Mid-term Efficacy of Local Repair Using Modified Altemeier Technique for Stomal Prolapse: A Case Series.

INTRODUCTION: Stomal prolapse (SP) is characterized by full-thickness protrusion of the bowel through the stoma site. The surgical procedures for SP include local repair, abdominal wall fixation, and stoma relocation. However, previous reports were mostly case reports or case series with a small number of patients and lacked long-term results. A modified Altemeier technique (MAT) has been used for the local repair of SP in our institution, and this study aimed to evaluate its mid-term efficacy. METHODS: We reviewed patients who underwent MAT for SP between August 2013 and December 2020. The v

Anastomotic leakage following restorative rectal cancer resection: treatment and impact on stoma presence 1 year after surgery-a population-based study.

PURPOSE: Anastomotic leakage (AL) continues to be a challenge after restorative rectal resection (RRR). Various treatment options of AL are available; however, their long-term outcomes are uncertain. We explored the impact of AL on the risk of stoma presence 1 year after RRR for rectal cancer and described treatment of AL after RRR including impact on the probability of receiving adjuvant chemotherapy and stoma presence following different treatment options of AL. METHODS: We included 859 patients undergoing RRR in Central Denmark Region between 2013 and 2019. Stoma presence was calculated as

The role of radiological classification of parastomal hernia as a predictor of the need for surgical hernia repair: a retrospective cohort study.

PURPOSE: Parastomal hernia (PSH) is a frequent complication of stoma creation during colorectal surgery. Radiological classification systems have been proposed for PSH but are primarily used for research. Our objective was to determine if PSH radiological classification at diagnosis could predict the need for surgical repair during follow-up. METHODS: In this retrospective cohort study, we reviewed 705 postoperative CT scans from 154 patients with permanent stoma creation from 2015 to 2018. Patients were included for analysis if a primary PSH was diagnosed on any exam. PSH were classified acc

Use of trephine stoma in sigmoid volvulus.

INTRODUCTION: Sigmoid volvulus is a disease of the elderly who often have severe comorbid conditions that increase their operative risk and limit treatment options. Conservative treatment with decompression via sigmoidoscopy with rectal tube placement has high success and recurrence rates. Surgical resection with primary anastomosis is the treatment of choice when decompression fails or if the volvulus recurs. Unfortunately, perioperative complications are frequent. Moreover, many patients with sigmoid volvulus are bedridden or incontinent of stool and do not benefit from extensive resection a

Risk Factors for Anastomotic Leakage Following Stoma Closure: A Retrospective Study in Tertiary Hospitals in Yemen.

Introduction Anastomotic leakage (AL) following stoma closure is a significant complication that can lead to increased morbidity and mortality. Identifying risk factors associated with AL is essential for improving surgical outcomes, especially in resource-limited settings like Yemen. Methods We conducted this retrospective study at Al-Thawra Modern General Hospital and the Republican Teaching Hospital Authority in Sana'a, Yemen, between August 2020 and April 2024. The analysis included 50 patients aged 18-65 years who underwent stoma closure. We analyzed patient data, including demographics,

Isoperistaltic hand-sewn side-to-side bowel primary anastomosis: a safe approach after bowel resection in children with neutropenic enterocolitis.

BACKGROUND AND AIM: Whether to perform primary anastomosis (PA) or create a stoma after bowel resection has always been a dilemma in pediatric cancer patients with neutropenic enterocolitis (NEC). The risk of leakage after PA must be weighed against the risk of stoma complications. We evaluated the outcomes of managing NEC patients with either PA or stoma and the utility of the isoperistaltic hand-sewn side-to-side anastomosis (ISSA) technique in PA. PATIENTS AND METHODS: A retrospective study on all Children's Cancer Hospital Egypt patients with NEC who underwent surgical exploration at our

Appendicostomy stomas and antegrade colonic irrigation after laparoscopic antegrade continence enema.

BACKGROUND: Children with spina bifida, high anorectal anomalies, or neuronal intestinal dysplasia who are treated with a laparoscopic antegrade continence enema to achieve a socially acceptable level of fecal continence sometimes have problems with the stoma and its catheterization. The goal of this study was to determine the nature and incidence of these problems, and their relationship to the underlying condition. MATERIALS AND METHODS: A retrospective review of the hospital case notes of 74 consecutive patients who had a laparoscopic antegrade continence enema was undertaken. RESULTS: Th

One-stage neonatal reconstruction without stoma for Hirschsprung's disease.

Interest in primary one-stage reconstruction for Hirschsprung's disease has been increasing steadily because of the obvious clinical quality issues and resource benefits. This report describes one surgeon's 13-year experience with 52 children who had proven Hirschsprung's disease. The patients were managed in a neonatal surgical unit by a one-stage neonatal reconstruction without stoma. Results indicate that the procedure is not age- or weight-dependent; it should be considered only when parameters for safe neonatal anesthesia, nursing, surgery, and expert pathology are available. Complication

A novel nonoperative technique in the initial management and treatment of congenital microstomia.

Congenital microstomia in the newborn can result in poor functional and aesthetic outcomes. In the past, treatment options have included surgical methods such as commissurotomies and z-plasties as well as nonsurgical treatments using oral splints. In severe microstomia, a surgical release may be required to permit splint placement. Because of the small diameter of the stoma, such surgical releases are not optimal and frequently must be repeated. We devised a nonoperative technique for the initial treatment of congenital microstomia that will slowly enlarge the stomal diameter without the need

Use of Meckel's diverticulum as a temporary diverting ostomy.

Meckel's diverticulum is among the most common congenital defects of the gastrointestinal tract, and is associated with a total lifetime risk of complications around 4%. While debate on prophylactic resection of incidental Meckel's diverticulum continues, there have been reports of its successful use for urinary diversion and reconstruction. In contrast, its use as a means of fecal diversion has been described anecdotally. Herein, we describe our technique of temporary fecal diversion using Meckel's diverticulum as reliable conduit for stoma formation in a toddler. The stoma functioned well un

Damage control surgery in perforated diverticulitis reduces the stoma rate: A multicentre international retrospective cohort study.

AIM: Perforated diverticulitis of the left colon with peritonitis remains life-threatening. However, the optimal surgical strategy is still under debate. The presented study compares the impact of a routinely applied two-stage damage control strategy (DCS) in perforated purulent or faeculent diverticulitis with currently used non-DCS strategies. METHODS: This international multicentre retrospective cohort study was conducted in nine European hospitals. In five hospitals, the therapeutic algorithm covered the routine application of a two-stage DCS as a surgical standard for treating perforated

Removal of an inhaled stoma button distal to a reactionary tracheal stenosis: a difficult airway case.

We present the complex and rare case of an inhaled stoma button causing proximal tracheal stenosis in a laryngectomy patient. The patient was unaware he had inhaled his button and presented with increasing shortness of breath and noisy breathing. In this case we discuss the challenging management of the stenotic tracheal segment above the impacted stoma button and the surgical approach to this difficult airway. The distal foreign body was safely removed using rigid bronchoscopy and balloon dilatation. This difficult airway required multidisciplinary input from the ENT, cardiothoracic and anaes

Management of a subcutaneous colostomy perforation. The role of a new synthetic skin.

Colostomy perforation is an infrequent but often disastrous and lethal complication. In the majority of patients, the traumatic perforation occurs during irrigation through the colostomy stoma. This case report reviews the clinical course of a patient with a subcutaneous colostomy perforation and the subsequent development of an extensive abscess. Aspects of the management included mobilization of the colostomy and thorough surgical debridement and drainage. In addition, the report introduces the use of the new semisynthetic biologic dressing, BioBrane. This synthetic, semipermeable skin subst
