Predicting dropout and non-response to psychotherapy for personality disorders: A study protocol focusing on therapist, patient, and the therapeutic relationship.

BACKGROUND: The abandonment of psychotherapeutic treatments is influenced by various factors, including patient characteristics, therapist traits, and the therapeutic relationship. Despite the well-documented importance of these factors, limited empirical research has focused on the role of the therapeutic relationship and the characteristics of therapist-patient dyads in predicting treatment dropout. This study protocol outlines a longitudinal research project aimed at predicting dropout and non-response in psychotherapy for individuals with personality disorders. The research seeks to identi

Paradoxical interventions in behavior therapy.

The use of paradoxical interventions in overcoming resistance to behavior therapy is discussed and a categorization and description of paradoxical interventions provided. Two factors are of primary importance in assessing the desirability of using paradoxical interventions: the individual's reactance potential and the perceived freedom of the problematic behavior. Paradoxical interventions themselves can be divided into two categories: compliance-based strategies and defiance-based strategies. Several types of paradoxical interventions are described and their use within therapy discussed, such

Expanding the role of Australian community dietitians in gout management.

AIM: Gout is a common form of inflammatory arthritis with suboptimal management. Management guidelines for gout highlight the importance of both pharmacological and non-pharmacological treatments. Dietitians can potentially assist in improving gout's associated dietary and lifestyle factors, and thereby play a role in improving its management. The aim of this study was to investigate perspectives of Australian community dietitians on whether their role in gout management could be expanded to improve management and treatment of gout. METHOD: A snowballing recruitment strategy was used. Dietiti

Parent's perceptions of interactions with health professionals in the pathway to gaining a diagnosis of tuberous sclerosis (TS) and beyond.

This paper presents selected findings from a qualitative study on the lived experience of young people with tuberous sclerosis (TS) and their families. In-depth interviews with 109 participants from 40 families were undertaken. This paper presents the findings in relation to parents' interactions with health professionals before, during and after a diagnosis of TS was given. The majority of parents recalled examples of both positive and negative experiences with health professionals. Negative experiences included having to 'fight' for the recognition of symptoms and the reluctance of doctors t

An empirical comparison of how fertility patients and health care professionals interpret verbal probability expressions.

OBJECTIVES: To investigate how fertility patients (FPs) and healthcare professionals (HCPs) interpret verbal probability expressions used to communicate the chance of pregnancy with fertility treatments, and whether patient-professional group membership accounts for a meaningful proportion of the observed variation. METHODS: Participants completed two online judgment tasks in a generalized fertility treatment context. In the ordering task, they ranked 16 probability terms from highest to lowest perceived chance of pregnancy. In the rating task, they assigned numerical probabilities (0-100%) t

"It's way more than just writing a prescription": A qualitative study of preferences for integrated versus non-integrated treatment models among individuals with opioid use disorder.

BACKGROUND: Increasingly, treatment for opioid use disorder (OUD) is offered in integrated treatment models addressing both substance use and other health conditions within the same system. This often includes offering medications for OUD in general medical settings. It remains uncertain whether integrated OUD treatment models are preferred to non-integrated models, where treatment is provided within a distinct treatment system. This study aimed to explore preferences for integrated versus non-integrated treatment models among people with OUD and examine what factors may influence preferences.

Partnering with parents to disclose Klinefelter syndrome to their child.

UNLABELLED: In paediatrics, a diagnosis of Klinefelter syndrome can occur after prenatal testing or because of symptoms such as learning difficulties or incomplete puberty. After the diagnosis, parents have to decide when and how to speak about this condition to their child. Parents and healthcare professionals (HCPs) may have different perceptions related to disclosure of this diagnosis. Due to the absence of clear guidelines, parents and HCPs may feel uncomfortable discussing the condition and may hide the truth in a prolonged fashion. CONCLUSION: For patients with a prenatal diagnosis, we

A therapist experiential model of treatment for brain injury.

The Therapist Experiential Model of Treatment (TEMT) captures the commonality of clinical reactions and challenges that therapists encounter when treating patients with acquired brain injury and their support networks in outpatient multidisciplinary treatment settings. A three-tier coping paradigm depicts therapists' fluctuating demeanor, adjustment, and overall outlook. Patient, system, and internal variables facilitate or impede professional efficacy and growth. The TEMT is an effective mentalizing tool, catalyzing self-exploration of countertransference, burnout, and fractionalized working

Self-care and communication issues at the end of life of recipients of a left-ventricular assist device as destination therapy.

PURPOSE OF REVIEW: The purpose of this review is to provide an overview of self- care and communication issues at the end of life of patients with left-ventricular assist devices (LVADs) for destination therapy, based on recent research on end-of-life communication in other diseases. RECENT FINDINGS: For many patients with advanced heart failure, LVADs as destination therapy improve survival and quality of life. However, LVADs can be associated with complications, new comorbidities or worsening of previous conditions, resulting in decreased quality of life and limited prognosis, raising the n

Professional Independence as the Foundation of Modern Pharmacy Practice: A Comparative Analysis of Deontological Frameworks Across 30 European Jurisdictions.

Background: The modern pharmacist is no longer just a person who prepares medicines, but has become an important health care provider involved in public health, including pharmaceutical care, prevention, chronic disease management and digital health. Such a transition requires the need for professional independence, supported by effective regulation. Deontological codes guarantee this independence and duties but variability in legal frameworks across Europe limits consistent innovation in pharmaceutical care. Objectives: The study analyses the deontological framework for pharmacist professiona

Therapist awareness and responsibility in working with sexual offenders.

The article aims to address the ethical implications of therapist reactions to sexual offender treatment and, in doing so, increase awareness and understanding of the interaction between the clinician and the challenges of working with sexual offenders. The effects of providing psychological services in general, and to sexual offenders specifically, are reviewed, including the impact on services provided. Practice implications are also discussed as the authors consider the negative effects (e.g., burnout) that therapists experience due to treatment quality and the ethical issues and responsibi

Literacy: a problem that managers must handle.

Patient and employee illiteracy is a problem that health care managers must deal with. More than half of all Americans may be unable to read and understand written materials used in hospitals, clinics, and other health care facilities. There is a mismatch between employees' and patients' reading level and the reading level of most written materials used by health care facilities. This can lead to noncompliance with treatment, missed appointments, wrong dosage of medications, uninformed consent, and undo fear among patients. Employees who are unable to read directions or policies and procedures

Overview: cerebrovascular accident and the hospitalized elderly--a multidimensional clinical problem.

A cerebrovascular accident is unwelcome at any age, but for the elderly it involves concerns. The abrupt onset of a cerebrovascular accident signals anew the already present fears of loss of control, death, insanity, disfigurement, loss of physical function, and sexual impairment. The accident can also result in worry about the possibility of explosive recurrence, disruption of thoughts and emotions, lengthy treatment away from home, and exhaustion of retirement funds. Therapeutic endeavors must be eclectic and individually tailored to address the special needs of the elderly patient, the high

Effective process or dangerous precipice: qualitative comparative embedded case study with young people with epilepsy and their parents during transition from children's to adult services.

BACKGROUND: Transition from children's to adult epilepsy services is known to be challenging. Some young people partially or completely disengage from contact with services, thereby risking their health and wellbeing. We conducted a mixed-method systematic review that showed current epilepsy transition models enabling information exchange and developing self-care skills were not working well. We used synthesised evidence to develop a theoretical framework to inform this qualitative study. The aim was to address a critical research gap by exploring communication, information needs, and experien

Outreach workers' experiences in a homeless outreach project: issues of boundaries, ethics and staff safety.

Mental health professionals and researchers have emphasized the importance of conducting outreach to locate homeless persons with mental illness, and of creatively engaging these persons into a therapeutic relationship. These outreach and engagement activities raise challenging issues in the areas of client-staff boundaries, professional ethics, and staff safety. While several issues in each of these three key areas have received attention in the growing literature on homelessness, certain issues within each area remain unexplored. The authors draw from the street experiences of outreach staff
