Transient synovitis as a cause of painful limps in children.

Transient synovitis of the hip is one of the most common causes of hip pain and limp in young children. Its cause is still largely unknown, but its natural history is that of a self-limiting disorder with no residual sequelae, although recurrences are possible. Because this benign condition is similar to more significant disorders, such as septic arthritis, the diagnosis should remain one of exclusion. Once transient synovitis is diagnosed, treatment consists of rest, anti-inflammatory agents, and a tincture of time.

Conservative treatment of hemarthrosis for prevention of hemophilic synovitis.

Acute hemarthroses are probably the most frequent type of bleeding in the patient with hemophilia. Delayed and/or inadequate treatment can trigger a series of pathologic changes within the joint leading to a painful and disabling arthropathy. Despite the advent of prophylactic treatment with factor concentrates, the majority of patients in the world have no access to even on demand factor replacement. Care for all patients involves a team approach led by the hematologist but including input from orthopaedic surgeons and physiotherapists. Optimal treatment involves a combination of factor repla

Physiotherapy for prevention and treatment of chronic hemophilic synovitis.

Hemophilia is a hereditary lifelong bleeding disorder affecting males. The nature of the condition predisposes the person to bleed intraarticularly and intermuscularly. Without intervention with replacement factor therapy and physiotherapy, the consequences can lead to chronic synovitis and severe joint hemarthropathy. Physiotherapeutic interventions are available that may help to prevent and treat the sequelae of recurrent hemarthrosis. No particular method of treatment has been shown to eradicate hemorrhages; however, there are protocols that can help. The use of physiotherapy techniques inc

Thorn synovitis of the knee joint with Nocardia pyarthrosis.

Pyarthrosis of the knee with Nocardia asteroides occurred concomitantly with thorn synovitis in an 11-year-old boy who was otherwise healthy. A review of the orthopedic and infectious disease literatures did not disclose a similar report. Contrary to common teaching, effusions from joints violated by thorns should not be presumed sterile. Culture for bacterial and fungal infections is recommended when treating cases of thorn synovitis.

Ultrasound-guided aspiration for transient synovitis of the hip.

We reviewed 15 children with transient synovitis of the hip who had aspiration of an effusion under local anaesthesia with ultrasound guidance. There were no significant complications; aspiration was useful for diagnosis and gave considerable symptomatic relief. We discuss the role of ultrasound in diagnosis and treatment.

Transient synovitis of the hip in children: role of US.

Transient synovitis of the hip remains a common diagnostic problem for the clinician. The physical signs are not pathognomonic of the condition, and the classic technical examinations are of little help. Therefore, the authors retrospectively studied the value of hip arthrosonography in 46 children with clinical symptoms suggesting pathologic hip conditions. In 20 of the 21 patients with a final diagnosis of transient synovitis, articular effusion was detected on ultrasound (US). Conventional radiography showed an increased medial joint space in only eight of these patients. Increased echogeni

Arthroscopic synovectomy for chronic hemophilic synovitis of the knee.

Synovectomy for chronic hemophilic arthropathy is performed for the main purpose of stopping recurrent hemarthroses. The indications are frequent recurrent bleeds and persistent synovial hypertrophy. Five patients with chronic hemophilic arthropathy of the knee who underwent arthroscopic synovectomy between June 1980 and June 1981 represent the source for this discussion. Two of these patients subsequently developed a stress-induced joint bleed, one persisted in having an effusion but no recurrent bleeds, whereas the other patient developed recurrent bleeds and required a second synovectomy 4

Localized pigmented villonodular synovitis of the vertebral column.

We studied two patients with paravertebral localized pigmented villonodular synovitis. This anatomic location has not, to our knowledge, been previously described as a site for lesions of this type. In one case, a cervical paravertebral lesion first appeared clinically as a hypopharyngeal mass. The other lesion was noticed incidentally by a roentgenogram and had eroded bone in the lumbosacral area of the spine. We believed both lesions to have arisen from synovial membranes of the accessory joints of the vertebral column.

Localized nodular synovitis of the knee: a case report.

A 25-year-old man with chronic swelling of the right knee had a mass that was eventually palpated in the suprapatellar pouch. Arthrotomy and biopsy revealed localized nodular synovitis. Since surgical excision of the mass the patient has been asymptomatic. While localized nodular synovitis generally appears as an internal derangement of the joint, this patient had signs and symptoms of chronic monoarthritis.

Granulomatous synovitis and osteitis caused by Sporothrix schenckii.

Sporotrichosis must be considered in the differential diagnosis of granulomatous inflammation involving bones and joints. The organisms are difficult to demonstrate in direct smears and in histiologic sections, but they grow readily on routine fungal culture media. The cases of two patients, one with sporothrix arthritis and one with sporothrix arthritis and osteitis, are presented. The latter patient underwent ten surgical procedures over a period of 6 1/2 years and was treated for tuberculous arthritis without a definite diagnosis before fungal cultures were obtained and Sporothrix schenckii

Studies on viral arthritis (teno-synovitis) in Italy.

Three of several virus isolates made from different groups of chickens affected with arthritis were serially passaged in chicken embryos, and then cultured on chicken kidney cells with the production of cytopathic effects and development of syncytia. Resistance to inactivating agents and electron microscopic features showed their relationship to reoviruses. By the agar gel diffusion test the isolates were shown to be serologically related to each other and to 2 American strains. A tissue suspension from infected embryos when inoculated into the footpad of one to 15 day-old chicks reproduced ar

Tenosynovial Giant Cell Tumor and Pigmented Villonodular Synovitis.

Tenosynovial giant cell tumors (TGCTs) are a spectrum of benign growths that can occur in both intra-articular and extra-articular locations. The pattern of involvement also varies from nodular, typically small-volume disease to extensive and diffuse synovial infiltration. Surgical resection remains the treatment of choice for most patients and resection techniques include arthroscopic, open, or a combined approach. However, TGCT can be locally aggressive and exhibit high recurrence rates even after adequate surgical removal. Improved understanding of the complex genetic and environmental fact

Transient Hip Synovitis, 146 Cases, Origin and Duration.

Transient hip synovitis is one of the most common paediatric orthopaedic diseases. This non-controlled interventional study investigated the origin, clinical findings, imaging and the duration of symptoms. 146 affected patients out of the total of 27659 patients under 18 years result in an incidence of 0.53%. 76.7% boys outweighed 23.3% girls (1.8-12.9 years [Ø 6.3 y, boys Ø 6.5 y, girls Ø 6.2 y]). Diagnoses were defined by ultrasound and the absence of concurrent diseases. In 60.5% of patients, the right hip was affected, in 39.5% the left. A single patient had CF on both sides but not at the

Association Between Hyperuricemia and Ultrasound-Detected Hand Synovitis.

OBJECTIVE: Although hand synovitis is prevalent in the older population, the etiology remains unclear. Hyperuricemia, a modifiable metabolic disorder, may serve as an underlying mechanism of hand synovitis, but little is known about their relationship. We assessed the association between hyperuricemia and hand synovitis in a large population-based sample. METHODS: We performed a cross-sectional study in Longshan County, Hunan Province, China. Hyperuricemia was defined as a serum urate level >420 μmol/L in men and >360 μmol/L in women. Ultrasound examinations were performed on both hands of 4,
