Does sealer extrusion influence endodontic treatment outcomes? Retrospective evidence from root-level and within-tooth analysis.

OBJECTIVES: This retrospective study aimed to evaluate the influence of sealer extrusion on treatment outcomes using a within-tooth comparative design and to identify associated prognostic factors. METHODS: A total of 126 patients who underwent nonsurgical root canal treatment in posterior teeth between 2020 and 2025 were included. Eligible cases had at least one root with sealer extrusion and other roots without extrusion with at least one year follow-up. Treatment outcomes were classified as success or failure using both strict and loose criteria. Comparisons between roots with and without

Endodontic management of an atypical permanent mandibular second molar with one buccal and two lingual roots.

This article describes an unusual location of the orifices and roots in a permanent mandibular second molar with symptomatic irreversible pulpitis and symptomatic apical periodontitis. During its micro-endodontic management, the cone-beam computed tomography evaluation revealed a rare variant of a three-rooted permanent mandibular second molar with four canals. The single buccal root had two canals with Vertucci's Type II pattern and a lingual root dividing into two with a single canal in each root, respectively. Interestingly, the concerned tooth had normal occlusal morphology and alignment i

Bacteremia in conjunction with endodontic therapy.

This study characterizes oral microorganisms believed to have spread from the root canal into the blood stream during and after endodontic therapy of teeth with Asymptomatic apical periodontitis. Microbiological samples were taken under aseptic conditions from the root canal of 26 single-rooted teeth in 26 patients. In the endodontic treatment of 13 of the patients (Group 1), the first 3 reamers, sizes 15, 20 and 25, were used to a level 2 mm beyond the apical foramen. In the other 13 patients (Group 2), the instrumentation ended inside the root canal 1 mm short of the apical foramen. Blood sa

Negotiating and Treating Lateral Canals: Report of 7 Cases.

This is a series of 7 cases in which the operator penetrated lateral canals with instruments. Two teeth presented with irreversible pulpitis, 4 with necrotic pulps, and 1 with previous treatment. Except for the teeth with pulpitis, all the others were associated with apical periodontitis. The main root canal was always relatively straight, with the lateral canals at the middle third of the root. Suggestive images of lateral canal presence were seen on periapical radiographs in five cases. The clinician introduced intentionally small hand instruments in the lateral canal in 5 cases, while the p

Mandibular first premolar with four canals.

A case of endodontic treatment of a mandibular first premolar exhibiting a total of four distinct root canals and four apical foramina is described. This occurrence in mandibular first premolar has rarely been reported in the endodontic literature. Endodontic treatment that considers the anatomic variation of root canal morphology is important to ensure a favorable healing outcome, and its identification could be enhanced by careful examination using a dental operating microscope. Obturation of root canals using a warm vertical compaction technique with a highly-radiopaque root canal sealer, s

Healing Outcomes Following the Treatment of Molars Using Different Root Canal Disinfection Methods: A Prospective Cohort Study.

INTRODUCTION: To address the shortage of clinical outcome studies on contemporary root canal disinfection methods, this prospective cohort study assessed healing of apical periodontitis (AP) after use of the GentleWave (GW) system or Ultrasonic Activation (UA) combined with negative pressure irrigation (NPI). METHODS: Maxillary and mandibular molars with AP (n = 230) were treated with either GW or UA + NPI. A board-certified endodontist performed all procedures following standardized protocols. Patients with follow-up assessments available (n = 177) had radiographs assessed independently by 2

[Irrigants and intracanal medicaments in endodontics].

Modern, biologic root canal therapy should be performed with suitable irrigating solutions and intracanal medicaments. The goal of endodontic treatment is to free the treated tooth from infection and prevent reinfection as thoroughly as possible by means which do not put the organism at risk. In this review of the literature, an evidence-based concept for irrigation and medication of root canal systems is presented. Irrigants and medicaments are discussed with respect to their antimicrobial, tissue-dissolving and endotoxin-decontaminating capacity in relation to their systemic toxicity. Recent…

Prevention and management of external inflammatory resorption following trauma to teeth.

External inflammatory resorption is one of the potential consequences of trauma to the teeth. It occurs when there has been loss of cementum due to damage to the external surface of the tooth root during trauma, plus the root canal system has become infected with bacteria. It is characterized by the radiographic appearance of loss of tooth substance with a radiolucency in the adjacent periodontal ligament and bone. The loss of cementum allows the intracanal bacteria and/or their endotoxins to reach the periodontal ligament more readily and this can lead to the development of the inflammatory r

Uses of calcium hydroxide: an online study guide.

The Editorial Board of the Journal of Endodontics has developed a literature-based study guide of topical areas related to endodontics. This study guide is intended to give the reader a focused review of the essential endodontic literature and does not cite all possible articles related to each topic. Although citing all articles would be comprehensive, it would defeat the idea of a study guide. This section presents uses of calcium hydroxide related to apexogenesis, apexification, direct pulp cap, indirect pulp cap, internal resorption that has perforated laterally, intracanal medication, con
