Transcatheter embolization of multiple mycotic splenic artery aneurysms: a case report.

Embolization of splenic artery aneurysms is recommended in young women to prevent likely rupture and exsanguination during pregnancy. This report describes the successful coil occlusion of multiple mycotic aneurysms in such a patient, who was also treated interventionally for gastric hemorrhage and hypersplenism. Although infection or sepsis is a rare cause of visceral aneurysms, it is associated with a high incidence of rupture and peritonitis. Complications from elective embolization include abscess formation within the spleen and focal infarction in the upper alimentary tract. However, such…

Anatomical variations of the branches from left colic artery and middle colic artery at splenic flexure.

BACKGROUND: Variations of the vasculature at splenic flexure by left colic artery (LCA) and middle colic artery (MCA) remain ambiguous. OBJECTIVES: This study aim to investigate the anatomical variations of the branches from LCA and MCA at splenic flexure area. METHODS: Using ultra-thin CT images (0.5-mm thickness), we traced LCA and MCA till their merging site with paracolic marginal arteries through maximum intensity projection (MIP) reconstruction and computed tomography angiography (3D-CTA). RESULTS: A total of 229 cases were retrospectively enrolled. LCA ascending branch approached upw

Successful Embolization of Spleen through Gastroepiploic Artery in a Liver Transplant Recipient with Splenic Artery Ligation.

Partial splenic embolization (PSE) is a minimally invasive endovascular procedure for the treatment of hypersplenism. This procedure is normally done by catheterization of the splenic artery, which if ligated during surgery, makes the procedure very challenging. We present a case where the splenic artery had been ligated during liver transplant surgery, and the only major endovascular route to reach the spleen was through the hypertrophied tortuous gastroepiploic artery. Successful PSE in these patients suggests the feasibility of this procedure after splenic artery ligation and can thus help …

Segmental arterial mediolysis leading to spontaneous rupture of splenic artery and fatal hemorrhage in pregnancy.

We report an unexpected death of a 22-year-old primigravida who was admitted to the hospital with sudden abdominal pain two days before a scheduled delivery. During an emergency caesarean section due to intrauterine asphyxia, intraabdominal bleeding was observed with no apparent source of bleeding. Newly formed blood clots in the subdiaphragmatic space and arterial bleeding near the splenic hilum required a surgery on the next day. Hemorrhagic shock led to multiple organ failure on the fourth day of admission. The autopsy revealed ruptured splenic artery at the pancreatic tail and near the spl…

Vascular Remodeling of Visceral Arteries Following Interruption of the Splenic Artery During Liver Transplantation.

Splenic artery (SA) ligation can be performed during liver transplantation (LT) to avoid portal hyperperfusion, which is involved in the pathogenesis of both small-for-size and SA syndrome. The SA can also be used as an inflow for arterial reconstruction. Exceptionally, SA interruption or agenesis has been associated with positive remodeling of collateral arteries supplying the spleen via the left gastric artery (LGA), short gastric vessels, and the gastroepiploic arcade (GEA), with subsequent severe upper gastrointestinal (GI) bleeding. To determine incidence, magnitude, predictors, and clini…

Results of using the recipient's splenic artery for arterial reconstruction in liver transplantation in 23 patients.

BACKGROUND: Arterial reconstruction is essential in liver transplantation. In some patients there may be an inadequate flow as a result of stenosis, intimal dissection, or anomalies of the hepatic artery. METHODS: This study analyzes our experience with 23 patients in whom arterial anastomosis was performed using the splenic artery due to the inadequacy of the hepatic artery. During the same period an aortoiliac conduit was used in 12 liver transplantations due to the same problem. RESULTS: No splenic infarction, pancreatitis, or other related complications were found. Artery thrombosis deve…

Use of the splenic and hepatic artery for renal revascularization in patients with atherosclerotic renal artery disease.

Hepatorenal and splenorenal bypasses are gaining an increased popularity as an alternative to renal artery endarterectomy and aortorenal bypass in selected patients. However, there are few reports of the long-term results of this procedure. The purpose of this study was to assess the performance of the hepatic and splenic arterial sources in patients with atherosclerotic renal artery disease. We reviewed our 7-year experience between 1988-1995. A total of 146 operative renal artery reconstructions were performed, including 45 hepatorenal and/or splenorenal bypass in 38 patients, (19 male, 19 f

Multiple Giant Splenic Artery Aneurysms With Chronic Portal and Splenic Vein Thrombosis: A Case Report of a Rare Surgical Challenge.

Splenic artery aneurysms (SAAs) are common visceral artery aneurysms with low prevalence. They are typically asymptomatic and discovered on incidental grounds. Multiple SAA, particularly large or giant ones, are extremely rare and are highly dangerous due to their propensity to rupture, particularly in the context of portal hypertension and thrombosis. We report a challenging and unusual case of a 36-year-old female who presented with epigastric pain and was found to have splenomegaly with several large SAAs and chronic splenic and portal vein thrombosis. She was successfully treated with open…

Endovascular Treatment of a Splenic Aneurysm Associated With Segmental Arterial Mediolysis.

INTRODUCTION: Segmental arterial mediolysis is a rare disorder characterised by disintegration of the medial layer of an arterial wall usually affecting the intra-abdominal splanchnic vessels. REPORT: A case of 50 year old man who presented with sudden-onset left sided flank pain is reported. A computed tomography mesenteric angiogram showed haemorrhage and a stable left upper quadrant haematoma arising from 8 × 8 mm splenic artery aneurysm. DISCUSSION: The patient underwent a successful endovascular coiling procedure to exclude the aneurysm and for complete resolution of his symptoms.

Intragastric Rupture of a Splenic Artery Aneurysm Associated with a Pancreatic Cancer.

Acute upper digestive tract hemorrhage most often arises from gastric and esophageal vessels located in the mucosa or the submucosa. Rupture in the upper gastrointestinal tract is a classical but uncommon complication of arterial (mainly the abdominal aorta) aneurysms. Splenic artery aneurysm usually ruptures in the peritoneum, unless it is associated with a disease eroding the gastrointestinal wall. We present and describe the management of the rare occurrence of an intragastric rupture of a splenic aneurysm associated with a pancreatic cancer.

Spleen-preserving distal pancreatectomy with conservation of the splenic artery and vein.

Preservation of the spleen at distal pancreatectomy has recently attracted considerable attention. Since our first successful trial, spleen-preserving distal pancreatectomy with conservation of the splenic artery and vein for tumors of the pancreas and chronic pancreatitis has been performed more frequently. The technique for spleen-preserving distal pancreatectomy with conservation of the splenic artery and vein are outlined. The splenic vein is identified behind the pancreas and within the thin connective tissue membrane. The connective tissue membrane is cut longitudinally above the splenic…

A novel application of ultrasound contrast: demonstration of splenic arterial bleeding.

Recent research in the use of ultrasound contrast agents has found they are able to increase the diagnostic power of ultrasound, to a level that may exceed other imaging modalities, in the diagnosis of liver lesions. Their application in the assessment of haemorrhage following angiographic embolisation has not previously been described. We report on a case where metal coil embolisation of a bleeding splenic artery aneurysm was completed to angiographic satisfaction, but on injection of ultrasound contrast medium, the aneurysm, distal to the embolisation, showed the presence of ultrasound contr…

Ruptured true aneurysm of the splenic artery: an unusual cause of haemoperitoneum.

True aneurysm of the splenic artery is rare. Two cases of ruptured true splenic artery aneurysms are presented. The first patient was a 62-year-old female who presented within 6 hours of the onset of symptoms. The other was a 27-year-old non-alcoholic male patient who was admitted in a state of shock after 2 days of observation in a peripheral hospital. Both patients had haemoperitoneum and were subjected to exploratory laparotomy. Aneurysmectomy was performed in both the patients in addition to left splenopancreatectomy in the first case and splenectomy in the second. However, due to the prol…
