This nonrandomized, open clinical investigation of tuberculous meningitis evaluated 53 children with Stage I (n = 8), Stage II (n = 29) and Stage III (n = 16) disease. The overall mortality was 20.8% (11 of 53) with a rate of sequelae of 35.7% (15 of 42) in survivors reflecting the advanced stages of children at diagnosis. We presented three patients with trochanteric tuberculosis and described the clinical and imaging findings of the infection. Histology revealed a necrotizing granulomatous bursitis and microbiology confirmed tuberculosis. All cases were successfully treated with bursectomy and curettage of the trochanteric lesion and antituberculous chemotherapy including isoniazid, pyrazinamide, rifampicin, and ethambutol.

Our 15-year retrospective review of reported pediatric tuberculosis (TB) cases in the state of Georgia indicates that most children in our state have nonsevere TB, and implementing a 4-month rifampin, isoniazid, pyrazinamide, and ethambutol-based regimen for drug-susceptible nonsevere TB would substantially reduce the burden of directly observed therapy.

The sensitivity of Mycobacterium lepraemurium for isoniazide, dapsone, ethionamide, pyrazinamide, rifampicin, p-aminosalicylic acid, ethambutol and various sulfonamides was determined using Ogawa medium. The sensitivities of M. lepraemurium for these drugs are different from those obtained with M. leprae. Our results illustrate that M. lepraemurium is not a model for the drug sensitivities of M. leprae.

Two families are reported affected with hereditary renal hypouricaemia associated with uric acid lithiasis. The propositi of both families were found also to have hyperabsorptive hypercalciuria. Based on the study of the effect of pyrazinamide and probenecid on uric acid excretion in both propositi, it is suggested that the defect in uric acid reabsorption is most probably at the pre-secretory site.

Exfoliative dermatitis to all four first line drugs singly or rarely in combination has been reported. A 35 year old black Somalian woman with miliary tuberculosis developed hepatotoxicity after a few days of treatment with isoniazid, rifampicin, pyrazinamide, and ethambutol. After withdrawal of all drugs the liver profile returned to normal and remained so after challenge with isoniazid. Hepatotoxicity recurred when rifampicin was added, but it was well tolerated when reintroduced without isoniazid.

Existing therapies are often tried when new diseases are discovered or new manifestations of known organisms are recognized. This article reviews the successful application of older commonly used antibiotics to several representative "new" or "newly recognized" illnesses. Examples include new uses for the antistaphylococcal agents, penicillin, the tetracyclines, erythromycin, vancomycin, trimethoprim-sulfamethoxazole, pyrazinamide, metronidazole, clindamycin, and others.

A 14 year-old boy with coeliac disease and poorly treated diabetes mellitus and pulmonary tuberculosis due to INH resistant BK presented with a permanent malabsorption of rifampicin. Pharmacokinetics of rifampicin was studied after oral administration and intravenous injection. Treatment of diabetes and coeliac disease did not improve the situation. Tuberculosis was cured by pyrazinamide and ethambutol.
