BACKGROUND: Percutaneous rhizotomy is a group of techniques used to treat trigeminal neuralgia. Radiofrequency thermocoagulation (RF) and Chemical Rhizotomy (CR) using glycerol are among the most frequently used methods. We have recently refined Ethanol Rhizotomy (ER) under Digital Subtraction Angiography (DSA) guidance.
OBJECTIVE: A descriptive, retrospective study was conducted to compare our refined ER and RF ablation in patients with trigeminal neuralgia in terms of long-term pain relief and side effects.
METHOD: Between 2012 and 2014, 33 patients with typical trigeminal neuralgia were e…

BACKGROUND: Neurovascular compression (NVC) in patients with trigeminal neuralgia (TN) can be a factor of treatment outcome, especially in microvascular decompression and stereotactic radiosurgery. No such effect has been reported in percutaneous radiofrequency rhizotomy (RF). This study aims to investigate whether NVC affects the efficacy of RF in patients with TN.
METHODS: We retrospectively reviewed patients with TN who received RF in our institution. Pretreatment magnetic resonance imaging was performed in every patient, and the presence of NVC was reviewed independently by two physicians…

Background A selective dorsal rhizotomy (SDR) is employed to treat spastic cerebral palsy. The surgical techniques and patient care protocols vary among hospitals. One of the variations is the age cut-off for SDR. We have been advocating SDR to be performed early - especially at ages 2 and 3. With this study, we are reporting the feasibility and parent-reported surgical outcomes of receiving SDR at an early age for the treatment of spastic diplegia. Objectives Our aim is to examine the safety and benefits of receiving SDR at the ages of 2 and 3 for the treatment of spastic diplegia. Methods Th…

INTRODUCTION: Infantile cerebral palsy is a well-known condition, the prevalence of which has varied only slightly over the years. The most common subtype is spastic diplegia, and spasticity is the most disabling symptom. Its treatment involves a multidisciplinary intervention that includes rehabilitation, the use of drugs, and orthopaedic and nervous system surgery, where selective dorsal rhizotomy is a prominent procedure.
AIM: To present a thorough review of the use, indication and long-term consequences of selective dorsal rhizotomy.
DEVELOPMENT: It is a minimally invasive procedure aime…

Children with spastic diplegia from cerebral palsy (CP) experience measurable improvement in their spasticity and motor function following selective dorsal rhizotomy (SDR). The role of this operation in the treatment of other spasticity causes is less well defined. A literature review was undertaken to survey outcomes from SDRs performed outside the CP population. Multiple sclerosis was the most common diagnosis found, accounting for 74 of 145 patients described. Selective dorsal rhizotomies have also been reported in patients with traumatic brain and spinal cord injuries, ischemic and hemorrh…

BACKGROUND: Degenerative changes, such as osteoarthritis, often lead to facet joint syndrome (FJS). Standard treatments include physical therapy, nerve blocks, and radiofrequency rhizotomy. Full-endoscopic lumbar rhizotomy enables direct visualization and precise ablation of the medial branch nerves. Recent studies have shown significant reductions in pain and disability. This study aimed to evaluate the efficacy and safety of full-endoscopic rhizotomy in patients with degenerative lumbar FJS, using pooled data from clinical studies reporting postoperative outcomes.
METHODS: A systematic lite…

OBJECTIVE: Percutaneous radiofrequency (RF) rhizotomy is one of the most effective minimally invasive treatments for trigeminal neuralgia (TN) that is unresponsive to medication. Although it is commonly used, the impact of intraoperative parameters, especially CSF outflow, prelesion impedance, and total thermal dose, on long-term pain control remains unclear. This study aimed to evaluate the relationship between these procedural factors and postoperative pain reduction.
METHODS: A retrospective analysis of a prospectively maintained database was performed of patients who underwent percutaneou…

PURPOSE: Intraoperative bradycardia is a recognized complication during radiofrequency (RF) rhizotomy for trigeminal neuralgia, primarily related to the trigeminal-cardiac reflex. Dexmedetomidine is commonly used for its sedative and analgesic properties during these procedures; however, its hemodynamic impact has not been well characterized. This study aimed to evaluate the association between dexmedetomidine use and intraoperative bradycardia and to identify relevant clinical risk factors.
PATIENTS AND METHODS: This retrospective observational study included 517 patients who underwent RF rh…

OBJECTIVE: Initial medical management fails in nearly half of patients with trigeminal neuralgia (TN), and patients will seek surgical management with either rhizotomy or microvascular decompression. The impact of preoperative medical management on post-rhizotomy outcomes is unclear. The authors aimed to further evaluate this relationship.
METHODS: In this single-center, retrospective cohort study, the authors recorded clinical and demographic data for patients with TN who underwent first-time rhizotomy at their institution (1995-2023). Univariate logistic regression and Cox regression analys…

BACKGROUND AND OBJECTIVES: Trigeminal neuralgia causes debilitating facial pain. Treatments, including decompression, radiosurgery, and minimally invasive percutaneous procedures such as mechanical compression, are low risk and offer immediate relief, making them a preferred option. Successful percutaneous treatment depends on safely and successfully accessing the foramen ovale. This study evaluated a novel electromagnetic (EM)-guided cannula used for percutaneous trigeminal rhizotomy.
METHODS: The cannula's accuracy and EM neuronavigation compatibility were evaluated in a cadaveric study. Su…

It is known that selective sacral roots rhizotomy is effective for relieving the neurogenic bladder associated with spinal cord injury. The goal of this study is to review the surgical anatomy of the lumbosacral nerve rootlets and to provide some morphological bases for highly selective sacral roots rhizotomy. Spinal cord dissections were performed on five cadavers under surgical microscope. At each spinal cord segment, we recorded the number, diameter and length of the rootlets, subbundles and bundles from the L1 to S2 spinal segments, and the length of the dorsal/ventral root entry zone. Per…

PURPOSE: To identify MRI characteristics that may predict the functional effect of selective dorsal rhizotomy (SDR) in children with bilateral spastic paresis.
METHODS: We performed SDR in a group of 36 patients. The gross motor functioning measure-66 (GMFM-66) was applied before and after SDR. Available cerebral MRIs were retrospectively classified into three diagnostic groups: periventricular leucomalacia (PVL; n = 10), hydrocephalus (n = 2), and normal (n = 6). In patients with PVL, we scored the severity of the MR abnormalities. We compared the changes in the GMFM-66 after SDR in the diag…

BACKGROUND AND OBJECTIVES: To examine the effect of cerebrospinal fluid (CSF) flow during percutaneous retrogasserian glycerol rhizotomy (PRGR) on long term pain relief in patients with trigeminal neuralgia.
METHODS: Eighty-nine patients with trigeminal neuralgia underwent 102 PRGR procedures. PRGR was conducted under fluoroscopy. After the egress of CSF, anhydrous glycerol (0.3-0.4 cc) was injected in the sitting position. In the absence of CSF flow, 0.25 mL 2% lidocaine was injected to elicit hypesthesia in the affected side. Once hypesthesia was elicited glycerol was injected. Patients wer…

Children with cerebral palsy who receive selective dorsal rhizotomy (SDR) for treatment of spasticity may show suprasegmental changes in upper limb function and control of speech musculature. Anecdotal reports suggest that supra-segmental effects may extend to cognitive functions such as attention and language. This study examined the performance of 16 children with spastic diplegic cerebral palsy on tests of visual attention and other cognitive functions 1 day prior to and 6 months following SDR. Children undergoing SDR were compared with 9 children with spastic diplegia, matched for severity…

Microsurgical vascular decompression (MVD) was used for 92 patients with trigeminal root entry zone (REZ) compression. In 2 patients, abnormal nerve roots were sectioned; 5 had extra-axial tumors, and 26 with no apparent REZ pathology had partial sensory rhizotomy (PSR). Mean follow-up was 26 months; 105 patients, including those with tumors, are pain-free. Nine have infrequent discomfort, requiring no medication. Eleven patients had recurrence, 9 within 1 year; 4 on medication and 3 after reoperation are pain-free. Complications were few. No patient died. Although PSR and MVD had equal effica…
