Balance, gait, and falls in spinal cord injury.

This chapter covers balance, gait, and falls in individuals with spinal cord injury (SCI) from a clinical perspective. First, the consequences of an SCI on functioning are explained, including etiology, clinical presentation, classification, and epidemiologic data. Then, the specific aspects of balance disorders, gait disorders, and falls are discussed with respect to motor complete (cSCI) and incomplete (iSCI) SCI. Typically, these activities are affected by impaired afferent and efferent nerves, but not by central nervous processing. Performance of daily life activities in cSCI depends on th

Emerging concepts in the physiological basis of dystonia.

Work over the past 2 decades has led to substantial changes in our understanding of dystonia pathophysiology. Three general abnormalities appear to underlie the pathophysiological substrate. The first is a loss of inhibition. This makes sense considering that it may be responsible for the excess of movement and for the overflow phenomena seen in dystonia. A second abnormality is sensory dysfunction which is related to the mild sensory complaints in patients with focal dystonias and may be responsible for some of the motor dysfunction. Third, evidence from animal models of dystonia as well as f

Olfactory evoked potentials and contingent negative variation simultaneously recorded for diagnosis of smell disorders.

Objective diagnosis of olfaction can be performed by registration of cortical olfactory evoked potentials (OEP) and of contingent negative variation (CNV). The CNV is a negative voltage developing at the vertex after discrimination of one of two smells while the patient is expecting a second stimulus. By an adequate procedure, including a long time window for averaging (2.56 seconds) with appropriate filters, the two tests can be performed simultaneously in a single session of less than 10 minutes. Anosmia is determinable by both OEP and CNV, although CNV shows less variability. On the other h

Sex- and Gender-Related Differences in Common Functional Gastroenterologic Disorders.

Functional gastrointestinal (GI) disorders (FGIDs) result from central and peripheral mechanisms, cause chronic remitting-relapsing symptoms, and are associated with comorbid conditions and impaired quality of life. This article reviews sex- and gender-based differences in the prevalence, pathophysiologic factors, clinical characteristics, and management of functional dyspepsia (FD) and irritable bowel syndrome (IBS) that together affect approximately 1 in 4 people in the United States. These conditions are more common in women. Among patients with IBS, women are more likely to have severe sym

Symptom Presentation and Symptom Meaning Among Traumatized Cambodian Refugees: Relevance to a Somatically Focused Cognitive-Behavior Therapy.

Among psychologically distressed Cambodian refugees, somatic complaints are particularly prominent. Cambodians interpret anxiety-related somatic sensations in terms of "Wind" (khyâl), an ethnophysiology that gives rise to multiple catastrophic interpretations; and they have prominent trauma-memory associations to anxiety-related somatic symptoms. In this article, we detail some of the common sensation-related dysphoric networks of Cambodian refugees, focusing on catastrophic cognitions and trauma associations. We argue that delineating symptom-related dysphoric networks is crucial to successfu

Sensitive scalp: An epidemiologic study in patients with hair loss.

Sensitive scalp is a common condition defined by the presence of erythema and/or subjective symptoms as pain, pricking, burning, pruritus of the scalp elicited by triggering factors. Trichodynia is a term that describes a sensation of pain or burning of the scalp and was assumed to be part of sensitive scalp. Main goal of the study was to establish the prevalence of sensitive scalp in patients with trichological disorders. We conducted a retrospective observational study recorded: age, sex, trichological disorder (telogen effluvium, androgenetic alopecia, alopecia areata, scarring alopecia, tr

Sensory profile in infants and toddlers with behavioral insomnia and/or feeding disorders.

BACKGROUND: Sleep and feeding difficulties are two common disorders in early childhood. It has been shown that feeding difficulties are more common among children with sleep disorders and vice versa. Since a child's characteristics play a substantial role in these two conditions, we aimed to investigate the sensory profile of infants and toddlers with behavioral insomnia (BI) or feeding disorders (FDs) in comparison with healthy age-matched controls. METHODS: Children aged 7-36 months with BI or FD were recruited from the sleep and feeding disorders clinics. Healthy controls were recruited fr

Insulinoma Presenting with Neuropsychiatric Symptoms.

An insulinoma is a rare pancreatic endocrine tumor which is typically a hypervascular, solitary small tumour. 90 % of tumors are benign and less than 2 cm in size. Some insulinomas are associated with MEN-1 syndrome. Some cases of insulinoma may present with neuropsychiatric symptoms and may be wrongly diagnosed as psychosis. We report a case of insulinoma in a 55 years old female who presented with episodes of abnormal behavior and altered sensorium. On detailed investigations she was diagnosed as a case of hyperinsulinemic hypoglycemia due to insulinoma (in her case MRI abdomen was normal) D

[Clinical symptoms of equilibrium disorders in patients with whiplash syndrome].

INTRODUCTION AND OBJECTIVE: There are many entities that cause equilibrium disorders. Whiplash syndrome is becoming an important entity as a trigger of equilibrium disorders because of an increase in traffic accidents. There are many hypotheses on the generation of vertigo and dizziness in whiplash syndrome. The objective of this study is to describe and analyze the clinical symptoms of patients who suffered whiplash. MATERIAL AND METHOD: Thirty six patients with equilibrium disorders who suffered whiplash syndrome were studied prospectively. None of these subjects had cranial trauma or a his

Bulimia: an empirical analysis of psychodynamic theory.

This study examined two aspects of psychodynamic theory concerning bulimia nervosa, that bulimic patients ascribe to a traditionally feminine sex-role and that bulimic women have difficulty differentiating emotional from visceral sensations. 18 bulimic and 18 control women were administered the Bem Sex-role Inventory and the Eating Disorder Inventory. Contrary to dynamic theory, bulimic women were not more likely classified as traditionally feminine than control subjects; however, significantly more controls than bulimic women were classified as androgynous. Analysis of scores on the Eating Di

Congenital insensitivity to pain with anhidrosis.

Congenital insensitivity to pain with anhidrosis is a rare disorder. Its primary clinical features include congenital analgesia, which leads to self-mutilation; inability to sweat, which leads to defective thermoregulation; and mental retardation. A five-year-old boy with consanguineous parents and no family history of the disorder presented with ulcerating lesions on both knees and elbows. His family had discovered the lack of sensation to pain and anhidrosis. Physical examination revealed ulcers on both knees and elbow, self-mutilation of the tongue, fingers, and toes. Sensation to touch was

From Sensation to Anxiety: The Mediating Effect of Physical Sensation and Experiential Avoidance on Exercise Anxiety Among College Students.

Negative physical sensation (i.e., negative reactions to the physical sensations with exercise) refers to the perception and attitude toward one's own physical health and exercise capacity under a specific stressful situation. It is affected by potentially stressful situations, negative emotions, and avoidance coping strategies, and it has an important influence on individual behavior. In the stress situation, the avoidance strategy of cognition, emotion and behavior caused by emotional disorder is called experiential avoidance. Negative physical sensation and experiential avoidance, as negati

Duane syndrome with prominent oculo-auricular phenomenon.

Duane syndrome is a congenital cranial dysinnervation disorder involving absent or anomalous innervation of the lateral and medial rectus muscles that is sometimes associated with other manifestations of dysinnervation. We describe a patient with right esotropic Duane syndrome with a long-standing retroauricular tugging sensation in right gaze who was noted to have prominent ipsilateral oculo-auricular phenomenon, representing either abnormal enhancement of existing innervation or an uncommon dysinnervation. After successful strabismus surgery the tugging sensation improved but the phenomenon

Dimensions of interoception in obsessive-compulsive disorder.

Interoceptive sensibility (IS) refers to the subjective experience of perceiving and being aware of one's internal body sensations, and is typically evaluated using self-report questionnaires or confidence ratings. Here we evaluated IS in 81 patients with OCD and 76 controls using the Multidimensional Scale of Interoceptive Awareness (MAIA), which contains 8 subscales assessing adaptive and maladaptive responses to sensation. Compared to controls, OCD patients showed hyperawareness of body sensations. Patients also demonstrated a more maladaptive profile of IS characterized by greater distract

Effects of aerobic exercise on anxiety sensitivity.

Anxiety sensitivity is a known precursor to panic attacks and panic disorder, and involves the misinterpretation of anxiety-related sensations. Aerobic exercise has been shown to reduce generalized anxiety, and may also reduce anxiety sensitivity through exposure to feared physiological sensations. Accordingly, 54 participants with elevated anxiety sensitivity scores completed six 20-min treadmill exercise sessions at either a high-intensity aerobic ( n = 29 ) or low-intensity ( n = 25 ) level. Self-ratings of anxiety sensitivity, fear of physiological sensations associated with anxiety, and g

Faecal incontinence associated with reduced pelvic sensation.

Eight patients with faecal incontinence associated with high threshold of rectal sensation are described. All had a normal anal sphincter on clinical and physiological assessment including anal manometry, pudendal nerve latency estimation and single fibre electromyography. In each case however rectal sensation was abnormal. The threshold of rectal sensation was 143 +/- 33 ml (range 110-200 ml) compared with control values of 39 +/- 14 ml (range 25-65 ml) (P less than 0.005) in 12 normal age-matched individuals. The degree of rectal distension required to initiate the recto-anal reflex was no d

Comorbid depressive disorders and left-side dominant occlusal discomfort in patients with phantom bite syndrome.

BACKGROUND: Phantom bite syndrome (PBS) is characterised by occlusal discomfort without corresponding dental abnormalities. Despite repeated, failed dental treatments, patients with PBS persist in seeking bite correction. PBS has been regarded as a mental disorder. However, we have reported that PBS patients with a dental trigger tend to have less psychiatric history than those without. Hence, the symptoms of PBS cannot be explained by a mental disorder alone, and it is unclear if mental disorders affect occlusal sensation. OBJECTIVE: To elucidate the pathophysiology of PBS, we analysed the d

Effects of antidepressants in patients with functional esophageal disorders or gastroesophageal reflux disease: a systematic review.

BACKGROUND & AIMS: Patients with functional esophageal disorders present with symptoms of chest pain, heartburn, dysphagia, or globus in the absence of any structural abnormality. Visceral hypersensitivity is a feature of these functional disorders, and might be modulated by antidepressant therapy. We evaluated evidence for the efficacy of antidepressant therapy for symptoms associated with esophageal visceral hypersensitivity in patients with functional esophageal disorders or gastroesophageal reflux disease (GERD). METHODS: We performed a systematic search of the Cochrane Comprehensive Tria
