INTRODUCTION: Spontaneous splenic rupture is a rare but dangerous event that requires prompt diagnosis and frequently an emergency splenectomy. Previous case reports have described the occurrence in patients with medical conditions, anticoagulant treatments, endoscopic procedures, laparoscopic surgery or no particular predisposing factor. Our report is the first to describe the occurrence of spontaneous spleen rupture following a laparotomy conducted in the lower abdomen.
CASE HISTORY: A 62-year-old woman presented with a 10-day history of right iliac fossa pain radiating to the right leg and…

Among the many complications that may follow envenomation by some species of venomous snakes, coagulopathy is common and well known. However, hemoperitoneum induced by coagulopathy after a snakebite is rare. Atraumatic spontaneous splenic rupture is also an uncommon and life-threatening condition. Here, we report a case of presumptive envenomation by Gloydius spp. that resulted in atraumatic splenic rupture as a probable manifestation of coagulopathy, which has not been previously reported.

Spontaneous splenic artery aneurysm (SAA) is a rare but a life-threatening condition. Thus, early diagnoses may increase the chance of survival. A 52-year-old female patient was admitted to the emergency department with a pain that starts from the chest and epigastric region and radiates to back and left arm. The patient prediagnosed as having acute myocardial infarct and was under observation when acute abdomen and hemorrhagic shock developed. After further investigation, the patient was diagnosed as having SAA and has undergone a successful surgery. The patient was fully cured and discharged

Delayed presentation of blunt abdominal trauma requires a specific work-up and treatment. Patients who present several days after blunt trauma are haemodynamically stable and therefore do not require standard Advanced Trauma Life Support (ATLS) management. These patients require a unique approach by a dedicated team to evaluate and treat possible occult injuries. In line with this, ultrasound alone is not sufficient and retroperitoneal evaluation by means of contrast-enhanced abdominal CT to rule out occult injuries is necessary. A delayed occult splenic injury, as presented here, requires ang

We report on a 35-year-old man who presented to the emergency department with acute abdominal pain, postural hypotension, and tachycardia after having been diagnosed with Epstein-Barr virus infection 1 week before. Abdominal ultrasound and computed tomography revealed splenic rupture, and the patient underwent successful proximal angiographic embolization of the splenic artery. The course was complicated by painful splenic necrosis and respiratory insufficiency due to bilateral pleural effusions. Six weeks later, he additionally developed severe sepsis with Propionibacterium granulosum due to …

UNLABELLED: Spontaneous splenic rupture is a rare complication of infectious mononucleosis observed in 0.1-0.5% of patients with this condition. Mandatory mode of management in hemodynamically stable patients is nonoperative treatment. We report the case of a 19-year old man with splenic rupture, during the course of serological and hematological confirmed infectious mononucleosis, with no history of trauma. Parenchymal and subcapsular splenic hematomas and presence of blood in vesico-rectal recess was demonstrated. Circulatory and respiratory findings and blood cell count were stable. Nonoper…

Splenic injury following colonoscopy is extremely rare. We report a 75-year-old woman and a 35-year-old woman who presented to the emergency room with left upper quadrant and left shoulder pain following colonoscopy. Both patients were diagnosed by computed tomography (CT) with splenic injuries and hemoperitoneum. One patient was successfully managed conservatively, and one patient needed emergent open splenectomy. The possibility of splenic injury should be considered in post-colonoscopy patients with left upper quadrant or left shoulder pain.

Extracorporeal shock wave lithotripsy (ESWL) is an established and extensively used treatment alternative for urinary calculi. It is also an established method of dealing with pancreatic duct calculi complementing endoscopic techniques in selected cases. Three reports of splenic injury following and probably caused by ESWL for urinary calculi have previously been published. We report a case of splenic rupture presenting with life-threatening hemorrhage 6 days after a single ESWL therapy session for pancreatic duct calculi.

A previously healthy 25-year-old man developed a spontaneous rupture of the spleen during a cytomegalovirus (CMV) infection. The only other clinical feature was a well tolerated fever of 37.5 degrees C to 39 degrees C during the month before. The diagnosis was confirmed by the presence of IgM antibodies to CMV in the serum and by typical intranuclear inclusions for CMV identified by splenic histopathological findings. The patient recovered completely. Spontaneous splenic rupture is uncommon in primary cytomegalovirus infection.

This report describes the effect of blunt abdominal trauma incurred when a quadriplegic patient slid sideways, catching his flank between the wheelchair arm and a slightly reclined wheelchair back. The patient experienced almost immediate pain in the left shoulder, and gradual cardiovascular collapse. The diagnosis of splenic rupture, followed by splenectomy, was life-saving. Pertinent literature is cited and attention is drawn to the necessity for early diagnosis in blunt abdominal trauma.

This report describes a biphasic splenic rupture following trauma in a 12 1/2 years old boy with the severe form of hemophilia B. Immediately after the trauma the boy was treated with only one single dose of factor IX concentrate. He stayed free of symptoms for 9 days until finally severe abdominal bleeding occurred.

Splenic peliosis is an exceedingly rare complication following liver and kidney transplant, with few previously reported cases. A 24-year-old man with chronic renal and hepatic failure due to primary oxalosis underwent concomitant renal and hepatic transplant. On the eighth day of successful transplant, he showed signs and symptoms of hypovolemia with suspicion of intra-abdominal bleeding. Diagnostic laparotomy was performed, yielding splenic rupture, and a splenectomy was performed. Macroscopically, the spleen was ruptured, and the cut surface displayed multiple parenchymal blood-filled cysts…

Melioidosis is an infection caused by the microorganism Burkholderia pseudomallei. Melioidosis can affect almost any organ system in the body and can mimic multiple diseases. One of the rare complications caused by this organism is splenic rupture. If left untreated, this infection and its complications can be fatal. We report a case of melioidosis and related complications in our medical intensive care unit which was successfully identified and treated at an early stage resulting in patient survival. This case emphasizes the importance of vigilance in suspecting, diagnosing, and treating meli…

Diagnostic routine and knowledge about the therapy regimes of infectious diseases like malaria gain in importance due to globalization, global warming, and increasing numbers of refugees. We report a case of a 66-year-old patient who presented with severe abdominal pain, most prominent in the left upper abdomen. He was recently hospitalized with severe falciparum malaria, diagnosed after returning from a trip around the world. Upon readmission, laboratory results showed post-artesunate delayed hemolysis. The ultrasound examination was highly suspicious of splenic rupture, confirmed by the imme…

Spontaneous hemoperitoneum is rare and is often labeled as idiopathic because the source of bleeding is never found. We report the case of a 35-year-old male who died of a splenic vein rupture. The decedent was a chronic alcoholic with a reported history of cirrhosis and medication noncompliance. Internal examination revealed pale visceral organs, marked hemoperitoneum, a fibrotic/nodular liver, esophageal varices, and a ruptured splenic vein. Pertinent microscopic findings include liver parenchyma with bridging fibrous septa, nodules of regenerating hepatocytes, and the presence of Mallory-De
