A clinically evident radiculopathy without correlation in the imaging studies represents a serious problem regarding the indication, planning and execution of an operative procedure for its treatment. Both the diagnosis and treatment of such cases are deemed to be difficult without clear morphological correlation. Moreover, the surgeon lacks an important basis for the adequate planning and above all the justification of surgical treatment. Although discography with post-discographic computer tomography (CT discography) is still controversially discussed as an invasive diagnostic measure, the l…

Posterior cervical foraminotomy is an attractive therapeutic option in selected cases of cervical radiculopathy that maintains cervical range of motion and minimize adjacent-segment degeneration. The focus of this procedure is to preserve as much of the facet as possible with decompression. Posterior cervical inclinatory foraminotomy (PCIF) is a new technique developed to offer excellent results by inclinatory decompression with minimal facet resection. The highlight of our PCIF technique is the use of inclinatory drilling out for preserving more of facet joint. The operative indications are r…

STUDY DESIGN: Case report.
OBJECTIVE: To present the case of a patient with low back and lower extremity pain suspected to result from lumbar radiculopathy but who was discovered to have symptomatic uterine fibroid (UF).
SUMMARY OF BACKGROUND DATA: UFs are usually asymptomatic but at times can produce symptoms such as excessive menstrual bleeding, intermenstrual bleeding, dysmenorrhea, pelvic pain, pelvic pressure, bloating, dyspareunia, urinary and bowel disturbance, subfertility, and pregnancy-related complications. In rare cases, they can cause neural impingement leading to lower extremit…

Chondroma is a very unusual cartilagenous neoplasm of the spine. Here we are present a case of spinal chondroma with radiculopathy. A 54-year-old female patient consulted with progressive low back pain and left femoral numbness. Lumbar spinal Magnetic resonance (MR) imaging studies showed an extradural mass lesion in the left L2 body. Computerized tomography (CT) did not reveal any osteolytic lesion of the bone. The mass lesion was excised totally by left partial hemilaminectomy and the intradural compartment was also checked. The histopathology of the lesion was confirmed as chondroma. Preope…

Lumbar epidural varices either primary or secondary to outflow obstruction have been implicated in causing lumbar radiculopathy. We report a case of an adolescent male who presented with lumbar radiculopathy attributable to thrombosed lumbar epidural varix as a result of inferior vena cava thrombosis emanating from factor V Leiden mutation. This rare cause of radiculopathy was diagnosed on contrast-enhanced computerized tomography. All symptoms resolved shortly after initiation of intravenous heparin therapy. The relevant literature is reviewed.

OBJECTIVE: An electrodiagnostic evaluation is often requested for patients with suspected lumbosacral radiculopathy. Although musculoskeletal disorders can produce lower-limb symptoms, their prevalence in this referral population is unknown. The purpose of this study was to determine the prevalence of common lower-limb musculoskeletal disorders in patients referred for lower-limb electrodiagnosis and determine whether these findings predict study outcome.
DESIGN: Subjects undergoing an electrodiagnostic study for lower-limb symptoms and suspected lumbosacral radiculopathy constituted the samp…

OBJECTIVE: The purpose is to describe active and passive physical maneuvers that provoke or alleviate lumbosacral radiculopathy and/or sciatica to demonstrate how these maneuvers function in common orthopedic and neurological tests/signs.
METHODS: Descriptions of the maneuvers and their countermaneuvers are provided to establish an understanding of their mechanisms and influence on lumbosacral nerve roots and the sciatic nerve. Common tests and signs for lower extremity radiculopathy and sciatica are described to demonstrate the clinical application of the maneuvers. Combinations of common te…

SETTING: Electrodiagnostic laboratories in departments of physical medicine and rehabilitation at 5 participating institutions.
PARTICIPANTS: A total of 191 subjects undergoing electrodiagnostic evaluations for upper-limb symptoms when cervical radiculopathy was suspected.
INTERVENTIONS: Not applicable.

We present a 57-year-old man with ankylosing spondylitis (AS) and significant dorsal lumbar dural ectasia (diverticulum). An L5 nerve root monoradiculopathy associated with AS has not been previously reported. The quantity and extent of such ectasia is variable but may be related to cauda equina syndrome. Long-term follow-up of these cases may reveal whether or not solitary nerve lesions gradually lead to cauda equina syndrome. We recommend that asymptomatic or symptomatic patients with dural ectasia should be closely observed without need for immediate surgical intervention.

Most synovial cysts of the lumbar spine appear as intraspinal extradural masses adjacent to the facet joint. Almost all symptomatic synovial cysts are located within the spinal canal and or the neural foramen. To our knowledge, only 6 cases of symptomatic extraspinal synovial cysts have been described in the literature. The authors report a case of lumbar radiculopathy caused by a synovial cyst located in the far lateral extraforaminal area. The patient underwent decompression of the L5 nerve root with recapping right isthmectomy and facetectomy. Postoperative recovery was uneventful, and the …

Interbody fusion after anterior discectomy may lead to acceleration of degenerative changes at adjacent levels. Although the posterior approach preserves the motion segment, decompression of the nerve root is indirect if "hard disc prolaps" is the main cause. Recently, a technique of microsurgical anterior cervical foraminotomy for the treatment of radiculopathy with preservation of the segment mobility was published. In this study, we present this technique with several modifications.Thirteen patients - 5 men and 8 women with an average age of 49 years - with unilateral radiculopathy resistan…

European spine journal : official publication of the European Spine Society, the European Spinal Deformity Society, and the European Section of the Cervical Spine Research Society2003eng
In general, deltoid paralysis develops in patients with cervical disc herniation (CDH) or cervical spondylotic radiculopathy (CSR) at the level of C4/5, resulting in compression of the C5 nerve root. Therefore, little attention has been paid to CDH or CSR at other levels as the possible cause of deltoid paralysis. In addition, the surgical outcomes for deltoid paralysis have not been fully described. Fourteen patients with single-level CDH or CSR, who had undergone anterior cervical decompression and fusion for deltoid paralysis, were included in this study. The severity of deltoid paralysis w…

A case of cervico thoracic junctional area spinal tuberculosis presenting as painful radiculitis of the upper extremity is reported. The predominant symptom of radicular pain and muscle weakness in the hand, along with a claw deformity, led to considerable delay in diagnosis. The presence of advanced bone destruction with severe instability was demonstrated on the MRI scan done later. Surgical management by radical anterior debridement and fusion, along with chemotherapy, led to resolution of the upper extremity symptoms. The brachial plexus radiculopathy secondary to tuberculosis has not been…

OBJECT: The aim of this study was to investigate the indications and treatment options in patients with lower-extremity neuropathies and radiculopathies caused by endometriosis.
METHODS: The authors identified five patients whose symptoms included catamenial pain, weakness, and sensory loss involving the sciatic and femoral nerves and multiple lumbosacral nerve roots. Radiographic studies supported the diagnosis of catamenial neuropathy or radiculopathy, but definitive diagnosis depended on surgical and pathological examination. Treatment of symptoms, including physical therapy and a course o…

Limited flip angle (LFA), gradient echo imaging was performed in 130 patients for evaluation of cervical radicular complaints. The LFA study … suggest that MR imaging is the initial procedure of choice for the evaluation of suspected cervical radiculopathy.
