Radiofrequency ablation of small follicular neoplasms: initial clinical outcomes.

BACKGROUND: In thyroid gland, radiofrequency ablation (RFA) has been applied to both recurrent cancers and benign nodules, although, according to the American Thyroid Association (ATA) and the Korean Society of Thyroid Radiology (KSThR) guidelines, surgery is the first-line treatment for follicular neoplasm. However, it has been argued that follicular neoplasm with lower risk of malignancy can be managed by close follow-up. In this study, we evaluated the effectiveness of RFA of small follicular neoplasms, examining reductions in volume and related clinical problems, and making observations ov…

Squamous cell carcinoma after radiofrequency ablation for Barrett's dysplasia.

Barrett's oesophagus (BO) is a usually indolent condition that occasionally requires endoscopic therapy. Radiofrequency ablation (RFA) is an effective endoscopic treatment for high grade dysplasia (HGD) and intramucosal cancer in BO. It has a good efficacy, durability and safety profile although complications can occur. Here we describe a case of RFA in a patient with high grade dysplasia. Although the response to treatment was initially very good with the development of neosquamous epithelium, the patient very rapidly developed a squamous cell cancer of the oesophagus confirmed on radiology, …

Radiofrequency ablation for early-stage nonsmall cell lung cancer.

This review examines studies of radiofrequency ablation (RFA) of nonsmall cell lung cancer (NSCLC) and discusses the role of RFA in treatment of early-stage NSCLC. RFA is usually performed under local anesthesia with computed tomography guidance. RFA-associated mortality, while being rare, can result from pulmonary events. RFA causes pneumothorax in up to 63% of cases, although pneumothorax requiring chest drainage occurs in less than 15% of procedures. Other severe complications are rare. After RFA of stage I NSCLC, 31-42% of patients show local progression. The 1-, 2-, 3-, and 5-year overall…

Asystole induced by radiofrequency catheter ablation of slow pathway.

Radiofrequency (RF) catheter ablation is a treatment of choice widely used for a variety of supraventricular tachycardia. Transient sinus and atrioventricular (AV) node dysfunctions may occur during RF application to sites remote from the sinus and AV nodes, but they generally resolve quickly after cessation of RF current. We present a case of two episodes of asystole in a 43-year-old man induced by RF catheter ablation of an AV nodal slow pathway. A Bezold-Jarisch-like reflex, direct stimulation of parasympathetic fibers traveling to the sinus and AV nodes, RF-induced myocardial injury or pai

Extending indication for radiofrequency ablation (RFA) in cancer surgery.

Radiofrequency ablation (RFA) represents one of the successful methods for the thermal ablation of unresectable tumors. The tumoricidal effect of this method consists in the tissular conversion of electromagnetic energy into thermal energy. The term refers to the electric current, whose frequencies reach high values, 480 KHz on average. The method involves the insertion of bipolar needles in parenchymal tumors. The thermal tissue damage depends on both the temperature reached and the duration. The most frequent use is for unresectable hepatic tumors (metastases, hepatoma) or for those with a c…

[Local recurrence following hepatic radiofrequency ablation: diagnosis and treatment].

BACKGROUND: Radiofrequency ablation (RFA) is an established treatment in irresectable malignant liver disease. The most severe constraint is re-occurrence at site of ablation. Whereas factors influencing local recurrence rates have been determined, little is known about the timespan within local recurrence (LR) is to be expected, and further treatment options. PATIENTS AND METHODS: In the presented trial, RFA was performed using two different types of monopolar devices. All procedures were conducted under general anesthesia. Follow-up examinations took part after 3, 6, 12 months and annually.…

A complicated postsurgical echinococcal cyst treated with radiofrequency ablation.

Surgery of hydatid cysts is often complicated with intrabiliary rupture (IBR), which if not recognized may lead to biliary fistula with rather high rates of morbidity and mortality. We report our experience with the application of radiofrequency (RF) ablation for the treatment of an operated hepatic echinococcal cyst which was complicated with biliocystic communication and cysteocutaneous fistula with bile leakage. RF ablation was performed under CT guidance into the remaining cyst through the cutaneous fistula. Since ablation of the cyst and the fistula the patient has been asymptomatic.

Radiofrequency ablation of a "concealed" Mahaim-type accessory pathway.

We report a case of supraventricular tachycardia associated with left bundle branch block due to a slowly conducting right sided accessory pathway. Preexcitation of the ventricles could not be demonstrated during sinus rhythm or incremental atrial pacing but its presence was confirmed during antedromic tachycardia with critically timed atrial extrastimuli. The pathway was mapped during tachycardia and successfully ablated.
