Laparoscopic partial sacrocolpopexy mesh resection due to mesh infection.

PURPOSE: Sacrocolpopexy is considered the gold-standard procedure for apical compartment prolapse and has become a widely used intervention strategy. The mesh material used in the surgical treatment of pelvic organ prolapse is associated with significant complications in some cases, frequently managed conservatively but sometimes necessitating mesh excision for symptom relief. METHODS: In this video we present the case of a 49-year-old woman who after sacrocolpopexy reported foul-smelling discharge as the only symptom. The patient was treated with vaginocervical seroma debridement and removal

Biologic Versus Synthetic Mesh in Ventral Hernia Repair: Participant-Level Analysis of Two Randomized Controlled Trials at Twenty-Four to Thirty-Six Months.

Background: Biologic mesh has been used increasingly in complex ventral hernia repair despite limited evidence at low risk of bias supporting its use. Patients and Methods: We performed a participant-level analysis of published randomized controlled trials (RCTs) comparing biologic to synthetic mesh with complex ventral hernia repair at 24 to 36 months. Primary outcome was major complication (composite of mesh infection, recurrence, reoperation, or death) at 24 to 36 months post-operative. Secondary outcomes included length of index hospital stay, surgical site occurrence, surgical site infect

Mesh-associated pain syndrome: predictors for continence and prolapse mesh removal surgery in a single centre.

OBJECTIVE: Over the last two decades one of the main surgical treatment for stress urinary incontinence (SUI) and pelvic organ prolapse (POP) surgery was the insertion of non-absorbable mesh to restore continence and prolapse respectively. Over time complications arose including mesh-associated pain syndrome (MAPS), mesh exposure, mesh, erosion, chronic bladder/vaginal infections, and dyspareunia. Consequently, women chose surgical mesh removal to counter these problems. However, little is known about the demographics, medical co-morbidities, mesh types involved and the timing from mesh insert…

Effectiveness of Prophylactic Intraperitoneal Mesh Implantation for Prevention of Incisional Hernia in Patients Undergoing Open Abdominal Surgery: A Randomized Clinical Trial.

IMPORTANCE: Incisional hernia is a frequent complication after open abdominal surgery. Prophylactic mesh implantation in the onlay or sublay position requires dissection of the abdominal wall, potentially leading to wound-associated complications. OBJECTIVE: To compare the incidence of incisional hernia among patients after prophylactic intraperitoneal mesh implantation with that among patients after standard abdominal closure. DESIGN, SETTING, AND PARTICIPANTS: An open-label randomized clinical trial was performed in 169 patients undergoing elective open abdominal surgery from January 1, 20

Report of novel application of T-line hernia mesh in ventral hernia repair.

INTRODUCTION: Ventral hernia repair is one of the most common surgeries performed in the United States. Failure of hernia repairs can be attributed to sutures pulling through tissue or mesh (anchor point failure). T-Line Hernia Mesh is the first mesh designed to specifically prevent anchor point failure by distributing tension. This case study of two patients is the first clinical application of the novel T-Line Hernia Mesh. PRESENTATION OF CASE: Two separate patients presented with symptomatic ventral hernia secondary to previous laparotomy. Patient 1 is a fifty-five year-old male who underw

Outcomes of Long-Term Absorbable Mesh Use for Ventral Hernia Repair in Immunosuppressed Patients.

BACKGROUND: Immunosuppressed patients are more at risk of perioperative infections. In case of ventral hernia repair, the use of a long-term absorbable mesh could potentially decrease the infectious risk in these patients. This study aimed to assess the short- and mid-term outcomes of ventral hernia repair with long-term absorbable mesh in immunosuppressed and non-immunosuppressed patients compared with permanent mesh in non-immunosuppressed patients. METHODS: A retrospective cohort study was conducted in two tertiary centers in France. Consecutive patients who underwent ventral hernia repair

[Late vaginal mesh exposure after prolapse repair].

Mesh exposure is the major complication of vaginal prolapse repair. Incidence rates are variable according to the series. Mesh exposure usually occurs during the year following the intervention. We report here the first case of a patient with a late exposure of an anterior vaginal mesh 4 years after a surgical cystocele repair. The mesh has been easily removed at the operative theatre by vaginal approach.

Polyester-based mesh for ventral hernia repair: is it safe?

INTRODUCTION: The ideal prosthetic material for ventral hernia repair has yet to be described. Each prosthetic material has unique advantages and disadvantages in terms of tissue ingrowth, adhesion formation, and shrinkage profiles. Polyester-based mesh has shown minimal shrinkage and excellent tissue ingrowth in animal models. However, the macroporous, braided nature of this material has raised several concerns regarding the incidence of infections, fistulas, and bowel obstructions. We have reviewed our experience with polyester-based mesh for the repair of ventral hernias. METHODS: All pati

A dual-stage approach to contaminated, high-risk ventral hernia repairs.

BACKGROUND: The Modified Hernia Grading System (MHGS) was developed to risk stratify complex ventral hernia repairs (VHRs). MHGS grade 3 patients have mesh infections, dirty or contaminated fields, and/or violation of the alimentary tract. Reported surgical site infection (SSI) rates are over 40% after single-stage VHR in contaminated fields. In an attempt to decrease the SSI rate in MHGS grade 3 patients, we developed a dual-stage VHR (DSVHR) approach. METHODS: We reviewed adult general surgery patients undergoing DSVHR between January 2010 and June 2014. All patients were MHGS grade 3. Prim

Application and research progress in composite stem cell materials of pelvic floor reconstruction.

As an important tool for the surgical treatment of pelvic floor dysfunction, the safety of mesh must be guaranteed. Although the short-term curative effect of most synthetic mesh is satisfactory, complications often occur due to its material. Planting stem cells on mesh through specific methods may resolve the problems of mesh with poor biocompatibility or an uncontrollable rate o degradation. Based on recent research, this paper summarizes the research progress of stem cells composited with common mesh materials, such as polypropylene, polylactic acid and acellular matrix.

Laparoscopic transabdominal preperitoneal (TAPP) groin hernia repair using n-butyl-2-cyanoacrylate (LiquiBandFIX8) for mesh fixation and peritoneal closure: case report about extraperitoneal hematoma.

INTRODUCTION: A symptomatic inguinal hernia is a prevalent condition that typically requires surgical intervention. Various surgical approaches have been established for hernia repair, including several techniques for peritoneal closure and mesh fixation in laparoscopic surgery. N-butyl-2-cyanoacrylate, such as LiquiBandFIX8, offers a time-saving alternative to invasive methods for both mesh fixation and peritoneal closure. While n-butyl-2-cyanoacrylate is employed in various closure procedures, LiquiBandFIX8 is specifically designed for mesh fixation in inguinal hernia repair. CASE PRESENTAT
