BACKGROUND: Since 1913 surgical myotomy has been an effective method for patients with esophageal achalasia and until today there is controversy between pneumatic dilatation or surgery and between the abdominal or thoracic approach for Heller's procedure.
AIMS: To find out the long-term results with thoracic short esophagomyotomy without an antireflux procedure for esophageal achalasia.

Hydatic cysts of the liver can rupture into the thorax. The aim of this work was to demonstrate how abdominal access can be used in most cases. We report 44 cases of hydatic cysts which ruptured into the thorax among a series of 1411 hydatic cysts of the liver operated between 1974 and 1995. The behavioural responses of Onchocerca lienalis microfilariae (mf) to tissue factors of the surrogate black fly host, Simulium vittatum, were studied using a novel in vitro bioassay. Mf accumulated towards thoracic tissues to a density 4 times higher than towards abdominal tissues, despite the larger surface area and volume of abdominal tissues. The author proposes an original method of determination of the degree of deformation in angular values on the basis of his experience in the treatment of 27 patients with infundibulum-like deformation of the chest. In case of surgical treatment the method of thoracoplasty after N. I. Kondrashin is preferable. The design formula of the size of the resected rib area is proposed. To stabilize the framework of the chest fixation by crossing wires on the ring support after G. A. Ilizarov is suggested. The role of fine needle aspiration (FNA) biopsy in the evaluation of lymphoreticular disorders involving the lung and mediastinum was evaluated in a study of 16 cases from the authors' institutions. The cytologic material from these cases was reviewed, and the original cytologic interpretations were compared to the tissue diagnoses, with the overall accuracy of the cytologic interpretation evaluated. BACKGROUND: Intralobar pulmonary sequestration (ILS) is defined as a portion of parenchyma that is contained within the normal pleural investment of the lung but not connected to the tracheobronchial tree, and supplied by anomalous systemic arteries. As ILS is enveloped within the lobe of the normal lung, it is extremely rare for ILS to invade into the mediastinum.
