Systemic sclerosis: Characterizing candidate retinal and choroidal imaging features using optical coherence tomography.

OBJECTIVES: Systemic sclerosis is a complex autoimmune disease characterized by vasculopathy and fibrosis, capable of affecting virtually any organ system, including the eyes. Optical coherence tomography is a non - invasive imaging technique that enables high-resolution cross-sectional visualization of ocular structures. This study aimed to characterize candidate retinal imaging features in systemic sclerosis patients using Optical Coherence Tomography. METHODS: A total of 41 systemic sclerosis patients (10 males, 31 females; mean age: 58.7 ± 11.7 years) and 38 age-matched healthy controls (

A comprehensive review on early detection of drusen patterns in age-related macular degeneration using deep learning models.

Age-related Macular Degeneration (AMD) is a leading cause of visual impairment and blindness that affects the eye from the age of fifty-five and older. It impacts on the retina, the light-sensitive layer of the eye. In early AMD, yellowish deposits called drusen, form under the retina, which could result in distortion and gradual blurring of vision. The presence of drusen is the first sign of early dry AMD. As the disease progresses, more and larger deposits develop, and blood vessels grow up from beneath the retina leading to leakage of blood, that damages the retina. In advanced AMD, periphe

Optical coherence tomography in papilledema: what am I missing?

BACKGROUND: Grading of papilledema severity is subjective and based on monocular fundus features of the optic nerve. Interobserver agreement on grading the severity of papilledema is poor among expert observers, even using well-defined criteria such as the Frisen scale, which is a non-continuous ordinal scale of grading. Furthermore, non-expert clinicians often find it difficult to properly view and interpret features of the optic nerve using ophthalmoscopy, which can lead to failure to diagnose papilledema in non-ophthalmologic care settings. This may delay treatment, which can result in visi

Alterations in Extracellular Matrix/Bruch's Membrane Can Cause the Activation of the Alternative Complement Pathway via Tick-Over.

Given the complex etiology of age-related macular degeneration (AMD), treatments are developed to target intermediate/late stages of the disease. Unfortunately, the design of therapies for early stages of the disease is limited by our understanding of the mechanisms involved in the formation of basal deposits and drusen, the first clinical signs of AMD. During the last decade, the identification of common and rare alleles in complement genes as risk AMD variants in addition to the presence of active complement components in basal deposits and drusen has provided compelling evidence that the co

Unique optical coherence tomographic features in age-related macular degeneration.

Age-related macular degeneration is a major cause of blindness worldwide characterized by the presence of drusen and leading to retinal pigment epithelium and outer retinal changes in advanced stages. Approximately 10% of eyes with age-related macular degeneration develop neovascular complications and present with retinal or sub-retinal pigment epithelium exudation, hemorrhage, or both. Recent advances in imaging techniques, especially optical coherence tomography (OCT), help in early identification of disease and guide various treatment decisions; however, not all signs are suggestive of ongo

Juxtapapillary Choroidal Neovascular Membrane as a Complication of Optic Disc Drusen: Multimodal Imaging With Swept Source-Optical Coherence Tomography and Optical Coherence Tomography Angiography.

A 55-year-old Caucasian man presented to the neuro-ophthalmology department for follow-up evaluation due to long-standing bilateral optic nerve head drusen (ONHD). On examination, the BCVA was 20/20-2 in both eyes. Dilated fundus examination revealed extensive ONHD in both eyes, retinal hemorrhages, exudates inferonasal to the macula, and macular edema inferotemporal to the disc margin. Automated visual field testing revealed generalized depression in both eyes. Late phase leakage was observed on fluorescein angiography (FA). Optical coherence tomography angiography identified a small juxtapap

MFRP variant results in nanophthalmos, retinitis pigmentosa, variability in foveal avascular zone.

BACKGROUND: Membrane frizzled-related protein (MFRP) plays a critical role in ocular development. MFRP mutations are known to cause nanophthalmos and, in some cases, retinitis pigmentosa, foveoschisis, and/or optic nerve head (ONH) drusen. The broad clinical spectrum of MFRP mutations necessitates further investigation of specific genotype-phenotype relationships. MATERIALS AND METHODS: We reviewed ophthalmologic and genetic medical records of two affected siblings and one unaffected sibling. RESULTS: Genetic testing revealed variants MFRP c.855T>A, p.(Cys285*) and MFRP c.1235T>C, p.(Leu412P

Long-term fundus changes in acquired partial lipodystrophy.

We describe long-term fundus changes in a patient with partial lipodystrophy (PL). Retinal pigment alterations, drusen and subretinal neovascularisation were seen without evidence for membranoproliferative glomerulonephritis. Fundus alterations similar to those seen in age-related macular degeneration can occur at an earlier age in patients with PL, even without renal disease. Dysregulation of an alternative complement pathway with low serum levels of C3 has been implicated as a pathogenetic mechanism.

Degenerative lamellar foramen post drusenoid detachment of the retinal pigment epithelium.

The macular hole is a prevalent pathology whose types and causes are due to various reasons. Sometimes it can coexist with other diseases such as age-related macular degeneration. Its pathophysiological basis may have a common relationship with the degenerative lamellar foramen. Optical coherence tomography plays a fundamental role in understanding its genesis. In this work, we present the case of a 60-year-old patient with age-related macular degeneration, who consults due to decreased visual acuity in the left eye. The ophthalmological examination revealed a drusenoid detachment of the retin
