BACKGROUND Bilateral posterior shoulder fracture-dislocation can be caused by forced internal rotation during a seizure and requires management with combined open reduction procedures to address instability and fractures to restore function. This report describes the case of a 27-year-old man with a delayed diagnosis of bilateral posterior shoulder fracture-dislocation following a seizure managed by bilateral open reduction, fixation of the greater tuberosity, biceps tenodesis, and subscapularis anchor repair using a deltopectoral approach. CASE REPORT A 27-year-old man with no prior history o

INTRODUCTION: Proximal humerus fractures constitute approximately 4-6% of all fractures, commonly affecting the elderly due to low-energy trauma and osteoporotic bones. Two-part surgical neck fractures are the most frequent subtype. In younger individuals, high-energy trauma is a usual cause. However, a proximal humerus fracture in a patient with a history of trapezius muscle transfer for brachial plexus injury is an extremely rare clinical scenario, with limited literature available on its management and rehabilitation.

Periprosthetic humerus fractures that occur around a shoulder arthroplasty can be treated nonoperatively, with revision arthroplasty, or with surgical fixation with osteosynthesis. The decision of which technique to employ depends on the unique fracture characteristics and the stability of the prothesis. When the prothesis is stable and able to be retained, limited bone stock about the prothesis for fixation makes these fractures challenging to treat. Fixation of these fractures with a utility plate that provides for the placement of fixed angle screws in a variety of orientations creates a ro

Pediatric midshaft humerus fractures are typically managed with a hanging arm cast, Sarmiento bracing, coaptation splint, or a combination of these treatment options. Here we report a novel use of a shoulder spica cast in the treatment of a midshaft humerus fracture in the presence of limb deficiency. Current treatments proved unsuccessful in maintaining adequate alignment, specifically the varus deformity of the fracture. A shoulder spica was able to successfully maintain acceptable alignment throughout the duration of the patient's healing process. This nontraditional use of a shoulder spica

RATIONALE: Bilateral posterior fracture-dislocation of the shoulders occurs rarely and the diagnosis is often challenging. This injury is often missed or delayed on initial presentation, leading to continuous pain, disability, and rising medical costs. Timely diagnosis and proper treatment are very important to restore shoulder function.
PATIENT CONCERNS: Here we report 2 rare cases. Case 1 was a 53-year-old physical worker with severe pain and limited shoulder movement after an unexpected fall. Case 2 was a 55-year-old man with pain in upper limbs and shoulders after an electric shock.

Axillary artery injury along with proximal humerus fracture as a result of blunt trauma is a rare incidence. We present a 65-year-old male with a pulseless cold hand, who had blunt trauma 3 days ago, leading to fracture of proximal end of humerus along with axillary artery injury. Surgical intervention revealed contusion of axillary artery with intravascular thrombosis causing complete occlusion. Internal fixation of fracture along with excision of contused axillary artery was done followed by interposition venous graft. The diagnostic modalities and treatment options for such a case are discu

In reverse total shoulder arthroplasty, retensioning of the deltoid muscle is essential for regaining active elevation. However, the procedure remains problematic, in that it may potentially produce complications not only because it is a complex procedure but also because it causes anatomical changes. We experienced a rare case of a 64-year-old woman presenting with non-traumatic clavicle fracture after reverse total shoulder arthroplasty via a deltopectoral approach. In our case, the patient presented with pain in the clavicle area, which worsened during joint movement. Therefore, surgeons sh

OBJECTIVE: To discuss the effectiveness of Poking reduction with shoulder arthroscopy-assisted surgery for displaced scapular neck fracture.
METHODS: Between January 2009 and January 2012, 9 cases of displaced scapular neck fracture underwent shoulder arthroscopy-assisted surgery for Poking reduction treatment. Of 9 cases, 6 were men, and 3 were women, aged 21-54 years (mean, 39 years). The causes were traffic accident injury in 7 cases, falling injury from height in 1 case, and hurt injury in 1 case. The shoulder abduction, flexion, and external rotation were obviously limited. X-ray films s

BACKGROUND: Complex posterior fracture-dislocations of the shoulder are rare and often associated with poor long-term function regardless of the choice of treatment. The purposes of this study were to evaluate the epidemiology and pathological anatomy of posterior fracture-dislocations of the shoulder and to assess the clinical and radiographic outcomes of a specific treatment protocol of open reduction and internal fixation.
METHODS: We studied the demographic details of a consecutive series of twenty-six patients (twenty-eight shoulders in nineteen men and seven women with a mean age of fif

We present two patients with complaints of shoulder pain after an epileptic seizure. Both patients had a posterior dislocation fracture of the shoulder. After reviewing the literature the following conclusions can be drawn: (1) A posterior shoulder dislocation fracture is rare. (2) One should not underestimate the muscular forces in seizure disorders and be alert for dislocation fractures of the shoulder and/or other joints. (3) The diagnosis is frequently missed, but an axillary radiograph or a CT scan always reveals the fracture.

The case of an 39-year-old man is presented, who sustained a bilateral locked fracture dislocation of the shoulders occurring during an epileptic seizure. Radiographs demonstrated a compression-fracture of the anteromedial aspect of the humeral head bilaterally (reversed Hill-Sachs-lesions). Additionally, fractures of the lesser and greater tuberosity were diagnosed at the right site. Open reduction and internal fixation was performed in both shoulders within 12 hours. 6 months later the patient has no complaints with a free range of motion. Diagnostics, treatment and result are discussed in c

Results after conservative or surgical treatment of comminuted fractures of the head of the humerus are often unsatisfactory and complicated by pain and a severely decreased range of motion. In these patients good results have been obtained by replacement of the humeral head with the Isoelastic Humeral Head Prosthesis. Fourteen cases are reported and their results discussed.

Proximal humerus fractures are very common injuries. In young people the treatment options include non-operative management, internal fixation, and in selected cases, shoulder arthroplasty. One of the most challenging complications after internal fixation is the fracture non-union that can lead to the dreaded implant breakage. In such a situation, the treatment depends on the patient's age, bone quality, bone loss, infection status, and functional demands, and consists of revision open reduction and internal fixation employing autologous bone grafting or conversion to shoulder arthroplasty. INTRODUCTION: This study evaluates the role of anatomic scapular morphology in acromion and scapular spine fracture (SSAF) risk after reverse shoulder arthroplasty (RSA).
METHODS: Twelve scapular measurements were captured based on pilot study data, including scapular width measurements at the acromion (Z1), middle of the scapular spine (Z2), and medial to the first major angulation (Z3). Measurements were applied to 3D-CT scans from patients who sustained SSAF after RSA (SSAF group) and compared with those who did not (control group). Measurements were done by four investigators, and the int

UNLABELLED: Complex proximal humerus fractures are difficult to treat particularly in patients above 65 years with osteoporotic bone and disturbed anatomical landmarks. Fracture-inverse total shoulder arthroplasty has shown a good treatment option for these injuries. However, better results are associated with meticulous reduction and fixation of the tuberosities around the implant. The purpose of this study is to describe a simple technique to pass cerclage ribbons to fix the tuberosities together around the inverse total shoulder arthroplasty.
METHODS: This is a retrospective cohort study t

Acromial stress fracture is a well-known complication after reverse shoulder arthroplasty. Although some patients are asymptomatic, others may present with reproducible pain to palpation and signs of impingement. Generally, the initial mode of treatment is conservative; however, surgical intervention is considered in patients with painful nonunion and displaced fragments. Although open reduction techniques are most common, Levy type I fragments may be amenable to excision. This technical note provides a stepwise approach to arthroscopic excision for painful nonunited acromial stress fractures
