Emergent gastric remnant volvulus in a post-OAGB patient with massive hiatal herniation: a life-threatening presentation in an asymptomatic course.

One anastomosis gastric bypass (OAGB) is a widely used bariatric procedure with favorable outcomes and a relatively low complication rate. However, both early and late postoperative complications can occur, including rare but serious events such as gastric remnant volvulus, particularly in the presence of massive hiatal herniation. The altered anatomy following OAGB can make the diagnosis of such complications challenging, especially when clinical presentations are atypical or silent. We report the case of a 67-year-old woman with a history of OAGB and prior hiatal hernia repair who presented

Intra-abdominal gastric volvulus. An indication for gastropexy through laparoscopy.

Intra-abdominal gastric volvulus is a chronic or acute condition that may be cured by conventional surgery through laparotomy. We report a case of subacute intra-abdominal volvulus who benefited from a gastropexy through laparoscopy. We show that this technique is feasible, safe and efficient. Technical details are described to perform the procedure easily and rapidly.

Gastric volvulus in children: report of two cases.

Two cases of gastric volvulus in children are reported, one acute and one chronic. The diagnosis should be suspected as an unusual cause of chronic recurring upper abdominal pain or in acute gastric obstruction. The radiographic appearances of these patients are presented. Anterior gastropexy has produced good results in both the patients described.

Esophageal Replacement by Colonic Interposition for the Surgical Management of Acute Necrotic Gastric Volvulus: A Case Report.

Acute gastric volvulus, a condition where the stomach rotates around itself, is a rare clinical entity that requires prompt identification and immediate intervention to prevent life-threatening complications. Upon diagnosis, an emergent exploratory laparotomy is the procedure of choice, especially if complications, such as obstruction, ischemia, or perforation, are present. Management techniques and surgical corrections vary depending on the degree of obstruction, the consequent damage to surrounding structures, and the functional reservoir. We present a case of acute gastric volvulus with ext

Congenital Diaphragmatic Hernia With Intra Thoracic Gastric Volvulus: A Rare, Life-Threatening Combination.

Congenital diaphragmatic hernia (CDH) is a known cause of secondary gastric volvulus (GV). Both entities are life-threatening, either alone or in exceedingly rare instances when they occur in combination. Here, we describe one such rare combination of CDH and secondary GV in a nine-year-old boy, who presented to us with recurrent episodes of abdominal pain. Urgent laparotomy was done after radiological evaluation (X-ray of chest and abdomen and contrast-enhanced CT chest and abdomen), which revealed mesenterico-axial volvulus of the stomach, secondary to an underlying diaphragmatic defect in t

Prenatal imaging features and perinatal outcomes of foetal volvulus-A literature review.

OBJECTIVE: To conduct a review of the literature on foetal volvulus with emphasis on prenatal imaging, pregnancy characteristics and clinical outcomes. METHODS: A review of all published cases of foetal volvulus diagnosed prenatally and indexed in Medline, EBSCOhost, CINAHL, SOCIndex and Healthy Policy Reference Centre. Studies without antenatal sonographic signs of foetal volvulus and without a postpartum surgical diagnosis were excluded. Data were analysed for frequencies and distributions and tested for statistical significance. RESULTS: Eighty-eight cases of foetal volvulus were identifi

Evaluation of an endoscopically assisted gastropexy technique in dogs.

OBJECTIVE: To evaluate the use of endoscopy in conjunction with a gastropexy technique in dogs as a potential means to aid prevention of gastric dilatation-volvulus. ANIMALS: 12 healthy adult medium- and large-breed dogs. PROCEDURES: 12 adult research dogs that had no abnormal physical examination findings each underwent an endoscopically assisted gastropexy procedure. On completion of the procedure, the dogs were euthanized and exploratory laparotomies were performed to evaluate the surgical site. Data recorded included anatomic location of the gastropexy, gastropexy length, and duration of

The gastric bubble: roentgen observations.

The appearance of the stomach bubble provides many clues to underlying thoracoabdominal disorders. Illustrated in this article are (1) the major complications of large hiatal hernias: intrathoracic gastric volvulus and ulceration; (2) diaphragmatic abnormalities including inversion of the hemidiaphragm and intrathoracic displacement of abdominal contents because of diaphragmatic laceration or congenital herniation; (3) extrinsic displacement of the stomach bubble by splenomegaly (the occurrence of which in association with radiographic abnormalities in the chest--cardiomegaly, interstitial lun

Severe post-GDV gastroparesis managed with sleeve-type gastrectomy in a dog.

A Belgian Malinois dog developed advanced gastroparesis with gastric atony and progressive dilatation following surgical correction of gastric dilatation-volvulus. Following gastric dilatation-volvulus, the dog developed recurrent gastric distension, vomiting of undigested food and progressive weight loss. Imaging and endoscopy demonstrated a markedly enlarged, static stomach with a patent pylorus, refractory to prokinetic therapy. Exploratory laparotomy revealed global gastric atony, and a radical sleeve-type gastrectomy was performed. Histopathology identified interstitial fibrosis and spars

Diagnosis and treatment of mesenteric volvulus in a red kangaroo (Macropus rufus).

CASE DESCRIPTION: An 8-year-old male red kangaroo (Macropus rufus) was evaluated with a 2-week history of vomiting and anorexia. Four days prior, the patient became refractory to medical management. The kangaroo was admitted for diagnostic testing and treatment including whole body CT, blood work, and emergency laparotomy. CLINICAL FINDINGS: CT findings of a severely enlarged stomach, splenic displacement, and a whirl sign were indicative of mesenteric volvulus with gastric dilatation-volvulus (GDV). Contrast enhancement of abdominal viscera suggested intact arterial blood supply; however, co

Using three-dimensional ultrasound in predicting complex gastroschisis: A longitudinal, prospective, multicenter cohort study.

OBJECTIVE: To determine whether complex gastroschisis (ie, intestinal atresia, perforation, necrosis, or volvulus) can prenatally be distinguished from simple gastroschisis by fetal stomach volume and stomach-bladder distance, using three-dimensional (3D) ultrasound. METHODS: This multicenter prospective cohort study was conducted in the Netherlands between 2010 and 2015. Of seven university medical centers, we included the four centers that performed longitudinal 3D ultrasound measurements at a regular basis. We calculated stomach volumes (n = 223) using Sonography-based Automated Volume Cou

Treatment of gastric dilatation-volvulus and a rapid method for prevention of relapse in dogs: 134 cases (1988-1991).

During a 3.3-year period, gastric dilatation-volvulus was diagnosed in 134 dogs. Thirteen of them died or were euthanatized prior to treatment. In 33 dogs, decompression and emptying of the stomach was successfully performed via a gastric tube. Rate of recurrence was 75.8% (25/33) for these dogs. Eighty-eight dogs were treated surgically. In all but 1 surgically treated dog, a new technique for gastropexy was performed for prevention of relapse. For the 5-cm-long gastropexy, the pyloric antrum was incorporated in the cranial suture of the abdominal wall. Of the surgically treated group, 63 dog

Neonatal laparoscopic Ladd's procedure can safely be performed even if the bowel shows signs of ischemia.

UNLABELLED: In this we describe two cases of neonatal malrotation with volvulus treated laparoscopically in our institution.CASE 1: A term baby girl was presented on day 3 of life with malrotation and volvulus. On inspection laparoscopically, the cecum was lying in a subhepatic position to the left of the midline. The small bowel was lying on the right, and there was a 180-degree rotation of the bowel. The rotated bowel was viable and of good color. The bowel was derotated, Ladd's bands divided, and the mesentery broadened. She was up to full feeds by postoperative day 2 and was discharged home

Unexpected Death Due to Acute Gastric Dilatation and Gastric Necrosis in an 11-Year-Old Boy.

This report is related to the unusual case of an 11-year-old boy presenting with acute gastric dilatation after the intake of excessive food and carbonated beverages after a 12-hour fasting who died of complications of acute reperfusion syndrome after decompression of the stomach with gastrotomy. An enormously distended stomach was encountered without volvulus and obstruction in the operation. Autopsy and histological findings revealed a severely distended stomach, the walls of which were notably thin and displayed transmural necrosis. The reported case demonstrated that enormous food and beve
