BACKGROUND: We present a case of short QT syndrome (SQTS) initially manifesting as atrial fibrillation (AF) and degenerating to polymorphic ventricular tachycardia (PMVT), successfully managed with quinidine.
CASE SUMMARY: A 66-year-old man presented with palpitations and was found to be in AF with a QTc interval of 335 ms. Tricyclic antidepressants have anticholinergic, adrenolytic and quinidine-like activity. These actions result in a variety of cardiac and blood pressure effects. Tricyclic antidepressants can reverse the antihypertensive effect of guanethidine and clonidine. Orthostatic hypotension may be increased with diuretics and hydralazine. Myocardial depression may occur with lidocaine, phenytoin or propranolol. Dangerous additive effects may result from concomitant use of a tricyclic antidepressant and either quinidine or procainamide.

A case of sustained supraventricular tachycardia of unknown aetiology in a two-year-old Thoroughbred filly is reported. The cardiac dysrhythm was successfully treated by the oral administration of quinidine sulphate. Conversion of the dysrhythm to sinus rhythm occurred approximately 80 min after the initial dose of 5 g of quinidine sulphate. The horse returned to training approximately 2 months after treatment and has since successfully returned to racing.

In a prospective study 45 patients with paroxysmal atrial fibrillation were randomly assigned to one of three groups (of 15 patients each): group I received oral digoxin, three times 0.125 mg up to twice 0.25 mg daily; group II oral digoxin twice 0.125 mg and quinidine hydrogen sulphate 750-1000 mg daily; group III oral digoxin three times 0.125 mg and flecainide 200-300 mg daily. Two cases of cerebral malaria imported from Guyana and Ghana are reported. These are the first cases of cerebral malaria diagnosed and treated in Trinidad and Tobago since malaria was eradicated. The management of both these cases was complicated because the patients' erythrocytes were glucose-6-phosphate dehydrogenase-deficient, and by the occurrence of blackwater fever, cerebral manifestations, renal impairment, hyperglycaemia and thrombocytopenia. The symptoms of cerebral malaria resolved following treatment with quinidine and doxycycline and quinidine and clindamycin.
