Splenic Infarction and Recurrent Pulmonary Embolism in Atrial Fibrillation Associated With Subtherapeutic Anticoagulation and Medication Nonadherence: A Case Report.

Atrial fibrillation (AF), the most common sustained arrhythmia worldwide, remains a major cause of cardioembolic stroke and systemic thromboembolism due to impaired atrial contraction, blood stasis, and endothelial injury within the left atrial appendage. Although oral anticoagulation with vitamin K antagonists or direct oral anticoagulants substantially reduces thromboembolic risk, maintaining stable and uninterrupted therapeutic exposure remains a persistent clinical challenge. Splenic infarction (SI) represents a rare extracerebral manifestation of systemic embolization and may present with

Cangrelor in a challenging scenario of concomitant ischaemic stroke, pulmonary embolism, and ST-elevation myocardial infarction: a case report.

BACKGROUND: Antithrombotic therapy in acute patients with both high ischaemic and bleeding risks remains challenging. CASE SUMMARY: We presented a challenging case involving a 48-year-old man referred to our hospital for headache and a left superior quadrantanopia. A CT scan revealed a right inferior occipital lobe ischaemic stroke. During the hospital stay, the patients developed pulmonary embolism (PE), and ST-elevation myocardial infarction (STEMI). A triple antithrombotic therapy was indicated, but the patient presented with high bleeding (anaemia, active malignancy, ischaemic stroke) and

Trajectory and correlates of pulmonary congestion by lung ultrasound in patients with acute myocardial infarction: insights from PARADISE-MI.

AIM: PARADISE-MI examined the efficacy of sacubitril/valsartan in acute myocardial infarction (AMI) complicated by reduced left ventricular ejection fraction (LVEF), pulmonary congestion, or both. We sought to assess the trajectory of pulmonary congestion using lung ultrasound (LUS) and its association with cardiac structure and function in a pre-specified substudy. METHODS AND RESULTS: Patients without prior heart failure (HF) underwent eight-zone LUS and echocardiography at baseline (±2 days of randomization) and after 8 months. B-lines were quantified offline, blinded to treatment, clinica

Improving outcomes in chronic obstructive pulmonary disease by taking beta-blockers after acute myocardial infarction: a nationwide observational study.

β-Blockers are a standard therapy for acute myocardial infarction (AMI) due to their better short-term and long-term outcomes. However, β-blockers are often under-prescribed in chronic obstructive pulmonary disease (COPD) patients with AMI, since they are thought be related to bronchospasm. The aim of this study was to investigate the association between the usage of β-blockers and the risk of mortality in COPD patients after first AMI via a nationwide, population-based cohort study. In this retrospective study, we identified 186,326 patients with AMI diagnosed between January 2000 and Decembe

Effect of Chronic Obstructive Pulmonary Disease on In-Hospital Mortality and Clinical Outcomes After ST-Segment Elevation Myocardial Infarction.

There is controversy regarding in-hospital mortality, revascularization, and other adverse outcomes in patients with ST-segment elevation (STEMI) and chronic obstructive pulmonary disease (COPD). We queried the 2003 to 2011 Nationwide Inpatient Sample databases to identify patients aged ≥18 years with a primary diagnosis of STEMI. Univariate and multivariate analyses were performed to evaluate the association of COPD with in-hospital clinical outcomes. Patients with COPD comprised 13.2% of 2,120,005 patients with STEMI. COPD was associated with older age, Medicare insurance, greater co-morbidi

[Postoperative Superior Mesenteric Artery and Cerebral Infarction Possibly due to the Thrombus at the Left Superior Pulmonary Vein Stump].

Pulmonary vein stump thrombus (PVST) was thought to be a rare complication after lung resection. Several cases of embolism due to PVST were reported previously. However, in recent paper, PVST was reported to be found in 13.5% of patients after left upper lobectomy ( LUL). We experienced a case of PVST that induced acute embolism of the superior mesenteric artery at 2 weeks after LUL. After discontinuation of anticoagulation therapy, development of PVST was confirmed by computed tomography scan at 12 months after LUL resulting in cerebral infarction.

When cough leads to infarction: systemic arterial embolism as a sentinel complication of insidious pulmonary mucormycosis in an immunocompetent patient.

BACKGROUND: Pulmonary mucormycosis is a rapidly progressive and often fatal invasive fungal infection typically associated with immunocompromised states. However, its emergence in immunocompetent individuals is increasingly reported. This case describes an unusual presentation of pulmonary mucormycosis in an immunocompetent adult complicated by arterial embolism, a rare and life-threatening vascular manifestation. CASE PRESENTATION: We report the case of an immunocompetent adult patient presenting with persistent respiratory symptoms and radiological abnormalities. Despite the absence of conve

Splenic Infarcts and Pulmonary Renal Syndrome in a Young Patient with Double-Positive Anti-GBM and ANCA-Associated Vasculitis.

Double-positive disease, defined by double-seropositivity for serum anti-glomerular basement membrane (GBM) antibodies and anti-neutrophil cytoplasmic antibodies (ANCA) is a rare cause of pulmonary-renal syndrome. Here, we present an exceptional course of a 20-year-old male with seropositivity for anti-myeloperoxidase anti-neutrophil cytoplasmic antibodies and anti-GBM antibody, who presented first with renal impairment due to focal necrotizing crescentic glomerulonephritis. After receiving treatment, he presented two years later with a relapse manifesting with diffuse alveolar hemorrhage and

Myocardial infarction due to coronary steal caused by a congenital unilateral absence of the right pulmonary artery: a rare case.

A 27-year-old male patient presented with symptoms that mimic an acute coronary syndrome and associated with significant myocardial perfusion ischaemic defects. The latter was detected by adenosine stress Tc-99m estamibi single photon emission CT. However, subsequently the CT angiogram revealed a normal coronary arteries outline and calibre with prominent coronary artery collaterals filling the distal branches of the absent right pulmonary artery.

Left main coronary artery-Pulmonary trunk fistula incidentally detected during acute coronary infarction: Complementary roles of coronary angiography and CCTA.

Coronary artery fistulas (CAFs) are rare vascular anomalies, and fistulas originating from the left main coronary artery (LMCA) are particularly uncommon. A 63-year-old woman with hypertension and obesity presented with acute anterior myocardial infarction and left ventricular systolic dysfunction. Coronary angiography incidentally identified a tortuous vascular tract arising from the proximal LMCA, but couldn't define its drainage site. Coronary computed tomography angiography (CCTA) demonstrated a 3.9 cm fistulous tract with an origin and maximal diameter of 6 mm, coursing anteriorly and to

Anomalous origin of the left coronary artery from pulmonary artery mimicking antero-lateral ST-elevation myocardial infarction: a case report.

BACKGROUND: Anomalous origin of the left coronary artery (LCA) from the pulmonary artery (PA) (ALCAPA) is a rare congenital abnormality. We present a case of an ALCAPA in a 25-year-old man. CASE SUMMARY: A 25-year-old male with no past medical history was admitted to our intensive cardiac care unit after sudden cardiac arrest due to ventricular fibrillation and suspected acute coronary syndrome. After being treated with shock by automated external defibrillator, an electrocardiogram (ECG) demonstrated sinus tachycardia with antero-lateral ST-segment elevation. Initial transthoracic echocardio

Successful treatment of pulmonary aspiration due to brain stem infarction by using cough exercise based on swallowing scintigraphy: preliminary observations.

Although dysphagia in stroke may result in lethal chest infection, it can be prevented by coughing. We report on a patient with dysphagia and aspiration who regained oral ingestion by swallowing with voluntary cough. A 54-year-old man with subarachnoidal hemorrhage underwent endovascular coiling and developed brain stem infarction. Scintigraphy showed pulmonary aspiration just after swallowing, but no radioactivity was detected in the lungs 10 min after swallowing with voluntary cough. Swallowing exercise with voluntary cough resulted in a safe swallow without enteral feeding. Even if laryngea

Prognostic value of cardiac magnetic resonance pulmonary transit time and myocardial strain across different stages of ST-elevation myocardial infarction.

BACKGROUND: This study evaluated the prognostic value of pulmonary transit time (PTT) and myocardial strain in ST-elevation myocardial infarction (STEMI) patients stratified by the Canadian Cardiovascular Society (CCS) classification. MATERIAL AND METHODS: A total of 521 STEMI patients underwent 3.0-T cardiac magnetic resonance (CMR) imaging between January 2017 and December 2025. PTT was measured as the number of cardiac cycles for contrast to travel from the right to left ventricle during first-pass perfusion. Global myocardial strain (longitudinal, circumferential, radial) was assessed via

Prognostic Utility of Pulmonary Arterial Elastance in Patients With ST-Segment Elevation Myocardial Infarction Classified as Killip Classes 3-4.

BACKGROUND: Invasive hemodynamic assessment is critical in patients with ST-segment elevation myocardial infarction (STEMI) classified as Killip classes 3-4. However, the prognostic utility of pulmonary arterial elastance (PaE) remains unclear. METHODS AND RESULTS: We retrospectively analyzed 145 consecutive patients with STEMI classified as Killip classes 3-4 undergoing primary percutaneous coronary intervention, all with complete pulmonary artery catheter data. PaE was calculated as systolic pulmonary artery pressure divided by stroke volume. In total, 27 patients died in hospital (18.6%),

Thrombolytic Therapy for ST-Elevation Myocardial Infarction and High-Risk Pulmonary Embolism in a Non-percutaneous Coronary Intervention-Capable Hospital.

Introduction ST-elevation myocardial infarction (STEMI) and pulmonary embolism (PE) are common life-threatening emergencies encountered in clinical practice. Options for reperfusion therapy are often unavailable in resource-limited settings like Ghana. We present a review of our 19-month experience with systemic thrombolysis for patients with STEMI and hemodynamically unstable PE in Tamale Teaching Hospital (TTH) in Tamale, Ghana. Methods We retrospectively reviewed patients ≥ 18 years old with acute STEMI and PE managed in TTH from January 2023 to July 2024. STEMI was defined using standard c

A Case Report: Cavitary Infarction Caused by Pulmonary Tumor Thrombotic Microangiopathy in a Patient with Pancreatic Intraductal Papillary Mucinous Neoplasm.

Pulmonary tumor embolism is commonly discovered at autopsy, but is rarely suspected ante-mortem. Microangiopathy is an uncommon and distinct form of simple tumor pulmonary embolism. Here, we present a 52-year-old male with tumor thrombotic microangiopathy and pulmonary infarction, which might have originated from intraductal papillary mucinous tumor of the pancreas. Multiple wedge-shaped consolidations were found initially and aggravated with cavitation. These CT features of pulmonary infarction were pathologically confirmed to result from pulmonary tumor thrombotic microangiopathy.
