Branch retinal artery occlusion secondary to prepapillary arterial loop.

PURPOSE: To report a case of branch retinal artery occlusion and prepapillary loop in a 10-year-old girl. METHODS: Case report with funduscopic and fluorescein angiography imaging. RESULTS: A 10-year-old girl presented with a history of the sudden onset of a superior visual field defect in her right eye. Fundus findings were consistent with thrombosis in a prepapillary arterial loop causing an inferior branch retinal artery occlusion. CONCLUSION: Bilateral congenital prepapillary vascular loops are rare, and are usually asymptomatic. However, they can be complicated by vitreous hemorrhage a…

Intra-arterial fibrinolytic therapy in central retinal artery occlusion.

We have treated 23 patients with central retinal artery occlusion by intra-arterial fibrinolysis, a method already in use for treatment of thromboembolic occlusion of the cerebral arteries. Fibrinolysis was carried out through a microcatheter placed in the origin of the ophthalmic artery. When ophthalmic artery cannot be catheterised, treatment can be carried out indirectly via the maxillary-ophthalmic anastomoses. In 18 cases urokinase was used in doses of 200,000-1,200,000 units; in 5 patients recombinant tissue plasminogen activator was used. Six patients showed marked improvement or total …

Retinal arterial occlusion with multiple retinal emboli and carotid artery occlusion disease. Haemodynamic changes and pathways of embolism.

OBJECTIVE: To introduce a special subgroup, retinal artery occlusion (RAO) with multiple emboli, which is highly associated with ipsilateral carotid artery occlusion disease (CAOD). METHODS AND ANALYSIS: This is a cohort study. Cases of RAO with multiple retinal emboli were consecutively enrolled. All patients underwent at least one of the carotid/cerebral evaluations: carotid arteriography, orbital/carotid colour Doppler ultrasonography and CT angiography to demonstrate haemodynamic changes and to discuss possible mechanisms and pathways of the emboli. RESULTS: Among 208 RAO eyes, 12 eyes (…

Symptomatic branch retinal artery occlusion as a presenting sign of carotid artery occlusion.

A man in his late 60s with a history of arterial hypertension, dyslipidaemia and a cerebrovascular accident 20 years ago presented to the emergency department with sudden-onset blurred vision in his right eye, accompanied by pain, dizziness, sweating and high blood pressure (160/110 mmHg). Ophthalmic examination revealed a decreased visual acuity in the right eye (less than 6/60), with best corrected visual acuity of 6/7.5. Fundoscopy showed retinal cholesterol emboli and ischaemic changes. Carotid duplex ultrasound indicated possible stenosis of the internal carotid artery. The patient was st

RETINAL ISCHEMIC PERIVASCULAR LESIONS IN THE FELLOW EYES OF PATIENTS WITH CENTRAL RETINAL ARTERY OCCLUSION.

PURPOSE: To evaluate the prevalence and characteristics of retinal ischemic perivascular lesions (RIPLs) in the clinically unaffected fellow eyes of patients with unilateral central retinal artery occlusion (CRAO) using spectral domain optical coherence tomography. METHODS: This retrospective case-control study included 39 fellow eyes of patients with CRAO and 57 age-, sex-, and hypertension-matched healthy controls. Macular spectral domain optical coherence tomography scans were assessed for the presence, number, and morphology of RIPLs, classified as narrow (<300 µ m, peaked apex) or wide (…

Retinal Degeneration in a Murine Model of Retinal Ischemia by Unilateral Common Carotid Artery Occlusion.

Retinal degeneration is a progressive retinal damage in ocular vascular diseases. There are several reasons for this, such as occlusion of arteries or veins, diabetic retinopathy, or hereditary retinal diseases. To study pathological mechanisms of retinal degeneration, it is required to develop experimentally reproducible and clinically relevant models. In our previous studies, we developed a murine model of retinal hypoperfusion by unilateral common carotid artery occlusion (UCCAO) which mimics the pathophysiology of ocular ischemic syndrome (OIS) in humans, and described broad pathological m

[Intra-retinal vein cannulation without tissue-type plasminogen activator for hemi-central retinal artery occlusion].

A 19-year-old woman presented sudden visual loss in the left eye for 1 day. Ophthalmic examination showed superior hemi-retinal whitening with thickening of the inner retinal layer and shadowing of the outer retina layer and delayed perfusion of the superior hemi-retinal artery and cilioretinal artery. We performed surgery at 34 hours after the onset. After vitrectomy, a 48G micro-needle was connected to the 1-cc syringe of the viscous fluid control unit of the vitrectomy machine. The needle tip was inserted into the lumen of the superior peripapillary branch retinal vein. Then balanced salt s…

Presumed toxoplasmic central retinal artery occlusion and multifocal retinitis with perivascular sheathing.

Central retinal artery occlusion (CRAO) and multifocal retinitis with perivascular sheathing are rare in ocular toxoplasmosis. We report a case of toxoplasmic CRAO and multifocal retinitis with perivascular sheathing. A healthy 83-year-old male developed left panuveitis. Funduscopic examination of the left eye showed a swollen optic disc and sheathing of the retinal artery with a dense vitreous haze and a white retinal lesion. Serum anti-toxoplasma antibodies were positive in a latex agglutination assay. Vitrectomy was performed to improve visualization of the retinal lesions and for examinati…
