BACKGROUND: pylephlebitis is a rare complication of intraabdominal infections. Clinical presentation can be varied; therefore, it is necessary to carry out imaging tests for diagnosis. The treatment of choice is wide-spectrum antibiotic therapy and surgery for the source of infection.
CLINICAL CASE: we report the case of a female with acute abdomen who was diagnosed with acute cholecystitis and right portal vein thrombosis. The patient underwent surgery and antibiotic treatment was necessary. CT scan showed hepatic infarction secondary to portal thrombosis.
CONCLUSIONS: pylephlebitis present

INTRODUCTION: Thrombophlebitis migrans is characterised by the development of recurrent (i.e. migratory) superficial thrombophlebitis. It is an acquired coagulopathy that is strongly associated with malignancy, especially solid tumours of the adenocarcinoma type.

In 1955, the first reported case of Mondor's disease (superficial thrombophlebitis) of the penis was published. Since then, there have been scattered reports of penile Mondor's disease in the literature. Most studies suggest sexual trauma or neoplasm as the most frequent etiologic factor. The authors report a case of a sickle cell episode presenting with superficial thrombophlebitis of the penis. The patient was treated with an antiinflammatory agent and reassured that this is a self-limiting process. Resolution of symptoms occurred within 6 weeks.

The authors report the clinical case of a young woman with thrombophlebitis of the right ovarian vein following delivery by caesarean section, initially presenting in the form of renal colic. In the light of a review of the literature, they recall the pathophysiological mechanisms of ovarian thrombophlebitis and the various features observed on imaging examinations. The most frequent clinical features are also described. The authors emphasize the potential, but rare severity of this disease, characterized by the risk of pulmonary embolism, and its treatment, which is usually medical.

A prospective study with 161 patients was performed to investigate the effects of intravenous local anaesthetics on the pain of injection following etomidate injection. After placebo injection 56.6% of the patients reported on pain after etomidate, but only 29% after 20 mg lidocaine i.v. Catheterization of subclavian veins was performed in 2800 patients including 18 children in order to fulfil infusion-transfusion massive antibacterial therapy. The article gives a detailed clinical picture of the complication and recommendations for the treatment of this dangerous complication. The outcomes of the obturating thrombosis with catheterized subclavian veins are described. The employed active conservative therapy contributed to recovery of all the patients having this complication.
