For 50 years, serotonin has been in the centre of the search for the mechanisms and control of sleep. Serotonergic neurotransmission is related to the behavioural state of the animal and plays an important role in modulation of the behavioural state, by interacting with other brain areas modulating circadian rhythm, sleep and waking. Serotonergic activity may be accompanied by waking or sleep depending on the brain area and receptor type involved in the response, on the current behavioural state and on the concomitant agonism/antagonism of other neurotransmitter systems.

Adenosine is directly linked to the energy metabolism of cells. In the central nervous system an increase in neuronal activity enhances ener … d wakefulness, decrease the activity of presumably cholinergic cells and via this mechanism promote sleep. Our hypothesis is that in the cholinergic basal forebrain prolonged wakefulness induces local energy depletion that generates increases in extracellular adenosine concentrations in this area. In addition to the immediate effects, high extracellular …

Naturally fluctuating hormones (menstrual cycle, through pregnancy or menopausal transition) are not related to marked sleep disturbances in women. It is likely, however, that subsets of women will display a central nervous system vulnerability to hormonal fluctuations so that sleep disturbances manifest as a part of a complex of discomforting symptoms. Sleep is impacted directly through the circadian system or brain sleep regulation or through the development of concurrent functional changes and symptoms. Women are susceptible to sleep-related disorders that are also common in men, such as pr…

Epilepsy and sleep are intricately associated. Influences of epilepsy on sleep as a result of drug treatment and the vigilance altering effects thereof as well as the temporal distribution and presentation of seizures are considered. The effect of sleep quality on seizure frequency is well known and must play a part in the management of patients with epilepsy. A careful scrutiny of potential sleep disturbing factors such as primary sleep disorders is equally important to the successful management of seizure frequency and of excessive daytime somnolence, which may be a confounding comorbidity i…

This paper reviews some of the recent advances in the sleep apnoea/hypopnoea syndrome. Sleep apnoea has recently been shown to be familial and in some of these patients this is due to retroposition of the maxilla and mandible. There is now good evidence that individuals with sleep apnoea have an increased risk of road accidents and increasing, although not yet 100% robust, evidence that they have an increased frequency of systemic hypertension. The last few years have seen a large number of randomised controlled trials of CPAP therapy which have shown clear evidence that CPAP improves symptoms…

A multimedia Sleep tutorial for General Practitioners was implemented from scratch. The implementation had into account the following features: 1) Specific needs of GPs evaluated in 3 different countries, related with Tutorial contents and technical features; 2) Multinational authorship from European experts; 3) Multilingual possibilities; 4) User friendliness and easy navigation. The tutorial was implemented and tested and its gama version is now available for commercialization.

Both Lewis Carroll and William Shakespeare appear to have made clinical observations of sleep apnea syndromes long before they were discovered by medical science, and to have understood something about their physiological mechanisms. The somnolent dormouse in Alice in Wonderland indicates that his problem is one of sleep and breathing and is subject to modern treatment for obstructive apnea. Shakespeare in Henry IV presents a case of obstructive apnea along with a case of Cheyne-Stokes breathing and uses the plot of these history plays to explain by analogy the theoretical basis for periodic b…

OBJECTIVE: A description of sleep-wake disorders in Alzheimer's disease and Creutzfeldt-Jakob disease, and analysis of their diagnostic usefulness.
DEVELOPMENT: Patients with Alzheimer's disease have fragmented sleep, with an increase in the time awake and reduction in deep slow sleep and REM sleep. These changes increase as the disease progresses and cognition deteriorates. In Creutzfeldt-Jakob's disease polysomnographic studies show a cyclical pattern and theta or theta-delta activity. These cyclical changes are modified as the disease progresses and disappear before death.
CONCLUSIONS: Al…

Sleep regulation calls for 3 processes: the first one is the homeostatic process increasing during wakefulness and decreasing during sleep, the second one is the circadian process depending on the circadian oscillator which controls temperature and alertness rhythms, and the third one is the ultradian process: it determines the NREM/REM periodicity. In the 2 process model of sleep regulation, the power density of the delta band (0.75-4.5 Hz) called slow wave activity (SWA) and obtained by spectral analysis, is supposed to reflect the variations of a homeostatic recovery process (process S) tha…

Generally sleep is considered a time of amnesia. It is not uncommon for an individual to experience 8 h of sleep and have no memory for events during that time. Similarly, a substantial proportion of the population has no memory for dreams that occurred during the night, despite the fact that the literature on awakening during rapid eye movement (REM) sleep clearly shows that individuals normally have four to six "dream experiences" a night. Research on this issue seems to indicate that the lack of memory cannot be explained by the organisms' inability to perceive stimuli. The data indicate th…

Epileptic mechanisms in the brain are subject to long-duration, time-ordered neuromodulatory processes controlled by endogenous oscillators which are responsible for appropriately phased modulation of various normal physiological processes, including the 24-h sleep/wakefulness cycle and the ultradian 100-min cycle of rapid eye movement/non-rapid eye movement sleep. Both focal and generalized types of epileptiform activity in humans are subject to biorhythmic modulation, and the various modulation patterns observed are in accord with a model which explains these patterns as a consequence of the…

Sleep is an enigma and the clinical importance of disturbed sleep is not clear. Much has been learnt in the past 25 years of the physiology of normal and abnormal sleep. Complaints of insomnia can be related to several causes--normal extreme patterns, psychiatric, physical, specific and drug-related. The treatment of insomnia is essentially that of the primary condition. Hypnotics have a limited role and their use should always be carefully considered.
