A 6-year follow-up of the effect of graft site on strength, stability, range of motion, function, and joint degeneration after anterior cruciate ligament reconstruction: patellar tendon versus semitendinosus and Gracilis tendon graft.

BACKGROUND: The choice of graft material for anterior cruciate ligament reconstruction remains controversial. Despite the need for well-cont … stability was restored to all patients other than to the 2 hamstring graft recipients who suffered reinjuries. The KT-1000 arthrometer side-to-side differences were similar in the patellar tendon (1.9 mm) and hamstring tendon (2.0 mm) groups but were significantly greater than that of uninjured control subjects (P < .001). There were no significant strength …

Impact of Psychological Factors on Rehabilitation After Anterior Cruciate Ligament Reconstruction: A Systematic Review.

CONTEXT: Anterior cruciate ligament (ACL) tears are a common orthopaedic injury, and the incidence of ACL reconstruction (ACLR) continues to increase. Current clinical practice guidelines (CPGs) recognize the role of psychological factors in rehabilitation, but patient-reported outcome measures (PROs) and psychological readiness are rarely incorporated into rehabilitation. OBJECTIVE: The purpose of this review was to highlight the importance of psychological health after ACL injury, understand the current metrics used to monitor psychological recovery, and outline how psychological recovery c

Exploring the role of artificial intelligence in anterior cruciate ligament injuries in high-performing & elite athletes.

Anterior cruciate ligament (ACL) injuries dramatically impact athletes and sporting organisations. Artificial Intelligence (AI) driven systems, such as those leveraging machine learning (ML) algorithms into wearable devices are showing promise in orthopaedic surgery from high level to the elite athlete population. The application of AI solutions has the potential to revolutionise injury risk assessment in elite sport by identifying abnormal movement patterns and providing real-time feedback during training with the potential to identify early, players at risk of career-ending injuries. This re

Return to Play Criteria After Adult Lumbar Spinal Fractures: A Review of Current Literature and Expert Recommendations.

PURPOSE OF REVIEW: Understanding the management of lumbar spinal fractures and return to play (RTP) criteria is an essential component of care for adult athletes. Appropriate management of lumbar spinal fractures must balance minimizing time away from physical activity while also minimizing risk of reinjury. The purpose of this review is to summarize current recommendations on lumbar spinal fracture management and RTP guidelines and to provide expert opinion on areas of discrepancy in the field. RECENT FINDINGS: There is a paucity of high-level evidence on the management and return to play cr

Association of the Graft Size and Arthrofibrosis in Young Patients After Primary Anterior Cruciate Ligament Reconstruction.

INTRODUCTION: This study investigated the association of graft-related surgical factors and patient characteristics with the odds of arthrofibrosis after primary anterior cruciate ligament reconstruction (ACL-R). METHODS: A retrospective case-control study assessed consecutive patients who underwent primary ACL-R in one tertiary pediatric hospital. Each arthrofibrosis case was matched to three controls for sex, calendar year, and age at the time of ACL-R, as well as the primary surgeon. Conditional multivariable logistic regression assessed the independent association of graft diameter, time

Acute treatment of inversion ankle sprains: immobilization versus functional treatment.

Inversion ankle sprains are one of the most common injuries in sports. Although these injuries are often considered minor, they can lead to persistent disability in athletes. We conducted a systematic review of the literature to evaluate the effect of immobilization versus early functional treatment on time to return to preinjury activity after inversion ankle sprain. Residual subjective instability, recurrent injury, and patient satisfaction were secondary outcomes. A systematic review identified 9 randomized controlled trials. Return to preinjury activity was less with early functional treat

Factors important in arterial narrowing.

BACKGROUND: Vascular narrowing in hypertension presents conflicting hypotheses about structural narrowing, vasconstriction, volume expansion, contractile function and other issues more obvious to the physiologist than the cell biologist. Even the cell biology in the injured large arteries used to study atherosclerosis is surprisingly complex. The key issue is that changes in mass, here mainly intimal mass or atherosclerotic plaque, correlate poorly with loss of lumen caliber. An analogy to remodeling of microvessels begs the issue of mechanism. RECENT STUDIES: To explore this, we have focused

Occupational cumulative trauma disorders of the upper extremity.

The umbrella term cumulative trauma disorders (CTDs) (also known as repetitive strain injuries, overuse syndromes, and repetitive motion disorders) covers a number of similar conditions arising from overuse of the joints or soft tissues of the upper extremity. Occupational CTDs have become a common problem in the workplace. These disorders are costly to the employer, the worker, and society in terms of time lost from work and resulting disability. Within the past decade, occupational therapists and physical therapists specializing in rehabilitation of work-related musculoskeletal injuries have

Multispecialty perspective on intradural disc herniation: diagnosis and management - A case report.

BACKGROUND: Intradural disc herniation (IDH) is a very rare and challenging diagnosis, with an estimated incidence of less than 1.5%. The pathogenesis of IDH remains uncertain. Definitive management remains surgical; however, some cases may initially be managed non-surgically. CASE: A middle-aged male with presented with acute right-sided lumbar radiculopathy following heavy lifting. History was significant for prior lumbar disc herniation managed non-surgically. Lumbar MRI demonstrated a large disc herniation. The patient was initially treated non-surgically with epidural steroid injections.

Fluid Biomarkers of Pediatric Mild Traumatic Brain Injury: A Systematic Review.

Public concern is growing about the short- and long-term effects of pediatric mild traumatic brain injury (mTBI). This concern is amplified because pediatric mTBI has the potential to go undiagnosed in acute care settings, placing children at increased risk for reinjury prior to complete recovery. The management of mTBI can be particularly challenging due to the lack of validated biomarkers that clinicians can use to objectively diagnose pediatric mTBI, predict risk for prolonged recovery, or demonstrate mTBI recovery. Fluid-based biomarkers have drawn increased attention as an objective measu

Submucosal Gland Myoepithelial Cells Are Reserve Stem Cells That Can Regenerate Mouse Tracheal Epithelium.

The mouse trachea is thought to contain two distinct stem cell compartments that contribute to airway repair-basal cells in the surface airway epithelium (SAE) and an unknown submucosal gland (SMG) cell type. Whether a lineage relationship exists between these two stem cell compartments remains unclear. Using lineage tracing of glandular myoepithelial cells (MECs), we demonstrate that MECs can give rise to seven cell types of the SAE and SMGs following severe airway injury. MECs progressively adopted a basal cell phenotype on the SAE and established lasting progenitors capable of further regen

The Influence of Visual Disruption and Dual-Task Conditions on Single-Leg Vertical Hop Performance 1 to 5 Years After Anterior Cruciate Ligament Reconstruction.

BACKGROUND: Current return-to-sport (RTS) testing after anterior cruciate ligament reconstruction (ACLR) lacks the visual and cognitive demands consistent with most sporting environments. Implementing RTS tests that challenge these systems may better assess RTS readiness, potentially reducing reinjuries. This study assessed the effects of additional visual-cognitive demands on single-leg vertical hop (VH) performance. HYPOTHESIS: Visual-cognitive conditions would result in reduced hop heights, success rates, and ground reaction force (GRF) impulses for both healthy controls and participants a…
