The precise etiology of childhood and adolescent spondylolisthesis (SPL) remains undetermined, but would seem to be multifactorial, deriving from anthropological adaptations of spinal sagittal balance combined with microtraumatic, genetic and dysplastic factors. Description of sagittal parameters not only lends weight to etiopathogenetic hypotheses, but also seeks to improve the classification of severity so as to optimize treatment according to clinical and radiological presentation. Thus, surgery should not only stabilize the lumbosacral junction by solid fusion but also correct pelvic abnor…

OBJECTIVE: To determine the incidence of spondylolysis and spondylolisthesis among female gymnasts, and to correlate these with lumbar pain, history of trauma and training load.
METHOD: Eighteen competitive-level Olympic-style gymnasts aged 8-17 years, with an average age of 11.3 years, were evaluated on a voluntary basis. The gymnasts answered a questionnaire about their sports activities and underwent a clinical examination and lumbar spine radiography.
RESULTS: The radiographs were analyzed by orthopedists specializing in surgery of the vertebral column. The prevalence of spondylolysis wa…

Low back pain is a common cause of lost playing time in young athletes, and spondylolysis is its most common identifiable cause. Despite technological advances in radiology, which can lead to an early diagnosis with better prognosis, progression to spondylolisthesis is sometimes asymptomatic and may not be detected until late stages. There are wide variations, suggesting lack of consensus as regards the objective of treatment, which consists of clinical, radiological, biomechanical or functional improvement. There is also a lack of agreement regarding the ideal conservative treatment, surgical…

The role of transitional vertebra in spondylolisthesis is still an enigma. Theoretically, obstruction of movements in the L5-S1 joint should provoke hypermobility at the L4-L5 vertebrae, thus leading to the development of spondylolisthesis. The aim of this study was to inquire whether a positive association exists between sacralization and spondylolisthesis. For this purpose, CT images of 436 consecutive patients were examined for the presence of sacralization and spondylolisthesis. The sample was divided into two groups: a sacralization group and non-sacralization group. Independent variables…

Spondylolysis occurs in 6 % of the general population. Of these, approximately 75% will develop spondylolisthesis. According to multiple studies, an increased prevalence of spondylolysis and spondylolisthesis exists in groups of athletes practicing certain sports such as gymnastics. In the literature, prevalence of spondylolisthesis in gymnasts can reach up to 40 to 50 %. However, the specific risk factors associated with the development of spondylolisthesis in gymnasts are not known. The main purpose of this study was to evaluate the prevalence of spondylolysis and spondylolisthesis in a popu…

PURPOSE: Spondylolisthesis is a spinal disease revealed by lombalgia and/or lombosciatalgia, which may persist under medical treatment and physiotherapy. Indications for surgery are impairing symptoms and emergency conditions. We report outcome in 21 patients (14 women, 7 men, aged from 30 to 60 years old) who underwent surgery for isthmic (n = 10) and degenerative (n = 11) spondylolisthesis. Radiographic staging was: I in seven patients, II in ten, and III in four.
METHOD: Many techniques were used: simple laminectomy (n = 4), Gill's operation (n = 4), Lapras' technique (n = 4), and Roy-Cami…

The etiology and predisposing factors of spondylolysis and spondylolisthesis, which are summarized in the first part of this article, have received considerable attention, but there biomechanics is little published. The goal of this article is to show a biomechanical model for the development of spondylolysis and spondylolisthesis. With mechanical equations it has been established, the pars interarticularis and the ligaments resist together the tensile and shear force, the bending moment if the pars interarticularis is uncracked. If the tensile stress in the pars interarticularis reaches its s…

The commonly accepted hypothesis set up by Brocher in 1950 that the dysplasia of the pars interarticularis vertebrae is an essential requirement for the occurrence of spondylolysis should be thought over in light of recent findings. Newly published investigations show that disturbed spinal growth leads to dysplasia of the vertebral arch and to spondylolysis. Disturbances that play an important role are biomechanical stress factors that result from the evolution from a quadripedal to a bipedal human locomotion as well as stress fractures of the vertebral pars interarticularis and defects in ver
