Diagnostic accuracy and reliability of sideline concussion evaluation: a prospective, case-controlled study in college athletes comparing newer tools and established tests.

OBJECTIVE: To assess diagnostic accuracy and reliability of sideline concussion tests in college athletes. METHODS: Athletes completed baseline concussion tests including Post-Concussion Symptom Scale, Standardised Assessment of Concussion (SAC), modified Balance Error Scoring System (m-BESS), King-Devick test and EYE-SYNC Smooth Pursuits. Testing was repeated in athletes diagnosed acutely with concussion and compared to a matched teammate without concussion. RESULTS: Data were collected on 41 concussed athletes and 41 matched controls. Test-retest reliability for symptom score and symptom s

Disorders of higher gaze control.

The neural centers in the cerebral hemispheres, both cortex and basal ganglia, involved in the generation of saccadic and smooth pursuit eye movements have been well delineated in terms of their location and function. For the generation of saccades these include the frontal eye fields, the supplementary eye field, and the intraparietal sulcus, and in the basal ganglia the caudate nucleus and the substantia nigra, pars compacta. The generation of pursuit eye movements involves the middle temporal (area V5) and medial superior temporal areas and the frontal eye field. These centers and their con

Specialized parallel pathways for adaptive control of visual object pursuit.

To pursue a moving visual object, the brain must continuously steer the object to the center of the visual field via feedback. The gain of this control loop is flexible, yet the biological mechanisms underlying such adaptive control are not well understood. Here, we show that adaptive control in the Drosophila pursuit system involves two parallel pathways. One detects objects in the periphery and steers them toward the center of the visual field. The other detects objects near the center of the visual field and steers them to the visual midline while also increasing forward velocity. This latt

Spatial deployment of attention influences both saccadic and pursuit tracking.

We examined the effects of changing spatial aspects of attention during oculomotor tracking. Human subjects were instructed to make a discrimination on either the small (0.8 degrees ) central or the large (8 degrees ) peripheral part of a compound stimulus (two counter-rotating concentric rings) while the stimulus either translated across the screen or was stationary. During this period, a transient perturbation with either step or ramp movement profile occurred. For perturbations leading to a change in position larger than the small ring, saccades occurred more frequently and had much shorter

Effect of intravenous diazepam and thiopental on voluntary saccades and pursuit eye movements.

The effects of diazepam and thiopental on voluntary saccades and pursuit eye movements were tested in 9 volunteers, with an interval of at least 2 weeks between tests. One, 4 and 8 h after intravenous injection of diazepam (0.3 mg/kg) or thiopental (6.0 mg/kg), voluntary saccades and pursuit eye movements were tested and blood samples taken for analysis of drug concentration. As compared to results of tests without drugs, a significant reduction both of saccadic peak velocity and gain of pursuit eye movements was found 1 h after injection of either drug, but not after 4 and 8 h. The amplitude

What eye-movements tell us about Disorders of Consciousness?

In everyday life, eye movements (including fixation, smooth pursuit and saccades) are essential for interacting with the visual environment. In patients with Disorders of Consciousness (DoC), visual fixation and pursuit are considered among the earliest clinical signs of awareness recovery. However, the traditional distinction between reflexive and voluntary eye movements may be insufficient to interpret these behaviours, as movements that appear automatic can still recruit cortical networks. This review re-examines eye movements in DoC through a neuroanatomical and translational lens. We char

[Rehabilitation of patients with vestibular disorders].

The connections between the two vestibular nuclei permit vestibular compensation after unilateral lesions. The interactions between the vestibular and the visual and proprioceptive system can support vestibular compensation. Our physical training program for the treatment of vertigo is based on these mechanisms. The training consists of fixation exercises, smooth pursuit and motor learning by proprioceptive cognition. The results are very convincing.

Cross-power spectral density analysis of pursuit tracking. Evaluation of central and peripheral pathology.

Twenty-one normal subjects were evaluated for pursuit tracking performance at frequencies of .2, .4, .8, 1.2 and 1.6 Hz. Measures of phase (lead or lag of target), gain (amplitude of response) and spectral purity (distortion of smooth tracking) were obtained by a cross-power spectrum analysis of the stimulus input and the eye movement output. It was determined that normal subjects could quite adequately track a target moving at .4 Hz. After establishing this parameter for normals, 43 patients with central and peripheral pathology related to the vestibulo-oculomotor system were tested at this f

Familial, congenital paralysis of horizontal gaze.

Eye movements were studied in a sister and brother with familial, congenital paralysis of horizontal gaze. Horizontal and vertical eye movements were recorded with DC electro-oculography and analyzed with a laboratory computer. All horizontal, conjugate eye movements were absent (saccades, pursuit, optokinetic nystagmus, vestibulo-ocular response, and visual-vestibular responses). Voluntary vergence eye movements were preserved and were used to track visual targets. An involuntary, horizontal, pendular nystagmus was found to represent disconjugate, smooth, vergence eye movements. Vertical sacc

Integration time for the perception of depth from motion parallax.

The perception of depth from relative motion is believed to be a slow process that "builds-up" over a period of observation. However, in the case of motion parallax, the potential accuracy of the depth estimate suffers as the observer translates during the viewing period. Our recent quantitative model for the perception of depth from motion parallax proposes that relative object depth (d) can be determined from retinal image motion (dθ/dt), pursuit eye movement (dα/dt), and fixation distance (f) by the formula: d/f≈dθ/dα. Given the model's dynamics, it is important to know the integration time

Vertical diplopia on downgaze with bifocals.

PURPOSE: To investigate if jump or diplopia is experienced at the dividing line of bifocal lenses and if the belief in jump may be, in part, the result of leading questions. METHODS: Ten subjects were asked an open question and 10 a closed question relating to jump for six bifocal lenses and three eye movement conditions. RESULTS: All subjects failed to see any effects other than near blur with saccadic movements. Many subjects observed vertical diplopia with round 38 segments at the bifocal edge during pursuit movements given an open choice of responses. With pursuit movements, when asked a

Short latency ocular-following responses in man.

The ocular-following responses elicited by brief unexpected movements of the visual scene were studied in human subjects. Response latencies varied with the type of stimulus and decreased systematically with increasing stimulus speed but, unlike those of monkeys, were not solely determined by the temporal frequency generated by sine-wave stimuli. Minimum latencies (70-75 ms) were considerably shorter than those reported for other visually driven eye movements. The magnitude of the responses to sine-wave stimuli changed markedly with stimulus speed and only slightly with spatial frequency over

Effects of alcohol and other psychotropic drugs on eye movements: relevance to traffic safety.

The effects of alcohol and other psychotropic drugs on eye movements are reviewed with particular attention to the possible relevance of these effects for traffic safety. Alcohol has been shown to have diverse effects, including reduction of the velocity of both saccadic and smooth pursuit eye movements, increased saccadic latency, impairment of convergence and induction of nystagmus. These effects probably contribute to impaired visual information processing, which reduces driving ability. Barbiturates have been reported to produce effects similar to alcohol, and the effects of benzodiazepine

Expanding the view of perceptual-cognitive training: The ecological validity of immersive 360° videos evidenced by differences in gaze behavior compared to traditional fixed-screen videos.

While 360° videos have shown promise for enhancing perceptual-cognitive skills in sport, the mechanisms underlying its advantages over traditional video-based training remain unclear. This study tested whether 360° videos promote different visual search strategies compared to traditional fixed screens (FS), and whether these strategies vary based on expertise level. Thirty-six participants (17 highly-trained basketball players, 19 novices) viewed the same 25 basketball video clips in 360° and FS modes while their eye and head movements were recorded by an eye tracker embedded in a head-mounted

Evaluation of Oculomotor Function Following Sleep Improvement With Lemborexant in Older Adults With Insomnia.

OBJECTIVE: Insomnia impairs cognitive and brain function, and its treatment improves outcomes. Eye movements are objective biomarkers of neurocognitive state and are sensitive to sleep loss, but whether treatment improves oculomotor function remains unclear. We examined associations between sleep improvement and eye movement changes after hypnotic intervention. METHODS: In this open-label, single-arm study, 31 Japanese adults aged ≥ 50 years with insomnia and preserved cognition received nightly lemborexant (5-10 mg) and were assessed at baseline, week 4, and week 12. Eye movements were measu

Multifactorial determinants of visual outcome in very and extremely preterm infants: A systematic literature review of risk factors for visual and oculomotor dysfunctions.

Preterm birth is often associated with neurological, neurodevelopmental and visual impairment arising from the complex interplay of ocular, visual pathway, and cerebral vulnerability in the immature infant. Visual dysfunction in premature infants may reflect ocular, anterior visual pathways, retrogeniculate pathways and visual associative areas abnormalities, as seen in Cerebral Visual Impairment. This systematic review synthesized evidence on the association between pre-, peri-, i and postnatal factors on basic visual (visual acuity, contrast sensitivity, visual field) and oculomotor (fixatio

Recovery Process for Sports-Related Concussion Assessed with Precise Ocular Motility.

Ocular motility has been linked to Sports Concussion Assessment Tool 5 scores. However, the link between ocular motility changes and assessment result changes remains unclear. Hence, we investigated that potential link in patients with sports-related concussions. We retrospectively included participants aged≥18 years who were diagnosed with a sports-related concussion. They underwent smooth pursuit eye movement assessment for allocation to the good improvement (rate of fundamental frequency≥15%) or minor improvement (<15%) groups. Sports Concussion Assessment Tool 5 scores were determined at b

Ocular Motor Findings Aid in Differentiation of Spinocerebellar Ataxia Type 17 from Huntington's Disease.

Differentiation of spinocerebellar ataxia type 17 (SCA17) from Huntington's disease (HD) is often challenging since they share the clinical features of chorea, parkinsonism, and dystonia. The ocular motor findings remain to be elucidated in SCA17, and may help differentiating SCA17 from HD. We retrospectively compared the ocular motor findings of 11 patients with SCA17 with those of 10 patients with HD. In SCA17, abnormal ocular motor findings included impaired smooth pursuit (9/11, 82%), dysmetric saccades (9/11, 82%), central positional nystagmus (CPN, 7/11, 64%), abnormal head-impulse tests

Eye movement recordings to investigate a supranuclear component in chronic progressive external ophthalmoplegia: a cross-sectional study.

BACKGROUND: It has been postulated that eye movement disorders in chronic progressive external ophthalmoplegia (CPEO) have a neurological as well as a myopathic component to them. AIM: To investigate whether there is a supranuclear component to eye movement disorders in CPEO using eye movement recordings. METHODS: Saccade and smooth pursuit (SP) characteristics together with vestibulo-ocular reflex (VOR) gain and VOR suppression (VORS) gain in 18 patients with CPEO and 34 normal patients were measured using Eyelink II video-oculography. RESULTS: The asymptotic values of the peak velocity ma
