Infectious sacroiliitis: Retrospective analysis of 18 case patients.

INTRODUCTION: Few infectious sacroiliitis reports are available in the literature. There is no standard clinical presentation, and diagnosis and treatments are therefore usually delayed. We aimed to describe this infection. METHODS: We performed a single-center retrospective study of patients hospitalized in the infectious diseases unit of the Limoges University Hospital from January 1, 2006 to January 31, 2016. We included all patients presenting with infectious monoarthritis of native sacroiliac joint. Clinical, biological, bacteriological, radiological, and therapeutic characteristics were…

Differential diagnosis between osteitis condensans ilii and sacroiliitis.

Sacroiliitis of seronegative spondyloarthropathy may sometimes show on pelvis plain films findings indistinguishable from those of osteitis condensans ilii. Computed tomography (CT) can differentiate earlier than plain radiography between the 2 conditions; furthermore, it should also be possible to make this differentiation clinically. The aim of our study was to verify whether the criteria recently proposed by the European Spondylarthropathy Study Group (EESG) for the classification of spondyloarthropathy are useful. CT scans through the synovial part of the sacroiliac joints of 7 consecutive…

Sacroiliitis during isotretinoin treatment: Causal association or coincidence?

Isotretinoin, a retinoid derivate used in acne treatment, has a variety of side effects involving the musculoskeletal system; however, sacroiliitis is rarely observed. Our aim was to present nine cases of sacroiliitis in patients being treated with isotretinoin. Sacroiliitis was identified and monitored using magnetic resonance imaging (MRI). Clinical symptoms were resolved with nonsteroidal anti-inflammatory drug therapy after isotretinoin treatment was ceased. Different from the other cases, follow-up MRI was done. Follow-up MRI revealed improvement in some patients. Although the association…

Bilateral sacroiliitis and uveitis comorbidity: brucellosis? Ankylosing spondylitis?

We present a rare case of a comorbidity of sacroiliitis and brucellosis infection. A 28-year-old woman received irregular medication due to ongoing backache and hip pain for 5 years. The patient presented to our hospital for evaluation of visual loss and was diagnosed with uveitis. Sacroiliac MRI was performed to investigate the inflammatory backache and hip pain, and the aetiology of the uveitis, revealing the presence of sacroiliitis. The patient's blood test results were as follows: positive brucellosis Rose Bengal test and positive tube agglutination test with a titre of 1/640. The patient…

Postpartum septic sacroiliitis coincident with labour epidural analgesia.

A 22-year-old woman presented to hospital 10 days after emergency caesarean section with severe back pain, fever tachycardia and a raised C-reactive protein. She had received labour epidural analgesia and was investigated for an epidural abscess. After repeat magnetic resonance imaging she was ultimately diagnosed with septic sacroiliitis. Although an uncommon cause of back pain, pregnancy-associated sacroiliitis should be considered in the differential diagnosis of post-epidural back pain, as the presentation and symptoms of an epidural infection and sacroiliitis are similar. We recommend ima…

Pregnancy-associated pyogenic sacroiliitis: case report and review.

BACKGROUND: Pyogenic sacroiliitis occurs infrequently during the peripartum period. CASE: A case at our institution and a review of the literature were analyzed. A total of 15 cases were discovered. The onset of illness was during pregnancy (40% of cases), within 3 weeks postpartum (40%) or post-abortion (20%), and the presentation was usually acute (< 7 days in 67% of cases). Frequent manifestations included localized pain in the hips or buttock, sacroiliac joint tenderness and fever. Computed tomography or magnetic resonance imaging revealed joint involvement in all cases tested. Microbiolo…

Bacterial sacroiliitis probably induced by lumbar epidural analgesia.

BACKGROUND: Properly administered, lumbar epidural analgesia provides adequate pain relief during labor and delivery, and is considered to be a safe procedure with limited complications. The prevalence of infection after lumbar epidural analgesia is negligible. INTRODUCTION: Infection of the sacroiliac joint, although very close to the pucture area, has never been reported as a procedure complication. CASE: In this report, we describe a patient who experienced bacterial sacroiliitis a few days after lumbar epidural analgesia for labor. No portal of entry was identified, and we evoked a new p…

Group B Streptococcal sacroiliitis: case report and review.

Streptococcus agalactiae, or group B Streptococcus (GBS), is the major cause of neonatal meningitis and sepsis but is an uncommon infection in adults. GBS arthritis is rare, and axial involvement with sacroiliitis is even more uncommon. Microbiological diagnosis frequently relies upon positive blood cultures as synovial fluid cultures are usually negative. Severe joint damage may result due to delay in the initiation of antibiotic treatment.

[Acute bacterial sacroiliitis caused by Salmonella cholerae-suis].

A 17-year-old schoolboy was admitted to hospital because of one-sided pelvic pain of uncertain aetiology and fever gradually rising over several days. Bacteriological analysis of blood cultures, skeletal scintigraphy and computed tomography revealed sacroiliitis caused by Salmonella cholerae-suis. Specific antibiotic treatment quickly stopped all symptoms and cured the infection. Radiologically there remained sclerosis of the sacro-iliac joint.

Pseudomonas aeruginosa sepsis with sacroiliitis in X-linked agammaglobulinaemia.

A boy under 5 years, with recurrent infections since 9 months of age, presented with fever and painful limping. Clinical examination revealed absent tonsils, antalgic gait and mild tenderness in the right hip joint. MRI of the pelvis demonstrated bone marrow oedema in the right sacroiliac joint, indicative of sacroiliitis. Blood cultures identified Pseudomonas aeruginosa as the causative organism. Immunological evaluation showed panhypogammaglobulinaemia and the absence of CD19-positive B cells in the lymphocyte subset analysis. Next-generation sequencing confirmed a hemizygous mutation (c.125…

[Septic sacroiliitis: early diagnosis due to specific physical examination].

BACKGROUND: Septic sacroiliitis is an uncommon disease which represents approximately 1-4% of all joint infections, therefore it is difficult to make the right diagnosis and to start early treatment. CASE REPORT: A 18 year old woman was admitted to the emergency room with a fever and pain in the left gluteal region. The patient was considered healthy and had no risk factors for septic arthritis. Edema and a small abscess was found in and around the left sacroiliac joint on pelvic MRI. The patient had positive blood cultures with Staphylococcus Aureus. Antibiotic treatment was initiated and la…

Brucellosis and Sacroiliitis Present as Acute Severe Hip Pain.

Brucellosis is a zoonotic disease that is endemic in Saudi Arabia. It is transmitted to humans through direct contact with infected animals or by consuming unpasteurized dairy products. A 36-year-old Saudi man presented with left hip pain, fever, and a history of consuming unpasteurized milk. Sacroiliitis was documented by positive results from serological tests (ELISA) and magnetic resonance imaging. Treatment consisted of non-steroidal anti-inflammatory drugs (NSAIDs), doxycycline, rifampicin, and IV gentamicin.
