INTRODUCTION: The degenerative pathology in Parkinson's disease (PD), multisystem atrophy (MSA) and progressive supranuclear palsy (PSP) have different spatial distribution in the brainstem. Our aim was find out in what way this pathology affects brainstem vestibular reflex pathways in each of these diseases.
METHODS: We studied 30 patients: 15 with PD, 9 with PSP (Richardson syndrome) and 6 with MSA (cerebellar) and compared their results with results from 37 age-matched healthy controls (HC). Motor functions of the patient groups were evaluated with the Unified Parkinson's Disease Rating Sc

The visual system interacts synergistically with the vestibular system. A normally functioning vestibulo-ocular reflex is necessary but not sufficient for optimum visual acuity during head motion. Studies of dynamic visual acuity, the acuity achieved during relative motion of visual targets or of the observer, indicate that motion of images on the retina markedly compromises vision. The vestibulo-ocular reflex normally provides a substantial measure of stabilization of the retina during head movements, but purely vestibular compensatory eye movements are not sufficiently precise for optimal vi

This is a very brief update on the major papers since August 1998. Unilateral vestibular loss causes oculomotor, postural and sensory symptoms, all of which would be appropriate responses in a healthy person to a strong maintained angular and linear acceleration stimulus directed towards the healthy side. Within hours or days these static symptoms (so called because they are present without any externally imposed vestibular stimulation) reduce, and their progressive disappearance is called 'vestibular compensation'. However, careful testing with natural vestibular stimuli shows that the dynami

We found abnormal supranuclear ocular or lid motility in all of 37 patients with Lytico-Bodig (amyotrophic lateral sclerosis/parkinsonism-dementia complex). Twenty-one patients had pursuit paresis, 18 abnormal vestibulo-ocular reflex (VOR) cancellation, 15 abnormal convergence, 13 abnormal optokinetic nystagmus (OKN), 12 conjugate gaze limitation, nine nystagmus, nine saccadic paresis, and six abnormal fixation. Lid abnormalities included glabellar hyperreflexia in 21, involuntary levator inhibition in three, and blepharospasm in two. Earlier reports have indicated infrequent ocular disturbanc

Recent investigations comparing the postural responses of normals and patients with bilateral peripheral vestibular deficit to a sudden backwards tilt of the support surface about the ankle joints have permitted an objective definition of the timing and amplitude of vestibulo-spinal reflex (VSR) EMG responses in ankle and neck muscles. The muscle contractions result in a sway stabilizing action correcting for induced body tilt, whose effect may be observed in ankle torque, and body and head angular acceleration records. The same techniques were used to monitor VSR responses of 10 patients with

Background/Objectives: Vestibular schwannoma (VS) is a benign cerebellopontine angle tumor causing audiological and vestibular symptoms. This pilot study aimed to describe the application of video Head Impulse Test (vHIT) in the diagnosis and follow-up of patients with unilateral VS treated surgically. The objective was to describe a detailed interpretation of vHIT-not only numerical parameters such as gain and corrective saccades, but also a visual analysis of vHIT curves. Methods: The results were presented in four cases for better understanding and more straightforward explanation. The pati

BACKGROUND: Unsteadiness during standing and walking is a frequent complaint of patients with polyneuropathy (PNP).
OBJECTIVE: To determine whether balance disorders in patients with PNP may be caused by reduced proprioceptive input from the feet alone or whether impaired vestibular input, resulting from involvement of the vestibular nerve, can be an additional factor.
METHODS: A total of 37 patients (mean age 65 years +/- 12 SD; 12 women) with electrodiagnostically confirmed PNP (predominantly axonal: 18; predominantly demyelinating: 19) underwent horizontal search-coil head-impulse testing

Vestibulo-ocular compensation following vestibular deafferentation was investigated in 26 acoustic neuroma patients following tumor removal and in 5 Menière's disease patients following vestibular nerve section by using sinusoidal harmonic acceleration testing. All three test parameters (phase lead, gain, and asymmetry), when averaged, shifted significantly on the first postoperative test (average 0.4 months after operation). A marked progression in central compensation with gain returning to the range of normal control values was seen in the tests performed 1 to 6 months (average 2.6 months)

The cervico-ocular (COR) and active and passive vestibulo-ocular reflexes (VOR) were measured in seven patients with spasmodic torticollis (ST) and six normal controls. The COR was found to be weak or absent in both groups. The VOR gain was similar in the two groups but five patients had a significant asymmetry of the response. There was no evidence of abnormal cervico-vestibular interaction during active head rotation. The study suggests that the VOR asymmetry frequently found in ST cannot be explained on the basis of an abnormal cervical input.

BACKGROUND: Self-motion misperception has been observed in vestibular patients during asymmetric body oscillations. This misperception is correlated with the patient's vestibular discomfort.
OBJECTIVE: To investigate whether or not self-motion misperception persists in post-ictal patients with Ménière's disease (MD).
METHODS: Twenty-eight MD patients were investigated while in the post-ictal interval. Self-motion perception was studied by examining the displacement of a memorized visual target after sequences of opposite directed fast-slow asymmetric whole body rotations in the dark. The dif

Fifteen panic disorder patients with prominent dizziness underwent audiologic, caloric, and vestibuloocular reflex activity testing and were compared with normal controls. There were no abnormalities detected on audiologic and caloric tests. Compared with normal controls, panickers with dizziness showed a greater discrepancy between eye and head movements on vestibulo-ocular reflex activity in the dark. Panickers with prominent dizziness did not differ from a second control group of panickers with severe heart palpitations on a number of psychological tests. The results did not support the hyp

The neuro-otological findings are described in 3 unrelated patients who had xeroderma pigmentosum. All had impaired hearing. Routine audiometric assessment suggested that the hearing loss was of cochlear origin; brainstem evoked potentials in 2 patients and electrocochleography in 1 support this conclusion. Two adult patients had a supranuclear ophthalmoplegia. Vestibular function was mildly deranged and visual suppression of the vestibulo-ocular reflex impaired.

Vestibular evoked myogenic potentials (VEMPs) are now widely used for the noninvasive assessment of vestibular function and diagnosis in humans. This review focuses on the origin, properties, and mechanisms of cervical VEMPs and ocular VEMPs; how these reflexes relate to reports of vestibular projections to brain stem and cervical targets; and the physiological role of (otolithic) cervical and ocular reflexes. The evidence suggests that both VEMPs are likely to represent the effects of excitation of irregularly firing otolith afferents. While the air-conducted cervical VEMP appears to mainly a

PURPOSE: The aim of this study was to devise a novel test for assessing the cervico-ocular reflex (COR) system via head vibration (termed h-COR) or neck vibration (termed n-COR) method.
MATERIALS AND METHODS: Thirteen patients with complete loss of bilateral vestibulo-ocular reflex (VOR) showing oscillopsia were assigned to Group A, while 13 patients with bilateral VOR loss but no oscillopsia were Group B. Another 13 healthy elderly served as a control. The COR test was performed via modifying the ocular vestibular-evoked myogenic potential (oVEMP) test by tapping at the forehead with head ro

An approximately 7-week-old female American Bulldog was presented for head injury and seizures. The dog was obtunded with absent left-sided postural reactions, absent left palpebral reflex, delayed vestibulo-ocular reflex bilaterally, and calvarial hyperesthesia. CT revealed a fragmented, depressed fracture of the right frontal bone, with an associated abscess and evidence of raised intracranial pressure. The dog initially declined on medical management. Following a CT-guided drainage procedure, the dog improved and remained clinically well twenty months later. This is the first report of mini

This chapter provides a detailed overview of the vestibular system's anatomy, physiology, and clinical relevance, emphasizing how inner-ear sensory structures and brainstem pathways interact to maintain balance, gaze stability, and spatial orientation. It begins with the peripheral vestibular apparatus, describing the semicircular canals and otolith organs in detecting angular and linear accelerations. These signals are transmitted via the vestibulocochlear nerve to the vestibular nuclei at the pontomedullary junction, where they integrate with visual and somatosensory inputs to drive coordina

BACKGROUND: Head rotation produces a vestibulo-ocular reflex (VOR). In horizontal rotation, not only lateral semicircular canals but also posterior semicircular canals are stimulated, because posterior canals cupulae are not horizontal in the sitting position. Therefore, theoretical nystagmus is horizontal and torsional. Convection of endolymph does not occur, because the centre of head rotation is a dens of the second cervical vertebra, not the center of lateral canal. Although per-rotational nystagmus is a result of VOR, whether it could be explained by the movement of cupula remains undeter
