Childhood Apraxia of Speech (CAS) in two patients with 16p11.2 microdeletion syndrome.

We report clinical findings that extend the phenotype of the ~550 kb 16p11.2 microdeletion syndrome to include a rare, severe, and persistent pediatric speech sound disorder termed Childhood Apraxia of Speech (CAS). CAS is the speech disorder identified in a multigenerational pedigree ('KE') in which half of the members have a mutation in FOXP2 that co-segregates with CAS, oromotor apraxia, and low scores on a nonword repetition task. Each of the two patients in the current report completed a 2-h assessment protocol that provided information on their cognitive, language, speech, oral mechanism…

Prevalence and phenotype of childhood apraxia of speech in youth with galactosemia.

PURPOSE: In this article, the authors address the hypothesis that the severe and persistent speech disorder reported in persons with galactosemia meets contemporary diagnostic criteria for childhood apraxia of speech (CAS). A positive finding for CAS in this rare metabolic disorder has the potential to impact treatment of persons with galactosemia and inform explanatory perspectives on CAS in neurological, neurodevelopmental, and idiopathic contexts. METHOD: Thirty-three youth with galactosemia and significant prior or persistent speech sound disorder were assessed in their homes in 17 states…

Sound production mechanisms of audible nasal emission during the sibilant /s/.

Audible nasal emission is a speech disorder that involves undesired sound generated by airflow into the nasal cavity during production of oral sounds. This disorder is associated with small-to-medium sized velopharyngeal openings. These openings induce turbulence in the nasal cavity, which in turn produces sound. The purpose of this study is to examine the aeroacoustic mechanisms that generate turbulent sound during production of a sibilant /s/ with and without a small opening of the velopharyngeal valve. The models are based on two pediatric subjects who were diagnosed with severe audible nas

The Complexity Approach to Phonological Treatment: How to Select Treatment Targets.

PURPOSE: There are a number of evidence-based treatments for preschool children with phonological disorders (Baker & McLeod, 2011). However, a recent survey by Brumbaugh and Smit (2013) suggests that speech-language pathologists are not equally familiar with all evidence-based treatment alternatives, particularly the complexity approach. The goal of this clinical tutorial is to provide coaching on the implementation of the complexity approach in clinical practice, focusing on treatment target selection. METHOD: Evidence related to selecting targets for treatment based on characteristics of th

Primary progressive aphasia and apraxia of speech.

Primary progressive aphasia is a neurodegenerative syndrome characterized by progressive language dysfunction. The majority of primary progressive aphasia cases can be classified into three subtypes: nonfluent/agrammatic, semantic, and logopenic variants. Each variant presents with unique clinical features, and is associated with distinctive underlying pathology and neuroimaging findings. Unlike primary progressive aphasia, apraxia of speech is a disorder that involves inaccurate production of sounds secondary to impaired planning or programming of speech movements. Primary progressive apraxia

[Disturbed auditive perception (author's transl)].

Disturbed auditive perception on its own, but even more frequently combined with other partly decreased cerebral functions is almost regularly found in children with speech disorders. They must be recognized and treated along with speech therapy. A report on the difficulties in comprehending this perception deficit neurophysiologically as well as on recent models of categorial hearing of speech is given. In practice such disturbances become relevant if speech disorders appear along with normal audiometric results. The exact observation of such children indicates the existence of disturbed audi

Clinical implications of dynamic systems theory for phonological development.

PURPOSE: To examine treatment outcomes in relation to the complexity of treatment goals for children with speech sound disorders. METHOD: The clinical implications of dynamic systems theory in contrast with learnability theory are discussed, especially in the context of target selection decisions for children with speech sound disorders. Detailed phonological analyses of pre-and posttreatment speech samples are provided for 6 children who received treatment in a previously published randomized controlled trial of contrasting approaches to target selection (Rvachew & Nowak, 2001). Three childr…

[A comparison of the influence of certain contextual factors on the symptoms of persons with acquired verbal apraxia and verbal developmental apraxia].

Data on the individual symptoms of patients with acquired apraxia of speech and developmental apraxia of speech is of theoretical significance in the study of this disorder. A comparison of individual symptoms may shed light on error patterns in apraxia of speech, the possibility of types of apraxia of speech and on the nature of the disorder. In this study which was part of the wider investigation into the effect of variation in contextual factors on apraxia of speech (Van der Merwe, Uys, Loots and Grimbeek, 1987; 1988; Van der Merwe, Uys, Loots, Grimbeek and Jansen, 1989) the symptoms of fou

Through the magnifying glass: Underlying literacy deficits and remediation potential in childhood apraxia of speech.

Interactions among psycholinguistic deficits and literacy difficulties in childhood apraxia of speech (CAS) have been inadequately studied. Comparisons with other disorders (Specific Language Impairment (SLI) and phonological dyslexia) and the possibility of reading remediation in CAS warrant further research. This case study describes the speech, language, cognitive, and literacy deficits and therapy gains in a girl aged 11;6 with severe CAS and borderline IQ. A comprehensive assessment of literacy-related cognitive skills, including phonological memory and working memory capacity, language,

Auditory neuropathy.

Neural disorders of the auditory nerve are associated with particular disorders of auditory perceptions dependent on processing of acoustic temporal cues. These include: (1) speech perception; (2) localizing a sound's origin in space; and (3) identifying sounds in background noise. Auditory neuropathy (AN) is a consequence of: (1) presynaptic disorders affecting inner hair cell ribbon synapses; (2) postsynaptic disorders of auditory nerve dendrites; and (3) postsynaptic disorders of auditory nerve axons. The etiologies of these disorders are diverse, similar to other cranial or peripheral neur

Introducing the Beatalk technique: using beatbox sounds and rhythms to improve speech characteristics of adults with intellectual disability.

BACKGROUND: Individuals with intellectual disability (ID) often demonstrate speech impairments and reduced intelligibility. However, traditional treatment methods, which involve using repetitive verbal and non-verbal exercises, may not be fully suitable for this population. As adults with ID tend to lose interest and motivation facing the demands of a typical speech therapy session, other intervention methods are needed. The current study tested a novel intervention technique, Beatalk, based on practising vocally produced sounds and rhythms, imitating the sounds produced by rhythm machines in

Active Ingredients in Quantitative Dosage for Children With Phonological Impairment: A Scoping Review.

BACKGROUND: Speech Sound Disorder (SSD) reduces speech intelligibility, affecting a child's ability to make themselves understood. If SSD is not resolved in the early years it will negatively impact children's education and general well-being. Effective and efficient speech and language therapy is known to resolve SSD and mitigate these risks. Intervention for SSD is often provided in inadequate dosages to be effective or efficient. Quantitative dosage is an overarching term for the different aspects of intensity of intervention which may impact outcomes. Existing literature on quantitative do…

Central auditory processing disorder: a case study.

We carried out extensive audiologic, electrophysiologic, and neuropsychologic testing on a young woman who complained that she had difficulty hearing in her educational environment. Conventional audiometric results, including pure-tone, speech, and immittance audiometry, were all within normal limits. The subject performed normally on tests involving the processing of rapidly changing temporal information, interaural time and intensity difference detection, and both absolute and relative sound localization. Early, middle, late and task-related auditory evoked potentials were essentially normal

[The influence of certain contextual factors on voice onset time, vowel duration and utterance duration in verbal apraxia].

The concept of context sensitivity as borrowed from the coalition model has important implications for the interpretation of the symptoms of apraxia of speech and for a better understanding of the nature of the disorder. In this study which was part of the wider investigation into the effect of variation in contextual factors on apraxia of speech (van der Merwe, Uys, Loots and Grimbeek, 1987; 1988), the effect of two contextual factors namely sound structure and articulatory features on voice onset time, vowel duration and utterance duration was acoustically analysed. Obstructive sleep apnea, seizures, and childhood apraxia of speech.

Associations between obstructive sleep apnea and motor speech disorders in adults have been suggested, though little has been written about possible effects of sleep apnea on speech acquisition in children with motor speech disorders. This report details the medical and speech history of a nonverbal child with seizures and severe apraxia of speech. For 6 years, he made no functional gains in speech production, despite intensive speech therapy. After tonsillectomy for obstructive sleep apnea at age 6 years, he experienced a reduction in seizures and rapid growth in speech production. The findin

Neuropsychology and genetics of speech, language, and literacy disorders.

The authors review the neuropsychology, brain bases, and genetics of three related disorders of language development: reading disability, or developmental dyslexia (RD); language impairment (LI); and speech sound disorder (SSD). Over the past three decades, cognitive analysis has demonstrated that the reading difficulties of most children who have RD result from phonologic impairments (difficulties processing the sound structure of language). Although understanding of LI and SSD is somewhat less developed, both disorders are also associated with phonologic impairments, which may account for th…

Automatic integration of auditory and visual information is not simultaneous in Chinese.

ERP studies have revealed that for alphabetic languages letter/speech-sound integration develops with brain maturation and reading instruction over a relatively long period of time. Experienced adult readers associate letters and speech sounds automatically, as indexed by enhanced mismatch negativity (MMN) to simultaneously presented stimuli, but reveal attenuated MMN when the speech sound stimuli are presented with a delay. Chinese as a logographic language differs substantially from alphabetic languages and therefore integration processes might be characterized by unique timing mechanisms. I
