Identifying key factors associated with subscapularis tendon tears and developing a risk prediction model to assist diagnosis.

BACKGROUND: There are still some challenges in diagnosing subscapularis (SSC) tendon tears as accurately as posterosuperior rotator cuff tears on MRI. The omission of SSC tendon tear can lead to muscle atrophy, fatty infiltration, and increased tearing accompanied by aggravated shoulder pain and loss of function. An effective non-invasive evaluation tool will be beneficial to early identification and intervention. The study aims to identify sensitive predictors associated with SSC tendon tears and develop a risk prediction model to assist in diagnosis. METHODS: Data on 660 patients who receiv

Editorial Commentary: You May Not Have Seen It, but It Has Seen You: Diagnosis of Long Head Biceps Tendon and Subscapularis Pathology in Association With Shoulder Rotator Cuff Pathology Can Be Challenging.

The shoulder can humble you. It is often challenging in determining the extent of pathology even when there is an adequate magnetic resonance imaging performed preoperatively and even during the so-called diagnostic portion of the arthroscopy. Abnormalities of the long head of the biceps tendon and the rotator cuff especially the subscapularis can be difficult to diagnose definitively. Experience can be very helpful in predicting pathology based on patterns seen before and knowing certain pearls that can make diagnosis and ultimately treatment more accurate.

Isolated Extra-articular Medial Dislocation of the Long Head of the Biceps Tendon in an Adolescent: A Case Report.

Isolated instability of the long head of the biceps tendon is uncommon and is usually associated with rotator cuff or bicipital pulley injuries. Isolated extra-articular medial dislocation in the absence of associated lesions is particularly rare in the adolescent population. We report the case of a 15-year-old male who presented with anterior shoulder pain after a recreational sports activity. Physical examination revealed localized anterior and anterolateral tenderness, with preserved active and passive range of motion and no clinical signs of instability. Magnetic resonance imaging using MR

Review of human supraspinatus tendon mechanics. Part I: fatigue damage accumulation and failure.

Repetitive stress injuries to the rotator cuff, and particularly the supraspinatus tendon (SST), are highly prevalent and debilitating. These injuries typically occur through the application of cyclic load below the threshold necessary to cause acute tears, leading to accumulation of incremental damage that exceeds the body's ability to heal, resulting in decreased mechanical strength and increased risk of frank rupture at lower loads. Consistent progression of fatigue damage across multiple model systems suggests a generalized tendon response to overuse. This finding may allow for interventio

Arthroscopic Repair for Chronic Massive Rotator Cuff Tears: A Systematic Review.

PURPOSE: To systematically review the available evidence for arthroscopic repair of chronic massive rotator cuff tears and identify patient demographics, pre- and post-operative functional limitations, reparability and repair techniques, and retear rates. METHODS: Medline, Embase, the Cochrane Database of Systematic Reviews, and the Cochrane Central Register of Controlled Trials were searched to identify all clinical papers describing arthroscopic repair of chronic massive rotator cuff tears. Papers were excluded if a definition of "massive" was not provided, if the definition of "massive" wa

[Subscapularis tendon lesions. Anatomy, diagnosis and importance of arthroscopic treatment].

The subscapularis tendon is involved in up to 43% of arthroscopically treated rotator cuff lesions. Due to the close anatomic relationship, participation of the long head of the biceps and supraspinatus tendon is common. Subscapularis tendon lesions are often not primary diagnosed correctly. Using specific clinical tests and modern sectional imaging, the percentage of correct diagnoses can be increased. Convincing clinical results, advantages of minimally invasive surgery, and superior visualization compared to the open approach argue for arthroscopic treatment of subscapularis lesions. Awaren

Subscapularis tendon tears.

Pathology of the subscapularis tendon is both infrequently identified and not commonly considered as a major source of shoulder pain and dysfunction. Subscapularis tendon pathology can present as isolated tears; partial-thickness tears; anterosuperior tears, also involving the supraspinatus tendon; complete rotator cuff avulsion; and rotator interval lesions, in which instability of the long head of the biceps tendon may dominate the clinical presentation. Although an accurate physical examination is paramount, modalities such as arthroscopy, magnetic resonance imaging, and ultrasound have adv

Overuse Injuries of the Shoulder.

Shoulder overuse injuries represent a broad and often overlapping spectrum shaped by repetitive loading, soft tissue adaptation, and changes in bone. Rather than distinct entities, they are better viewed along a continuum, where repeated microtrauma gradually leads to structural and functional alterations in tendons, capsulolabral structures, bursae, and bone. This narrative review covers the broad anatomical spectrum of shoulder overuse injuries, including rotator cuff disorders, impingement syndromes, labral and capsular abnormalities, biceps tendon pathology, bursal disease, and osseous and

[Conservative treatment of chronic tendinopathies].

The majority of insertional and noninsertional tendinopathy cases are associated with repetitive or overuse injuries. Certain tendons are particularly vulnerable to degenerative pathology; these include the Achilles and patella tendon, the rotator cuff, and forearm extensors/flexors. Disorders of these tendons are often chronic and can be difficult to manage successfully in the long term. Eccentric exercise has the strongest evidence of therapeutic efficacy. Extracorporeal shock wave treatment, sclerosing agents as well as nitric oxide patches show promising early results but require long-term

Acute shoulder injuries.

The shoulder is the most mobile joint in the human body. The cost of such versatility is an increased risk of injury. It is important that family physicians understand the anatomy of the shoulder, mechanisms of injury, typical physical and radiologic findings, approach to management of injuries, and indications for referral. Clavicle fractures are among the most common acute shoulder injuries, and more than 80 percent of them can be managed conservatively. Humeral head fractures are less common and usually occur in elderly persons; 85 percent of them can be managed nonoperatively. Common acute

Simultaneous repair of chronic full-thickness rotator cuff tears during fixation of proximal humerus fractures and clinical results.

OBJECTIVES: We investigated the incidence of chronic rotator cuff tears encountered during fixation of proximal humerus fractures with locking plate-screw systems, and evaluated the functional results of simultaneous surgical repair of these injuries. METHODS: A total of 111 patients underwent surgical treatment for proximal humerus fractures. Of these, nine patients (8 females, 1 male; mean age 73 years; range 56 to 84 years) who had concomitant chronic full-thickness rotator cuff tears were included in the study. According to the AO classification, the fractures were type 11A1 (12C1 also pr

Shoulder Injuries in the Throwing Athlete.

The throwing athlete with shoulder pain presents a diagnostic and treatment challenge to the orthopaedic surgeon. Because pitching a baseball requires the arm to accelerate at 7,000 degrees per second, tremendous forces are experienced at the shoulder joint. Electromyographic studies have shown that the larger scapular and trunk muscles are primarily responsible for arm acceleration. The smaller and more fragile rotator cuff muscles play a significant role in decelerating the arm. During the entire throwing mechanism, the rotator cuff and the capsulolabral complex act to stabilize the humeral

The scapula in rotator cuff disease.

The scapula serves as the platform or base for the muscles of the rotator cuff. Rotator cuff function and dysfunction is therefore directly related to the functional state of the scapula. When the scapula is well stabilized, optimal positioning in both static and dynamic tasks allows the rotator cuff to function at an optimal level. However, scapular dysfunction produces an unstable base which in turn negatively affects the rotator cuff's biomechanical contributions to arm motion. Scapular alterations may be causative in rotator cuff dysfunction or may be the result of rotator cuff injury and

Arthroscopic Matrix Augmentation of Partial-Thickness Rotator Cuff Tear Repairs Using a Full-Thickness Human Perinatal Tissue Allograft.

Partial-thickness rotator cuff tears (PTRCTs) are common and many progress to full-thickness tears. A persistently symptomatic PTRCT can result in pain, weakness, sleep disturbances, and functional loss. The high incidence, reported progression rates, limited intrinsic healing capability, and functional impairments motivate investigation into effective surgical strategies when conservative care fails. Human perinatal tissue allografts have been previously used for full-thickness rotator cuff tear repair in limited instances, however their fragility makes arthroscopic handling challenging. Here

Editorial Commentary: Tobacco or Not-All Nicotine Products Negatively Impact Rotator Cuff Surgery.

Rotator cuff repair is one of the most common elective orthopaedic surgeries performed; however, failure rates have been reported to range from 10% to 94% depending on the tear characteristics and other patient risk factors. One of the most common risk factors for failure is tobacco use. However, despite decreasing rates of tobacco use in recent years, the use of nontobacco-based nicotine products has become more commonplace, particularly in younger populations. The impact of these newer nicotine delivery products on orthopaedic surgery has been a topic of recent interest but minimally explore

Editorial Commentary: Estrogen Influences Shoulder Health.

Estrogen influences bone mineral density, muscle quality and quantity, and collagen synthesis, rotator cuff tearing, worse outcomes after rotator cuff repair, hyperlaxity, and adhesive capsulitis. In this commentary, we discuss the potential explanations for the highly significant associations between hormonal systemic birth control and a variety of shoulder pathologies. Ultimately, recent literature suggests that future research should be performed to better understand the connection between sex hormones and musculoskeletal function.
