Shoulder-hand syndrome after stroke. A complex regional pain syndrome.

Complex regional pain syndrome (CRPS) types I and II are neuropathic pain disorders that develop as an exaggerated response to a traumatic lesion or nerve damage, that generally affects the extremities, or as the consequence of a distant process such as a stroke, spinal lesion or myocardial infarction. It rarely appears without an apparent cause. CRPS of upper limbs after stroke is frequently today called shoulder-hand syndrome (SHS). The onset and severity of SHS appears to be related with the aetiology of the stroke, the severity and recovery of motor deficit, spasticity and sensory disturba

Referred shoulder pain from ventriculoperitoneal shunts. Report of three cases.

The authors report on three pediatric patients with ventriculoperitoneal (VP) shunts who presented with chronic right shoulder pain. Imaging revealed that the distal peritoneal catheter was positioned between the right hemidiaphragm and liver. Following surgical repositioning of the distal tubing, all patients experienced resolution of their shoulder pain, which has not recurred. Although seemingly rare, referred shoulder pain from a VP shunt should be added to the list of complications seen with this method of cerebrospinal fluid diversion. The clinician who cares for patients with VP shunts …

[A case of shoulder pain following right basilic vein catheterization].

We have experienced a case of total laryngectomy with graft of the rectus abdominis muscle skin flap because of the recurrent lingual cancer. Just before the operation, we used right basilic vein for introduction of a central venous catheter. Right shoulder pain occurred on the first postoperative day. After the operation, we found that the catheter from the right basilic vein was inserted to the right jugular vein. We pulled out it and inserted another catheter from the left subclavian vein. Shoulder pain diminished and then pyrexia and symptom of infection diminished. After recatheterization…

Acute brachial plexus neuritis: an uncommon cause of shoulder pain.

Patients with acute brachial plexus neuritis are often misdiagnosed as having cervical radiculopathy. Acute brachial plexus neuritis is an uncommon disorder characterized by severe shoulder and upper arm pain followed by marked upper arm weakness. The temporal profile of pain preceding weakness is important in establishing a prompt diagnosis and differentiating acute brachial plexus neuritis from cervical radiculopathy. Magnetic resonance imaging of the shoulder and upper arm musculature may reveal denervation within days, allowing prompt diagnosis. Electromyography, conducted three to four we

Shoulder-arm pain from cervical bands and scalene muscle anomalies.

Fourteen patients were identified with (1) pain and sensory changes in a brachial plexus distribution, (2) aggravation of pain with use of the affected extremity, and (3) pain on palpation over the brachial plexus. All patients had minimal or no intrinsic hand muscle atrophy. Only one patient had cervical ribs. Nerve conduction studies were normal, and electromyography (EMG) showed mild chronic neuropathic changes in 2 patients. None of the patients responded to conservative therapy over a prolonged period (7-12 months). A compressive brachial plexopathy from abnormally attached or enlarged sc

Shoulder pain and functional disability in spinal cord injury patients.

Sixty spinal cord injured patients were examined to determine the incidence of nontraumatic shoulder pain and associated functional disability during the first 18 months after spinal cord injury (SCI). Seventy-eight percent of quadriplegics and 35% of paraplegics had pain in the first six months. When reexamined six to 18 months after SCI, 33% of the quadriplegics and 35% of the paraplegics continued to have pain. The functional disability resulting from shoulder pain was not a significant problem for the paraplegics; however, 84% of the quadriplegics having pain had either moderate or severe …

Associations of sickness absence for pain in the low back, neck and shoulders with wider propensity to pain.

OBJECTIVES: To explore the association of sickness absence ascribed to pain at specific anatomical sites with wider propensity to musculoskeletal pain. METHODS: As part of the CUPID (Cultural and Psychosocial Influences on Disability) study, potential risk factors for sickness absence from musculoskeletal pain were determined for 11 922 participants from 45 occupational groups in 18 countries. After approximately 14 months, 9119 (78%) provided follow-up information about sickness in the past month because of musculoskeletal pain, including 8610 who were still in the same job. Neck or Shoulder? Establishing Consensus for Spine Screening in Patients With Shoulder Pain: An International Modified Delphi Study.

OBJECTIVE: There is no established consensus for screening the spine in patients with shoulder pain. The aim of this study was to explore the role of the spine in shoulder pain and generate a set of recommendations for assessing the potential involvement of the spine in patients with shoulder pain. METHODS: A modified Delphi study was conducted through use of an international shoulder physical therapist's expert panel. Three domains (clinical reasoning, history, and physical examination) were evaluated using a Likert scale, with consensus defined as Aiken Validity Index ≥0.7. RESULTS: Twenty…

Influence of enhancing dynamic scapular recognition on shoulder disability, and pain in diabetics with frozen shoulder: A case report.

BACKGROUND: Frozen shoulder (FS) is a familiar disorder. Diabetics with FS have more severe symptoms and a worse prognosis. Thus, this study investigated the influence of enhancing dynamic scapular recognition on shoulder disability and pain in diabetics with FS. CASE SUMMARY: A Forty-five years-old male person with diabetes mellitus and a unilateral FS (stage II) for at least 3 mo with shoulder pain and limitation in both passive and active ranges of motion (ROMs) of the glenohumeral joint of ≥ 25% in 2 directions participated in this study. This person received dynamic scapular recognition …

Revision total shoulder arthroplasty for painful glenoid arthrosis after humeral head replacement: the nontraumatic shoulder.

BACKGROUND: Patients treated with humeral head replacement (HHR) may require revision to total shoulder arthroplasty (TSA) due to glenoid arthrosis. This study characterizes the outcomes of revision TSA in patients who initially underwent HHR for nontraumatic glenohumeral arthritis. METHODS: From 1982 to 2005, 68 shoulders underwent revision TSA for glenoid arthrosis. The initial HHR was performed for non-fracture-related arthritis. Revisions were grouped according to complexity for analysis. Stem revision and soft tissue reconstruction were assessed in relation to outcome. RESULTS: Pain sco

Revision total shoulder arthroplasty for painful glenoid arthrosis after humeral head replacement: the posttraumatic shoulder.

HYPOTHESIS: This study was conducted to test the hypothesis that patients would have improved pain and range of motion after conversion total shoulder arthroscopy but that overall outcome would be substantially affected by the need for removal of the humeral component and associated alterations of bony anatomy or soft tissue deficiencies. MATERIALS AND METHODS: Thirty-four patients (34 shoulders) with HHR after a proximal humeral fracture underwent revision total shoulder arthroplasty for painful glenoid arthrosis, with mean follow-up of 9.4 years (range, 2.3-20.4 years). The three-dimensional shoulder pain alignment (3D-SPA) mobilization improves pain-free shoulder range, functional reach and sleep following stroke: a pilot randomized control trial.

Background and purpose: Following a stroke, three-dimensional clavicular/scapular/humeral joint rotations may become restricted and contribute to post-stroke shoulder pain. This study examined whether a treatment group provided with current standard treatment plus the proposed "Three-dimensional Shoulder Pain Alignment" mobilization protocol demonstrated improved pain-free shoulder range, functional reach and sleep compared to a control group provided with standard treatment alone. Methods: In this double-blinded parallel-group randomized control trial, treatment and control subjects with moder…

Peripheral Nerve Stimulation for Chest Wall and Persistent Shoulder Pain After Reverse Total Shoulder Arthroplasty: A Case Report.

BACKGROUND: Persistent pain after reverse total shoulder arthroplasty (RTSA) warrants evaluation for surgical and infectious causes. Peripheral nerve stimulation (PNS) may be considered when these causes have been assessed, but evidence for its use in chest wall and shoulder pain after RTSA is limited. CASE REPORT: A 70-year-old man had persistent right anterior shoulder pain and new chest wall pain after revision RTSA. Diagnostic testing did not establish a surgical or infectious cause. A right pectoral nerve (PECS I) block provided temporary relief. A temporary PNS lead was placed near the …
