Postpartum Psychosis and Bipolar Disorder: Review of Neurobiology and Expert Consensus Statement on Classification.

Postpartum psychosis (PP) is an acute and severe psychiatric illness with onset within weeks after delivery and a high risk of suicide and infanticide. Most women with PP experience severe mood symptoms, including mania, mixed episodes, or depression with psychotic features. Impaired cognition, irritability, and agitation are also common. The specific timing of PP strongly suggests a biological basis, because the postpartum time period is characterized by profound endocrine, immune, neuroanatomical, and physiological changes in the brain. Genetic studies show a unique risk architecture, partly

The tyramine test is not a marker for postnatal depression: early postpartum euphoria may be.

Abnormally low tyramine test values are known to be markers for vulnerability to unipolar, but not bipolar, endogenous depression. In the present study, 37 women with recent postnatal depression (25 major, 12 minor) and 22 puerperal controls with no depressive disorder, all assessed by Schedule for Affective Disorder and Schizophrenia (SADS-L) interview, together with 17 other controls, underwent the test. No significant differences in tyramine sulfate output were demonstrated between the different groups. Those subjects with endogenous features according to Newcastle score (n = 7) or Research

Reproductive factors affecting the course of affective illness in women.

Women are at higher risk than men to develop depressive episodes during the reproductive years. Furthermore, women are vulnerable to depressions associated with oral contraceptives, abortion, the premenstrual period, the puerperium, and menopause. The phenomenology and the biologic mechanisms involved in these illnesses perhaps should be viewed in the context of other manifestations of the link between depression and female reproductive functions. For example, women are especially vulnerable to a rapid cycling form of affective illness and to hypothyroidism, an associated factor for this form

Catatonia during pregnancy and the postpartum period.

While the psychopathology of mental disorders during pregnancy and the postpartum period is a growing area of research, the prevalence and significance of catatonic symptoms has been relatively neglected. To address this gap in knowledge, a systematic review of articles on catatonia occurring during pregnancy and the postpartum period was conducted. PubMed, Excerpta Medica, (later EMBASE) databases were queried for articles published in English from their inception in 1966 and 1946, respectively to May 31. 2022 using the terms "catatonia", AND "perinatal", "puerperal", "postpartum", "antepartu

[Epidemiological surveillance and obstetrical dystocias surgery in Senegal].

Maternal morbidity and mortality remain major problems of public health in developing countries. Having long been neglected, maternal health is now being included among the priorities of a large number of countries. The rate of maternal mortality in Senegal is 850 per 100,000 live births, among the highest in the world. The main causes of maternal mortality in Africa are obstructed labour and uterine rupture, hypertensive disorders of pregnancy, puerperal infection and haemorrhage. An epidemiological survey of obstetric disorders was initiated in 1992 in Senegal to characterise the requirement

A prospective study of postpartum psychoses in a high-risk group. 1. Clinical characteristics of the current postpartum episodes.

Postpartum psychotic episodes (PPPs) occurring during the first 6 months after delivery were prospectively studied in 88 pregnant index women with a history of nonorganic psychosis and 104 pregnant controls with no such history. While no control developed a PPP, PPPs were found following 28% of the index deliveries, almost all of these 25 cases being psychiatrically hospitalized. PPPs were especially frequent among cases with total illness diagnoses of Cycloid Psychosis and Affective Illness. More than half of the 25 cases had symptom onset within 3 weeks of delivery, and these early onset cas

State-of-the-art treatment of postpartum bipolar disorder.

PURPOSE OF REVIEW: To summarize recent updates in the treatment of postpartum bipolar disorder (PBD).PBD requires timely and comprehensive management, as childbirth is a period of elevated relapse risk in women with preexisting illness and may also mark first-onset presentations. Episodes can manifest as depression, mixed states, mania, or psychosis, with severe consequences for maternal safety, infant well being, and early bonding. Sleep loss around labor and postpartum further increases vulnerability. RECENT FINDINGS: PBD demands a multifaceted therapeutic approach, with pharmacotherapy as

Psychometric ante-natal predictors of post-natal depressed mood.

One hundred and nine women were studied during the course of pregnancy in an attempt to predict which of them would later rate themselves as suffering from depressed mood up to twelve months post-natally. The psychometric measures used comprised the Zung Self-Rating Scale to measure depression, the DSSI/SAD sub-scale to measure anxiety and the Hostility and Direction of Hostility Questionnaire. Post-natal depression observed up to twelve months post-partum was positively associated with first trimester ante-natal scores on all three measures. The HDHQ and the DSSI/SAD measures were good predic

Substance use and use disorders during pregnancy and the postpartum period.

Substance use and substance use disorders among pregnant and postpartum individuals have risen dramatically, necessitating enhanced clinician education and intervention strategies. This expert review offers obstetricians and gynecologists a comprehensive overview of substance use during the prenatal and postpartum periods. We discuss the epidemiology, maternal and fetal adverse effects, as well as treatment approaches for major substances: nicotine, cannabis, alcohol, benzodiazepines, stimulants, and opioids. Additionally, we address the ethical and legal implications of substance use during p

Autoerthrocyte sensitization---a psychogenic purpura?

There are two schools of thought regarding the pathogenesis of this syndrome. Gardner and Diamond and others demonstrated that these patients have a hypersensitivity to extravasated RBCs. They assumed that a fixed tissue antibody reacted with red blood cell stroma producing edema, increased capillary permeability, and further extravasation of blood into the tissues. Ratnoff, Agle, and others, however, believe the syndrome is a psychiatric disorder and that the ecchymotic lesions represent a form of conversion reaction--a mechanism that could explain the patient's myriad complaints. There is ev

Biological and psychosocial predictors of anxiety worsening in the postpartum period: A longitudinal study.

BACKGROUND: As many as 20% of women will experience an anxiety disorder during the perinatal period. Women with pre-existing anxiety disorders are at increased risk of worsening during this time, yet little is known about its predictors. STUDY AIM: To investigate the psychosocial and biological risk factors for anxiety worsening in the postpartum in women with pre-existing anxiety disorders. METHODS: Thirty-five (n = 35) pregnant women with pre-existing DSM-5 anxiety disorders were enrolled in this prospective study investigating the psychosocial (e.g., childhood trauma, intolerance of uncer

Effectiveness of Electroconvulsive Therapy in Postpartum Psychosis: A Systematic Review.

Postpartum psychosis is a severe psychiatric condition that emerges shortly after childbirth. Electroconvulsive therapy provides rapid symptom relief, particularly in severe and treatment-resistant cases. Despite evidence supporting its effectiveness, the use of electroconvulsive therapy in postpartum psychosis remains limited and unstandardized. This systematic review evaluates the clinical effectiveness of electroconvulsive therapy in treating postpartum psychosis. A comprehensive search of PubMed/MEDLINE, Cochrane, SciELO, SCOPUS, and Web of Science identified 255 studies, of which 7 met th

Postpartum anxiety: a state-of-the-art review.

Although there has been increasing interest in the study of postpartum anxiety in recent years, it remains an emerging field. We present a state-of-the-art review of postpartum anxiety, with the aim of comprehensively surveying postpartum anxiety literature and presenting a synthesis of the complete body of knowledge around postpartum anxiety. We found an estimated global prevalence of 12·3% for postpartum anxiety. Postpartum anxiety is associated with primiparity and younger maternal age. Multiple screening tools are validated for use in postpartum anxiety, although none of these tools are va
