Thoracic outlet syndrome after corrective surgery for pectus excavatum.

Two patients are described who developed a thoracic outlet syndrome (TOS) after undergoing Ravitch's operation for the correction of pectus excavatum. In one case the syndrome developed a few days after surgery, whereas in the second patient it manifested more gradually. A third patient presented with latent TOS and pectus excavatum. It is recommended that prior to the correction of pectus excavatum, the patient should be examined to detect signs of neurovascular compression due to latent thoracic outlet syndrome. After surgery the possibility of this complication should be kept in mind to avo

Classics in thoracic surgery: Total pneumonectomy. Rudolf Nissen, M.D.

In July, 1931, a 12-year-old girl sustained a crushing injury to the thorax. Tension pneumothorax, which suggested a tear in the left main bronchus, developed. Because of chronic pulmonary suppuration over the next several months, the decision was made to extirpate the left lung. Total pneumonectomy was attempted, but during the operation strong traction on the hilum resulted in temprary asystole. The operation was halted and resumed fourteen days later. The lung was freed and suture ligatures tied around the hilum, central to the bronchial obstruction. Two weeks later the necrotic lung slough

Thoracic Ultrasound as an Alternative to Chest X-ray in Thoracic Surgery Patients: A Single-Center Experience.

Background/Objectives: Chest X-ray (CXR) is currently the most used investigation for clinical follow-up after major noncardiac thoracic surgery. This study explores the use of lung ultrasound (LUS) as an alternative to CXR in the postoperative management of patients who undergo major thoracic procedures. Methods: The patients in our cohort were monitored with both a CXR and a lung ultrasonography after surgery and the day after chest drain removal. The LUS was performed by a member of the medical staff of our unit who was blinded to both the images and the radiologist's report of the CXR. Fin…

Mobilization of the phrenic nerve in the thoracic cavity by video-assisted thoracic surgery. Techniques and initial experience.

BACKGROUND: The use of video-assisted thoracic surgery (VATS) techniques to mobilize the phrenic nerve in the thoracic cavity for neurotization after brachial plexus injury was studied. METHODS: From August 1999 to January 2000, 10 men and 1 woman with brachial plexus injury (left side in 5 and right side in 6) joined the study group. Their ages ranged from 20 to 38 years (average, 28 years). Supine after general anesthesia, all the patients had double-lumen trachea cannulas to guarantee complete lung collapse on the operative side. Three port incisions were made to allow introduction of the …

Nonintubated Robotic-Assisted Thoracic Surgery for Tracheoesophageal Fistula Repair.

We report the application of nonintubated spontaneous ventilation anesthesia (tubeless anesthesia) combined with robotic-assisted thoracic surgery for tracheoesophageal fistula repair. The use of tubeless anesthesia optimized the surgical workflow by eliminating endotracheal intubation interference, thereby improving surgical efficiency and minimizing iatrogenic trauma. robotic-assisted thoracic surgery has distinct advantages in confined spaces (eg, the cervicothoracic junction), such as enhanced visual acuity, superior precision, and greater operational flexibility, thereby effectively mitig…

Infectious morbi-mortality in thoracic surgery after major resections.

BACKGROUND: Eight thousand major pulmonary resections are performed annually in France for bronchopulmonary cancer. This surgery is associated with a significant mortality rate, estimated at 3% within 30 days, mainly due to infectious pulmonary complications. In recent series, early mortality reaches 12.6% in cases of postoperative pneumonia (POP). Our case-control study aims to describe the incidence, mortality and risk factors of infectious complications in the Department of Thoracic, Cardiac, and Vascular Surgery, Tours University Hospital, France. METHODS: We identified 404 major resectio

Biochemical markers in emergency thoracic surgery in penetrating trauma.

BACKGROUND: Violent interpersonal acts account for a large proportion of unnatural deaths in South Africa. A significant proportion of unnatural deaths are due to penetrating thoracic trauma and preventable haemorrhage. Current indications for emergent thoracotomy are unreliable. We propose the use of lactate, shock index (SI) and base deficit (BD) as a triage tool in patients with penetrating thoracic injuries to identify those requiring surgical intervention. METHODS: A review of the trauma registry of the Charlotte Maxeke Johannesburg Academic Hospital (CMJAH) was carried out between March

Damage control thoracic surgery: What you need to know.

Damage control surgery in trauma prioritizes patient stabilization through an initial temporizing surgical approach to rapidly control hemorrhage and contamination, minimizing intraoperative time to allow for resuscitation and the correction of hypothermia, coagulopathy, and acidosis in the intensive care unit. This is followed by definitive repair of injuries once physiological parameters have improved. While damage control techniques for traumatic intra-abdominal and extremity injuries are well established and frequently utilized, the same cannot be said for damage control thoracic surgery. …

Console and bedside surgeon fused robot-assisted thoracic surgery.

In the last decade, even thoracic surgery has seen an increase in the use of robotic surgical systems, and robot-assisted thoracic surgery (RATS) is considered one of the main issues. While RATS is associated with solo manipulative freedom and high-definition optical systems, several disadvantages, such as the lack of tactile sensation and difficult learning curves for the whole team, have been raised. Therefore, to overcome these issues, we developed a 'fusion surgery' approach combining a robotic procedure with manual maneuvers, where the table surgeon retracts the lung and staples the pulmo…

Thoracic surgery in the non-intubated spontaneously breathing patient.

BACKGROUND: The interest in non-intubated video-assisted thoracic surgery (NIVATS) has risen over the last decade and numerous terms have been used to describe this technique. They all have in common, that the surgical procedure is performed in a spontaneously breathing patient under locoregional anaesthesia in combination with intravenous sedation but have also been performed on awake patients without sedation. Evidence has been generated favouring NIVATS compared to one-lung-ventilation under general anaesthesia. MAIN BODY: We want to give an overview of how NIVATS is performed, and which d…

[Robot-assisted thoracic surgery-Areas of application and limitations].

The proportion of robot-assisted thoracic surgery (RATS) is continuously increasing. The main areas of clinical application are anatomical lung resections with lymphadenectomy and resection of mediastinal tumors. Especially in the area of the thymus robot-assisted procedures can now be considered the gold standard. Overall, the limitations of video-assisted thoracoscopic surgery (VATS) can be overcome by the use of robot-assisted surgery and thus a larger proportion of patients can be treated with minimally invasive procedures. The safety of the RATS procedure has been repeatedly demonstrated,…

Impact of Thoracic Radiation on Patients Undergoing Cardiac Surgery.

Prior thoracic radiation has been associated with worse outcomes after cardiac surgery. This study sought to report long-term outcomes in patients undergoing surgery for radiation-associated heart disease. This was an observational study of open cardiac surgeries from 2011 and 2018. Patients with a history of malignancy that required thoracic radiation were identified, and this cohort was matched against a non-irradiated comparison group via Mahalanobis distance matching. Kaplan-Meier survival estimation and multivariable Cox regression analysis was performed to assess the long-term impact of
