Treatment of late stage sleeping sickness caused by T.b. gambiense: a new approach to the use of an old drug.

Melarsoprol is the standard treatment of late stage trypanosomiasis. The development of treatment schedules was previously purely empirical. Generally melarsoprol is given in 3 series of three to four consecutive injections, given every 24 hours, with an interval of about one week between the series. Based on pharmacokinetic analysis, computer simulations and extensive literature research covering all schedules previously used and tested, a new schedule, consisting of ten daily consecutive doses of 2.16 mg/kg of the drug was suggested. The pharmacokinetic model was validated in uninfected verv

Relapses following treatment of early-stage Trypanosoma brucei gambiense sleeping sickness with a combination of pentamidine and suramin.

616 patients with early Trypanosoma brucei gambiense trypanosomiasis (no trypanosomes in the cerebrospinal fluid [CSF] and a CSF white cell count [WCC] of 1-5/mm3) were treated with a combination of pentamidine (6 intramuscular [i.m.] injections of 4 mg/kg) and suramin (2 intravenous [i.v.] injections of 20 mg/kg) in Nioki hospital, Zaire, between 1983 and 1992; 46 (7.5%) of them subsequently relapsed. There was no increase in the frequency of treatment failure during this 10 years' period. Relapses were more frequent in children aged 0-17 years (19/163 [11.7%]) than in adults (26/420 [6.2%] (

Effectiveness of a lightweight portable auto-CPAP device for the treatment of sleep apnea during high altitude stages of the Dakar Rally: a case report.

Sleep-related breathing disturbances are exacerbated at altitude in patients with Obstructive Sleep Apnea (OSA). The objective of this case report was to determine if a portable auto-CPAP device effectively treated sleep apnea across different altitudes. We report the severity of sleep apnea from 60 to 12,000 feet high in a man with severe OSA (Apnea Hypopnea Index at diagnosis = 60 events/hour) during the 2017 Dakar rally over the Andes mountains. The man was equipped with a lightweight portable auto-CPAP device with a narrow window [6-8 cmH2O]. Pressures delivered and corresponding residual

Correlation analysis of anxiety, depression, and sleep quality in end-stage renal disease patients undergoing maintenance hemodialysis.

End-stage renal disease (ESRD) patients undergoing maintenance hemodialysis (MHD) often experience significant psychological distress and sleep disturbances, with anxiety and depression being particularly prevalent. These conditions may negatively affect sleep quality. However, comprehensive studies examining the interplay between anxiety, depression, and sleep quality in ESRD patients are limited. Understanding these relationships is crucial for improving patient care. This study aimed to evaluate the prevalence and severity of anxiety, depression, and sleep disorders in ESRD patients on MHD

BTCRSleep: a boundary temporal context refinement-based fully convolutional network for sleep staging with single-channel EEG.

Objective. Sleep staging studies on single-channel EEG mainly exploit deep learning methods that combine convolutional neural networks (CNNs) and recurrent neural networks. However, when typical brain waves (such as K-complexes or sleep spindles) that identify sleep stages span two epochs, the abstract process of a CNN extracting features from each sleep stage may cause the loss of boundary context information. This study attempts to capture the boundary context, which contains the characteristics of brain waves during sleep stage transition, to improve the performance of sleep staging.Approac…

Effect of nocturnal hemodialysis on sleep parameters in patients with end-stage renal disease: a systematic review and meta-analysis.

INTRODUCTION: Recently, a small but growing literature has depicted the beneficial effects of nocturnal hemodialysis (NHD) over conventional hemodialysis (CHD) in the fields of sleep disorders such as sleep apnea. The impact of various dialysis models on sleep disorders, however, has not been determined. The objective of our meta-analysis is to examine the potential effects of NHD, compared with CHD, on sleep disorders in HD patients. METHODS: Several electronic databases including PubMed, EMBASE, Cochrane Central Register of Controlled Trials, ClinicalTrials.gov and CNKI were searched, using

Modified Z-palatoplasty with one-layer closure in one-stage multilevel surgery for severe obstructive sleep apnea.

OBJECTIVE: The meticulous two-layer closure is a step to complete the modified Z-palatoplasty, which has been reported to serve as an effective element in multilevel sleep surgery for patients with severe obstructive sleep apnea, especially with Friedman anatomical stages II and III diseases. A single layer closure-the suture closure as originally described in uvulopalatopharyngoplasty by Fujita et al., is an alternative of the two-layer closure, featured by simplicity while its efficacy has not been completely proved in patients with severe obstructive sleep apnea. METHODS: By apnea-hypopnea

Enantiospecific reassessment of the pharmacokinetics and pharmacodynamics of oral eflornithine against late-stage Trypanosoma brucei gambiense sleeping sickness.

This study aimed to characterize the stereoselective pharmacokinetics of oral eflornithine in 25 patients with late-stage Trypanosoma brucei gambiense sleeping sickness. A secondary aim was to determine the concentrations of L- and D-eflornithine required in plasma or cerebrospinal fluid (CSF) for an efficient eradication of the T. brucei gambiense parasites. Patients were randomly allocated to receive either 100 (group I, n=12) or 125 (group II, n=13) mg/kg of body weight of drug every 6 h for 14 days. The concentrations of L- and D-eflornithine in the plasma and CSF samples were measured usi

Automatic sleep staging: from young adults to elderly patients using multi-class support vector machine.

Aging is a process that is inevitable, and makes our body vulnerable to age-related diseases. Age is the most consistent factor affecting the sleep structure. Therefore, new automatic sleep staging methods, to be used in both of young and elderly patients, are needed. This study proposes an automatic sleep stage detector, which can separate wakefulness, rapid-eye-movement (REM) sleep and non-REM (NREM) sleep using only EEG and EOG. Most sleep events, which define the sleep stages, are reduced with age. This is addressed by focusing on the amplitude of the clinical EEG bands, and not the affect…

DSleepNet: Disentanglement Learning for Personal Attribute-Agnostic Three-Stage Sleep Classification Using Wearable Sensing Data.

Long-term non-invasive sleep stage monitoring is instrumental in comprehending the progression of sleep disorders, cardiovascular diseases, and the interplay between sleep, type 2 diabetes, and neurodegenerative diseases. However, the conventional deep learning approach is susceptible to personal attributes (PAs) such as age, Body Mass Index, and severity of sleep apnea existing in the training dataset, potentially hindering its generalisation capacity to unseen cohorts. This paper introduces DSleepNet, a novel approach that disentangles the feature space into PA-specific and PA-agnostic compo

Factors Associated With Poor Sleep Quality in Patients With Early-Stage Chronic Kidney Disease: A Cross-Sectional Study.

The prevalence of poor sleep quality tends to increase progressively as renal function deteriorates. Patients with declining kidney function frequently encounter both physical and psychological discomfort. However, research investigating the factors influencing sleep quality in individuals with early-stage chronic kidney disease (CKD) has been limited. This study aims to identify specific factors associated with early-stage CKD that may contribute to poor sleep quality and to elucidate the relationship between these factors and sleep quality. A hundred and six patients completed the assessment

Reorganization of intrinsic functional connectivity in early-stage Parkinson's disease patients with probable REM sleep behavior disorder.

REM sleep behavior disorder (RBD) symptoms in Parkinson's disease (PD) suggest both a clinically and pathologically malignant subtype. However, whether RBD symptoms are associated with alterations in the organization of whole-brain intrinsic functional networks in PD, especially at early disease stages, remains unclear. Here we use resting-state functional MRI, coupled with graph-theoretical approaches and network-based statistics analyses, and validated with large-scale network analyses, to characterize functional brain networks and their relationship with clinical measures in early PD patien

Interleukin (IL)-6 and IL-10 Are Up Regulated in Late Stage Trypanosoma brucei rhodesiense Sleeping Sickness.

BACKGROUND: Sleeping sickness due to Trypanosoma brucei rhodesiense has a wide spectrum of clinical presentations coupled with differences in disease progression and severity across East and Southern Africa. The disease progresses from an early (hemo-lymphatic) stage to the late (meningoencephalitic) stage characterized by presence of parasites in the central nervous system. We hypothesized that disease progression and severity of the neurological response is modulated by cytokines. METHODS: A total of 55 sleeping sickness cases and 41 healthy controls were recruited passively at Lwala hospit

Sleep quality and fatigue after a stress management intervention for women with early-stage breast cancer in southern Florida.

BACKGROUND: Sleep disruption and fatigue are ubiquitous among cancer patients and are sources of stress that may compromise treatment outcomes. Previously, we showed that a cognitive behavioral stress management (CBSM) intervention reduced anxiety and other stress-related processes in women undergoing primary treatment for breast cancer. PURPOSE: This study examined secondary outcomes from a CBSM intervention trial for women with early-stage breast cancer to test if CBSM would improve sleep quality and fatigue among these patients at a single site in southern Florida. CBSM-related effects hav

New treatment option for second-stage African sleeping sickness: in vitro and in vivo efficacy of aza analogs of DB289.

African sleeping sickness is a fatal parasitic disease, and all drugs currently in use for treatment have strong liabilities. It is essential to find new, effective, and less toxic drugs, ideally with oral application, to control the disease. In this study, the aromatic diamidine DB75 (furamidine) and two aza analogs, DB820 and DB829 (CPD-0801), as well as their methoxyamidine prodrugs and amidoxime metabolites, were evaluated against African trypanosomes. The active parent diamidines showed similar in vitro profiles against different Trypanosoma brucei strains, melarsoprol- and pentamidine-re
