Drill Penetration Injury to Extensor Tendons: A Biomechanical Analysis.

BACKGROUND: Little is known about extensor tendon failure following drill injury at the time of volar plate fixation. Our goals were to analyze extensor tendon injury following simulated drill penetration, and change in tendon displacement during cyclic loading following simulated drill penetration injury. METHODS: Extensor pollicis longus (EPL) and extensor carpi radialis brevis (ECRB) tendons were harvested from 9 fresh frozen cadaveric arms. Eighteen EPL and 18 ECRB samples were created from harvested tendons. Drill penetration injury was performed in either a continuous or an oscillating

Lacerations to Zones VIII and IX: It Is Not Just a Tendon Injury.

Extensor tendon injuries are widely believed to be straightforward problems that are relatively simple to manage. However, these injuries can be complex and demand a thorough understanding of anatomy to achieve the best functional outcomes. When lacerations occur in the forearm as in Zones VIII and IX injury, the repair of the extensor tendon and muscle, and posterior interosseous nerve (PIN) is often challenging. A review of the literature shows little guidance and attention for these injuries. We present four patients with injuries to Zones VIII and IX as well as a review of surgical techniq…

Peroneal tendon subluxation: the other lateral ankle injury.

Ankle injuries are a frequent cause of patient visits to the emergency department and orthopaedic and primary care offices. Although lateral ligament sprains are the most common pathologic conditions, peroneal tendon subluxations occur with a similar inversion mechanism. Multiple grades of subluxation have been described with a recent addition of intrasheath subluxation. Magnetic resonance imaging is the best imaging modality to view the peroneal tendons at the retrofibular groove. Currently, point-of-care ultrasound is gaining clinical ground, especially for the dynamic viewing capability to

Achilles tendon rupture associated with injury of the calcaneofibular ligament.

A 49-year-old man collided against an infielder when he slid into second base during a recreational baseball game. He was unable to continue in the game due to diffuse pain and swelling of his hindfoot. A rupture of the Achilles tendon was diagnosed incidentally on palpation and observation of a positive Thompson's squeeze test. Subcutaneous hemorrhage at the lateral aspect of the heel and a small bone fragment under the lateral malleolus on an anteroposterior plain radiograph indicated a fracture of the calcaneal wall. At surgery, a complete rupture of the Achilles tendon and an avulsion of t

Tendon transfers in patients with cervical spinal cord injury.

Between 1983 and 1988, 32 patients with cervical spinal cord injuries underwent 124 upper limb tendon transfers during 85 procedures. Of the numerous surgical procedures that have been recommended for treatment of this condition, my experience indicates that the most successful are posterior deltoid-to-triceps transfer, restoration of finger flexion, and restoration of thumb opposition. Longitudinal incisions prove to be quite cosmetic.

Surgical treatment of high-grade acute intramuscular hamstring tendon injuries in athletes leads to predictable return to sports and no re-injuries.

PURPOSE: The purpose of this study was to assess the post-operative return to sport and re-injury rates following surgical repair of acute, first-time, high-grade intramuscular hamstring tendon injuries in high level athletes. METHODS: Patients were identified using the databases of two sports surgeons. Once patients were identified their clinical notes and imaging were reviewed to confirm that all patients had injuries to the intramuscular portion of the distal aspect of the proximal biceps femoris tendon. All imaging was reviewed by an experienced musculoskeletal radiologist to confirm diag…

Bone-Anchored Tendon Autograft for Sutureless Digital Extensor Tendon Stabilization After Sagittal Band Injury.

Digital extensor tendon instability or subluxation occurs in the setting of a sagittal band injury, which allows the extensor to subluxate off the metacarpal head upon flexion at the metacarpophalangeal joint. This can cause painful snapping of the tendon and limit the initiation of active extension. Surgical tendon stabilization is appropriate for patients who fail initial nonoperative splint immobilization after acute injury or those with chronic tendon subluxation. Existing surgical techniques are limited by bulky subcutaneous repairs, reliance upon compromised tissues, and/or the need for

Multiple extensor tendons reconstruction with hamstring tendon grafts and flap coverage for severe dorsal hand injuries.

UNLABELLED: Treatment of patients with traumatic loss of skin and multiple extensor tendons on the dorsum of the hand is a challenge. The aim of this study was to assess the outcome after reconstruction of soft tissues and multiple extensor tendons in patients who suffered traumatic loss of skin and multiple extensor tendons. Ten patients were enrolled in the study. These patients underwent single-stage reconstruction with autogenous hamstring tendon grafts for multiple extensor tendon defects and fasciocutaneous flaps for coverage of dorsal hand defects. In total, 25 tendons (2 tendons in 5 p

Extensor tendon injury due to repetitive wrist dorsiflexion: morphological study of extensor retinaculum and extensor tendon.

Most etiological studies of extensor tendon injury were based on the normal anatomy of extensor tendon and extensor retinaculum of the wrist. Further understanding of the morphological changes of the extensor tendon and extensor retinaculum during wrist dorsiflexion might contribute to improved and more accurate understanding of the etiology. The morphology of the extensor tendon of the mid-finger and the fourth compartment of the wrist extensor retinaculum was studied by sonography, and the anatomy was studied in 15 extremities from 11 young male cadavers. Compared with anatomical images, ult

Macrophages in rotator cuff injuries and tendon-to-bone healing: Heterogeneity, pathophysiology, and therapeutic strategies.

Rotator cuff tears (RCTs) are a leading cause of shoulder disability, with high postoperative failure rates primarily attributed to poor tendon-to-bone healing. Macrophages act as central regulators of the inflammatory and regenerative microenvironment in the injured enthesis, tendon stump, subacromial bursa, and rotator cuff musculature. This review highlights the spatiotemporal heterogeneity and phenotypic plasticity of macrophages revealed by single-cell and spatial transcriptomics. Rather than following a simple M1-to-M2 transition, macrophages adopt context-dependent inflammatory, resolvi

Self-Inflicted Wrist Injuries in Flexor Tendon Zone V: Clinical Patterns, Management Strategies, and Outcomes.

Self-inflicted wrist injuries, particularly those involving flexor tendon zone V, constitute a distinct and clinically challenging subset of upper-extremity trauma with profound surgical, functional, and psychosocial implications. For every suicide, many involve wrist lacerations capable of compromising tendinous, neural, and vascular structures and resulting in long-term disability. Despite their clinical relevance, the literature remains fragmented, with no universally accepted definition of "spaghetti wrist," a term originally described in early surgical series of extensive volar wrist lace

The diagnostic performance of MRI signs to distinguish Pectoralis major tendon avulsions from Myotendinous injuries.

BACKGROUND: Management of pectoralis major (PM) injuries is largely determined by the anatomic location of the injury, with tendon avulsions from the humerus requiring surgery while myotendinous (MT) injuries are typically managed non-operatively. Because physical examination cannot reliably make this distinction, MRI is often used for staging. However, correct classification can also be difficult with MRI where there is extensive soft tissue edema and distorted anatomy. OBJECTIVE: To determine the diagnostic performance of primary and secondary MRI signs of PM injury for distinguishing tendo
