouped as follows: time consuming procedures (physical examination of the mother or baby, PKU testing), administration-related (the number of other visits that day, the need to liaise with general practitioners or health visitors, whether the mother was already known to the midwife, the time kept waiting at th

Outcomes and perspectives of woman-newborn dyads following discharge from a quaternary maternity service in Australia: A cross-sectional survey.

BACKGROUND: The average postnatal stay for most Australian mothers is two days. Postnatal length of stay is dependent on various factors, su … rge' and 'Home now, but support missing'. CONCLUSION: Participants identified that improvements in postnatal care require more than extending in-hospital length of stay. Rather a more individualised woman-centred focus, in-home supported options, with flexibility in timing needed, especially for those following a complicated birth.

Inequity in maternal health-care services: evidence from home-based skilled-birth-attendant programmes in Bangladesh.

OBJECTIVE: To explore use-inequity in maternal health-care services in home-based skilled-birth-attendant (SBA) programme areas in Bangladesh. METHODS: Data from a community survey, conducted from February to May 2006, were analysed to examine inequities in use of SBAs, caesarean sections for deliveries and postnatal care services according to key socioeconomic factors. FINDINGS: Of 2164 deliveries, 35% had an SBA, 22.8% were in health facilities and 10.8% were by caesarean section. Rates of uptake of antenatal and postnatal care were 93% and 28%, respectively. There were substantial use-ine…

Conductive thermal mattress versus routine care to reduce neonatal hypothermia during transport among low-birthweight neonates: An experimental study with historical controls.

BACKGROUND: Neonatal hypothermia at delivery, during transport and in the postnatal wards is common, under-recognized and infrequently monitored with prevalence ranging from 32% to 85%. This study compared conductive thermal mattress to routine care for prevention of hypothermia among low-birthweight (LBW) neonates during transport. METHODS: From July 2015 to November 2016 (historical controls), all eligible LBW neonates (1500-2499 g) were transported from the labour room/operation theatre to the neonatal intensive care unit (NICU)/postnatal wards using routine care (towels, blankets, cap, mi

Rooming-in for new mother and infant versus separate care for increasing the duration of breastfeeding.

BACKGROUND: Mother-infant proximity and interactions after birth and during the early postpartum period are important for breast-milk production and breastfeeding success. Rooming-in and separate care are both traditional practices. Rooming-in involves keeping the mother and the baby together in the same room after birth for the duration of hospitalisation, whereas separate care is keeping the baby in the hospital nursery and the baby is either brought to the mother for breastfeeding or she walks to the nursery. OBJECTIVES: To assess the effect of mother-infant rooming-in versus separation on

A structured review and exploration of the healthcare costs associated with stillbirth and a subsequent pregnancy in England and Wales.

BACKGROUND: In contrast to other pregnancy complications the economic impact of stillbirth is poorly understood. We aimed to carry out a preliminary exploration of the healthcare costs of stillbirth from the time of pregnancy loss and the period afterwards; also to explore and include the impact of a previous stillbirth on the healthcare costs of the next pregnancy. METHODS: A structured review of the literature including cost studies and description of costs to health-care providers for care provided at the time of stillbirth and in a subsequent pregnancy. Costs in a subsequent pregnancy wer

The obstetric management of sickle cell disease.

Sickle cell disease (SCD) is the most common inherited disease worldwide and is associated with anaemia and intermittent severe pain. Pregnant women who are affected have increased maternal and fetal mortality and morbidity. In view of this obstetricians should have an awareness of this condition and its complications, and pregnancies in women with SCD should be managed by a multidisciplinary team with experience of high risk pregnancies. Ideally women should be seen preconceptually for optimisation of their SCD and partner screening. Antenatal care should include regular outpatient visits wit

Crying babies, tired mothers - challenges of the postnatal hospital stay: an interpretive phenomenological study.

BACKGROUND: According to an old Swiss proverb, "a new mother lazing in childbed is a blessing to her family". Today mothers rarely enjoy restful days after birth, but enter directly into the challenge of combining baby- and self-care. They often face a combination of infant crying and personal tiredness. Yet, routine postnatal care often lacks effective strategies to alleviate these challenges which can adversely affect family health. We explored how new mothers experience and handle postnatal infant crying and their own tiredness in the context of changing hospital care practices in Switzerla…

Association between prenatal depressive symptoms and receipt of recommended maternal and infant care postpartum.

BACKGROUND: Maternal mental health can impact health care access and utilization for both the birthing parent and infant. We examined the association between prenatal depressive symptoms (episodic and chronic) and receipt of the postpartum 6-week visit and infant vaccinations in the first year postpartum. METHODS: Postpartum individuals (N = 672) who attended Expect With Me group prenatal care in Nashville, Tennessee and Detroit, Michigan completed surveys during the second and third trimesters of pregnancy, as well as 6- and 12- months postpartum. We conducted multiple logistic regression to

ACOG Committee Opinion No. 736: Optimizing Postpartum Care.

The weeks following birth are a critical period for a woman and her infant, setting the stage for long-term health and well-being. To optimize the health of women and infants, postpartum care should become an ongoing process, rather than a single encounter, with services and support tailored to each woman's individual needs. It is recommended that all women have contact with their obstetrician-gynecologists or other obstetric care providers within the first 3 weeks postpartum. This initial assessment should be followed up with ongoing care as needed, concluding with a comprehensive postpartum

[Congenital cytomegalovirus infection: current status and future perspectives].

In recent years, infection with cytomegalovirus (CMV) was the subject of numerous studies, one side being the most frequent congenital infection in newborns and one that determines the increased incidence of sequelae distance (about one third of childhood deafness is due to CMV), and the other one of the most frequent and serious opportunistic infections in immunocompromised individuals. Of newborns with congenital infection, approximately 10% are symptomatic at birth. Of the remaining 90%, asymptomatic at birth, 10-15% will develop sequelae from a distance. Regarding the possibility of vertic
