Diagnosis and management of postrefractive surgery ectasia.

Postrefractive surgery ectasia is a serious, sight-threatening complication seen after the following procedures: laser in situ keratomileusis, photorefractive keratectomy, small-incision lenticule extraction, radial keratotomy, and/or arcuate keratotomy. Specific risk factors may include age, corneal thickness, degree of refractive error, corneal topographic changes including irregular astigmatism, percent tissue ablation, and residual stromal bed. Biomarkers may be a new option to help indicate who is at greatest risk for ectasia. Visual aids including spectacles or contacts lenses are often

Management of extreme ametropia after penetrating keratoplasty: a series of surgical procedures for high myopia and astigmatism.

A series of surgical interventions - relaxing corneal incisions, intraocular lens, and intrastromal rings - were used to correct a case of extreme ametropia in a thin cornea after a penetrating keratoplasty in an 18-year-old patient who presented with a -10.25 -8.50 × 120 preoperative refraction and 20/200 best-corrected visual acuity (BCVA). After a series of surgical procedures, the patient's BCVA in his left eye improved to 20/30 with +0.50 -1.00 × 170, the slit lamp examination showed no significant findings, and the patient's visual complaints disappeared. At the 1-year follow-up, the BCV

Five year follow-up of epikeratophakia in children.

Epikeratophakia alters the anterior curvature of the cornea by the addition of a machine-carved donor lenticule. Since March 1980, 88 patients under eight years of age have had epikeratophakia, with at least six months of follow-up. Eighty percent of the original surgeries were successful; some failed grafts were replaced successfully, so that in all, 89% of the patients had successful grafts. The average increase in curvature of the cornea was 14.7 diopters, and the average spectacle overcorrection was +0.56 diopters. In these growing eyes, we documented a myopic shift of 1.5 diopters per yea

Assessment of Human Corneal Biomechanical Properties After Refractive Surgery With Inflation Test Using Optical Coherence Tomography.

PURPOSE: To evaluate the biomechanical properties of the cornea after refractive surgery using an inflation test combined with optical coherence tomography (OCT). METHODS: Nine human donor corneas were divided into two groups: healthy (control) and treated (divided into three subgroups: photorefractive keratectomy [PRK], laser in situ keratomileusis [LASIK], and small incision lenticule extraction). Refractive treatments were applied to simulate -4.00 diopters of myopia correction. The corneas were mounted on an artificial anterior chamber, and an inflation test was performed using a custom s

An analysis of treatment of congenital glaucoma by goniotomy.

We used goniotomy as the initial surgical procedure in 34 patients (50 eyes) with congenital glaucoma. An overall success rate of 88% was achieved with one or more goniotomies. The age at disease onset, level of intraocular pressure, and corneal diameter did not significantly influence the surgical outcome. The refractive error proved to be a valuable indicator of operative success or disease progression. A satisfactory visual outcome was associated with treatment by goniotomy.

Survey of Ehlers-Danlos Patients' ophthalmic surgery experiences.

BACKGROUND: Ehlers-Danlos Syndrome (EDS) is a rare disease affecting approximately 1 in 5,000 people. Although ophthalmic conditions associated with EDS have been described, little data exist concerning ophthalmic surgical outcomes experienced by EDS patients. METHODS: Patients with EDS were surveyed via the EDS Society and asked about their ophthalmic surgical experiences including procedure, complications, and the timing with respect to receiving the EDS diagnosis. Complications were confirmed as such by subspecialists. RESULTS: Of 579 respondents, 467 reported confirmed EDS, and 112 of th

[Applications of anterior segment optical coherence tomography in corneal refractive surgery].

Anterior segment optical coherence tomography (AS-OCT) has the characteristics of non-contact, noninvasive, high sensitivity, and repeatability, and offers high-resolution in vivo imaging of the structures of the anterior eye segment. It can be used in the diagnosis and assistance of conditions related to the anterior eye segment. This review provides an update on the research and clinical applications of AS-OCT in corneal refractive surgery, including preoperative keratoconus screening, intraoperative real-time visualization of corneal structures, postoperative corneal evaluation, and managem

A new trifocal corneal inlay for presbyopia.

Corneal inlays (CIs) are the most recent surgical procedure for the treatment of presbyopia in patients who want complete independence from the use of glasses or contact lenses. Although refractive surgery in presbyopic patients is mostly performed in combination with cataract surgery, when the implantation of an intraocular lens is not necessary, the option of CIs has the advantage of being minimally invasive. Current designs of CIs are, either: small aperture devices, or refractive devices, however, both methods do not have good performance simultaneously at intermediate and near distances i

[Corneal refractive surgery].

During the past 8 years surgery of the cornea for refractive errors has risen to the rank of an independent, specialized discipline. It covers a range of techniques devised to correct or reduce all types of ametropia. Radial keratotomv is the operation most frequently performed to correct mild or moderate myopia from -1 to -6 diopters. Keratomileusis with or without freezing is a more complex procedure used to correct severe myopia of -8 to -16 diopters. Epikeratophakia for myopia or hypermetropia has a wider range of correction, ranging from 5 to 30 diopters. Each of these techniques has its

Visual and Refractive Outcomes of Corneal Refractive Surgery for Hyperopic and Mixed Astigmatism: A Systematic Review and Meta-Analysis.

TOPIC: To evaluate the visual and refractive outcomes of corneal refractive surgery (PRK, LASIK, and SMILE) for hyperopic and mixed astigmatism. The clinical question addressed was: in adults with hyperopic or mixed astigmatism, how effective and safe are modern corneal refractive procedures, without alternative surgical comparators, in reducing refractive cylinder and improving uncorrected distance visual acuity? CLINICAL RELEVANCE: Hyperopic and mixed astigmatism are less predictable to correct than myopic errors due to biomechanical and ablation-profile complexity. Historically, these trea

Some aspects of wave aberrations of the human eye and supervision: a review.

Supervision is defined by a visual acuity of 20/10 or 20/8 and may be attained by custom-correcting the aberrations of higher order of the human eye. Higher order aberrations are those aberrations which are left in the eye after having corrected lower order aberrations, i.e., defocus (myopia, hypermetropia) regular astigmatism, and which can be corrected by ordinary spectacle lenses or contact lenses. Higher order aberrations are found to a higher or lesser degree in normal or pathological human eyes and in eyes having undergone conventional corneal surgery. According to custom keratorefractiv

[Current state of wavefront guided corneal surgery to correct refraction disorders].

The present review describes the current status of wavefront-guided corneal surgery and up-to-date results. Wavefront-guided LASIK procedures performed to date achieved uncorrected vision of i.o or better in a larger number of patients than with conventional LASIK surgery, but the "eagle eye" has remained the exception. In part of the patients reductions of higher order aberrations could be obtained, but in the majority of ca-ses an increase was observed. This increase was however less than with conventional LASIK surgery. Correction of higher order aberrations has not yet been perfected since
