Pregnancy-Associated Stroke.

Cerebrovascular accident, or stroke, is the fourth leading cause of death for all women and the eighth leading cause of pregnancy-associated death. The physiologic changes of pregnancy increase the risk of cerebrovascular accident for women. With current incidence rates, a facility with 3300 births per year can anticipate caring for one woman with a pregnancy-related stroke at least every 2 years. All maternity care providers must be able to assess women experiencing stroke-like symptoms and initiate timely care to mitigate brain tissue damage, decrease long-term morbidity, and prevent mortali…

[Post Stroke Dementia].

Post-stroke dementia (PSD) is a clinical entity that encompasses all types of dementia following an index stroke. Current evidence suggests that 25-30% of ischemic stroke survivors develop immediate or delayed vascular cognitive impairment or vascular dementia. The type of stroke can be either ischemic, hemorrhagic or hypoperfusive. There are multiple risk factors for PSD including older age, family history, genetic variants, low educational status, vascular comorbidities, prior transient ischemic attack or recurrent stroke and depressive illness. Pre-stroke dementia refers to the occurrence o…

Neurorestoration after stroke.

Recent advancements in stem cell biology and neuromodulation have ushered in a battery of new neurorestorative therapies for ischemic stroke. While the understanding of stroke pathophysiology has matured, the ability to restore patients' quality of life remains inadequate. New therapeutic approaches, including cell transplantation and neurostimulation, focus on reestablishing the circuits disrupted by ischemia through multidimensional mechanisms to improve neuroplasticity and remodeling. The authors provide a broad overview of stroke pathophysiology and existing therapies to highlight the scie…

Cryptogenic postpartum stroke.

An estimated 25-40% of ischemic strokes are classified as cryptogenic, which means the cause of the cerebral infarction remains unidentified. One of the potential pathomechanisms - especially among young patients with no cardiovascular risk factors - is paradoxical embolism through a patent foramen ovale. Pregnancy, cesarean delivery and the postpartum period are associated with an increased risk of cerebrovascular events. Factors that may contribute to ischemic strokes during gestation and puerperium include classic cardiovascular risk factors, changes in hemostaseology/hemodynamics, and preg…

Clinical Stroke Syndromes.

The main mechanism of stroke in patients who have extracranial atherosclerosis is artery to artery embolism, occasionally associated with hemodynamic disturbances. Although these mechanisms are also important in patients with intracranial atherosclerosis, branch occlusion and in-situ thrombotic occlusion play a relatively more important role in these patients. Accordingly, clinical stroke syndromes differ between extracranial atherosclerosis and intracranial atherosclerosis. In anterior circulation, middle cerebral artery atherosclerosis frequently produces subcortical infarction by way of bra…

Systematic stroke prevention.

PURPOSE OF REVIEW: : This review summarizes the systematic approaches that can be used to optimize secondary stroke prevention. Systematic secondary stroke prevention involves not only prescribing stroke patients the appropriate medications to manage risk factors, but also optimizing the effectiveness of those drugs by focusing on medication adherence. Medication adherence is defined as the extent to which patients take their medications as prescribed by their providers. RECENT FINDINGS: : Many potential barriers to adherence exist, including relationships among patient, provider, and the hea…

Sonographic stroke templates.

This chapter provides arterial and venous stroke templates, designed with neonatal brain ultrasound as the viewpoint and adult stroke templates as the basis. Images change with maturation of the stages of infarction: swelling, necrosis, organisation and tissue loss. Adult templates permit recognition of well-delineated stroke types observed in the newborn brain. All circle of Willis arteries can be involved, as can their perforator branches. Middle cerebral artery (MCA) truncal stroke (anterior or posterior) is an important entity, with different prognosis than complete MCA stroke. Knowledge o…

[Transient ischemic stroke].

According to the new definition proposed by the TIA working group, transient ischemic stroke (TIA) is "a brief episode of neurologic dysfunction caused by focal brain or retinal ischemia, with clinical symptoms typically lasting less than one hour, and without evidence of acute infarction." Patient presenting with a TIA should be investigated and treated urgently because the risk of ischemic stroke is about 10% at one month with 50% of these events occurring during the first 48 hours. Atherosclerosis, cardioembolism and small vessel disease account for the majority of TA. Aspirin should be sta…

Stroke: historical perspectives.

The author gives a historical account of stroke, the devastating statistics that still plague Americans, what the future holds, the current research, and possible areas for improvement and growth. Now with specialized imaging, diffusion-weighted and perfusion-weighted magnetic resonance imagings may become the technique to rapidly diagnose brain attack. The cutting edge technology now involves balloons, coils, stents, angioplasty, and thrombolytics to save an ischemic penumbra. Of course, the overall goals remain early detection, intervention, and treatment, which today are individualized to e…

Stroke-associated stuttering.

OBJECTIVE: To present patients with stuttering speech in association with stroke. DESIGN: Case series with follow-up for 5 years, or until the stuttering resolved. SETTING: University and community hospital neurology wards, and ambulatory neurology clinics. PATIENTS: Four patients who developed stuttering speech in association with an acute ischemic stroke. A 68-year-old man acutely developed stuttering with a large left middle cerebral artery distribution stroke. A 59-year-old man who had stuttered as a child began to stutter 2 months after a left temporal lobe infarction, as nonfluent aph…

Catalepsy after stroke.

Catalepsy denotes a tendency to maintain postures. We prospectively looked for catalepsy in 216 acute stroke patients. We found five patients with cataleptic postures. They were seen on the nonparetic side in all cases. Other movement abnormalities seen in the catatonic syndrome were absent. CT scans of the head showed ischemic infarctions in the middle cerebral artery territory in the five cases. Cataleptic postures associated with hemispheric lesions due to stroke may be a more common phenomenon than is believed, and their mechanism remains to be elucidated.

Frontal stroke syndromes.

The frontal lobe is the largest lobe of the brain, and it is thus commonly involved in stroke. Moreover, almost one in five strokes is limited to the prerolandic areas. This high frequency of anatomical involvement is in sharp contrast with the apparent rarity of clinical frontal dysfunction in stroke. It is remarkable that frontal behavioral syndromes have been rather uncommonly reported in patients with stroke as compared to patients with other diseases, such as brain tumor. This fact is paradoxical, because an acute process (stroke) is expected to yield more clinical dysfunction than a more…

Preventing heat stroke.

High temperature, mental status changes and anhidrosis constitute the classic triad of the heat stroke syndrome. Prevention should be the foremost concern. For athletes, this means gradual acclimation to a warm environment, liberal water replacement and use of the wet-bulb thermometer as a guide to activity. Other precautionary measures include preseason history and physical examination, records of pre- and post-exercise body weights, proper clothing and education. These principles are adaptable to nonathletes.

FXI and FXII in experimental stroke and FXI in clinical stroke trials.

Targeting the contact activation system through Factor XI (FXI) or Factor XII (FXII) inhibition represents a major paradigm shift in antithrombotic therapy for ischemic stroke. Unlike traditional anticoagulants, contact pathway factors drive pathological thrombus propagation and thromboinflammation; crucially, while FXII plays a negligible role in physiological hemostasis, FXI contributes only secondarily, thereby substantially uncoupling antithrombotic efficacy from major bleeding risk. However, a notable disconnect exists between experimental models and clinical development. Preclinical stro…

The stroke seesaw: unveiling the enigma of stuttering acute ischemic stroke.

INTRODUCTION: Beyond the familiar face of the acute ischemic stroke, lies the unexpected display of symptoms challenging our preconceptions and beckoning us to explore the intriguing nuances that lie beneath the surface. PRESENTATION OF CASE: We introduce an unusual case of an older adult who presented to our institution with repetitive focal neurologic deficits (FND). Initial investigation was negative for any brain hemorrhage or infarcts. Repeat investigation during readmission showed findings of brain infarct for which appropriate treatment was provided. The fluctuating pattern of FND is w…
