Total urogenital sinus mobilization: expanded applications.

OBJECTIVE: To report further applications of total urogenital sinus mobilization, earlier described as an easier method to correct a cloaca. PATIENTS AND METHODS: Seven children (six girls and one boy, mean age 4 years, range 3 months to 10.5 years) underwent surgery and were followed for a mean of 1 year; their diagnoses included persistent cloaca and congenital adrenal hyperplasia (CAH) in two each, and a urogenital sinus (UGS), bladder exstrophy and penile agenesis in one each. The UGS is approached through a posterior sagittal incision and dissected circumferentially to the retropubic spa

An approach to daytime wetting in children.

Daytime wetting is a common problem with various causes that can usually be identified through a careful history, thorough physical examination, and urinalysis. Conservative approaches to therapy have a successful outcome in most children. Invasive diagnostic imaging studies and pharmacologic or surgical intervention are necessary only for carefully selected children.

[Recommendations on the diagnosis and treatment of urinary tract infection].

Urinary tract infection (UTI) is defined as the growth of microorganisms in a sterile urine culture in a patient with compatible clinical symptoms. The presence of bacteria without any symptoms is known as asymptomatic bacteriuria, and does not require any treatment. In neonates and infants, fever is the guiding sign to suspecting a UTI. Classic urinary tract symptoms become more important in older children. Urine cultures collected before starting antibiotics is always required for diagnosis. Clean-catch (midstream) specimens should be collected for urine culture. In the case of non-toilet-tr

Solar powered nanofiltration for drinking water production from fluoride-containing groundwater - A pilot study towards developing a sustainable and low-cost treatment plant.

The following paper summarizes the findings of a pilot study to develop a simple, low-cost, holistic water concept on fluoride removal from groundwater in rural communities of Tanzania; an ideal representative community for other areas in the world with similar problems. A small photovoltaic powered nanofiltration (NF) pilot plant was installed at a vocational training center in Boma Ng´ombe in northern Tanzania. The groundwater in this region is contaminated with fluoride at very high concentrations of up to 60 mg/L. The pilot plant was equipped with a single membrane module containing a spir

Urinary tract infections in infants and children: Diagnosis and management.

Recent studies have resulted in major changes in the management of urinary tract infections (UTIs) in children. The present statement focuses on the diagnosis and management of infants and children >2 months of age with an acute UTI and no known underlying urinary tract pathology or risk factors for a neurogenic bladder. UTI should be ruled out in preverbal children with unexplained fever and in older children with symptoms suggestive of UTI (dysuria, urinary frequency, hematuria, abdominal pain, back pain or new daytime incontinence). A midstream urine sample should be collected for urinalysi

Tibial nerve stimulation compared with sham to reduce incontinence in care home residents: ELECTRIC RCT.

BACKGROUND: Urinary incontinence is prevalent in nursing and residential care homes, and has a profound impact on residents' dignity and quality of life. Treatment options are limited in these care contexts and care homes predominantly use absorbent pads to contain incontinence, rather than actively treat it. Transcutaneous posterior tibial nerve stimulation is a non-invasive, safe, low-cost intervention that is effective in reducing urinary incontinence in adults. OBJECTIVE: To determine the clinical effectiveness of transcutaneous posterior tibial nerve stimulation to treat urinary incontin

Awe and premature efaculation: a case study.

The analysis of a man suffering from premature ejaculation is described. As is typical of a large group of men who ejaculate prematurely, he was passive and masochistic in his marriage and obsessive-compulsive in his character. Analysis of his affect of awe led first to a deeper understnding of his self-representations and object relationships, and later, with the discovery of the specific unconscious fantasies associated with the feeling of awe, to the alleviation of his symptom of premature ejaculation.

Depth-augmented diffusion policy with pseudo-depth for robust robotic manipulation.

Diffusion Policy has emerged as a powerful approach for imitation learning in robotic manipulation. However, policies trained solely on RGB observations often fail to capture the spatial structure of a scene, which limits performance on tasks requiring precise distance-aware motions, such as approaching an object along the camera axis. A natural remedy is to provide depth as an additional input, but two practical challenges remain: designing a conditioning mechanism that encourages the policy to use depth effectively and obtaining reliable depth when hardware measurements are unavailable or no

Management principles of organic causes of childhood constipation.

Constipation is extremely common in the pediatric population; however, it is a symptom and not a disease. Organic causes of constipation include Hirschprung's disease, anorectal malformation, and spina bifida. This article also discusses constipation and urinary tract pathology and dysfunction. The function of the gastrointestinal and urinary tracts are intertwined, and this concept will assist the practitioner in managing patients accordingly. Children and adolescents who experience bowel dysfunction require patience and sensitive support from their health care providers so that they can live

Improving Hygiene Practices to Prevent Maternal and Neonatal Infections in Healthcare and Communities in Benin: Findings and Policy Recommendations.

Maternal and neonatal health remains a major challenge in sub-Saharan Africa, with high mortality rates also observed in Benin. This policy brief summarizes findings from two hygiene and infection prevention and control (IPC) projects implemented in Benin between 2023 and 2025 and translates them into prioritized policy recommendations. Findings are drawn from a baseline survey of 169 healthcare workers across six hospitals, focus group discussions with 42 community health workers (CHW), structured interviews and stakeholder consultations, and outcomes of two structured training programs. Thro

Despite its relevance as a causative … s human-to-human transmission, mainly in children of all age groups when they have not yet received toilet training, making them more vulnerable to infection.

Pelvic floor electromyography and urine flow patterns in children with vesicoureteral reflux and lower urinary tract symptoms.

OBJECTIVE: To determine the different urine flow patterns and active pelvic floor electromyography (EMG) during voiding in children with vesicoureteral reflux (VUR) as well as presenting the prevalence of lower urinary tract symptoms in these patients. MATERIALS AND METHODS: We retrospectively reviewed the charts of children diagnosed with VUR after toilet training from Sep 2013 to Jan 2016. 225 anatomically and neurologically normal children were included. The reflux was diagnosed with voiding cystourethrography. The study was comprised an interview by means of a symptom questionnaire, a voi…

The effectiveness of transcutaneous nerve stimulation in treating children with functional constipation.

BACKGROUND AND STUDY AIMS: Functional constipation (FC) is a prevalent condition in paediatric populations and while standard therapies, including laxatives and toilet training are commonly used, a substantial proportion of children exhibit an inadequate response. This study aimed to evaluate the effectiveness of transcutaneous nerve stimulation (TENS) as a non-invasive treatment for FC in children. PATIENTS AND METHODS: Children diagnosed with FC according to the Rome IV criteria and treated with TENS were included in this retrospective cohort study. Data were collected in two Belgian hospit…

A Case Study on Community Treatment Center for Response to COVID-19 in Korea: Focusing on Surge Capacity 4S Component Analysis and Social Well-Being Perspective.

In March, 2020, during the COVID-19 pandemic in Korea, the first Community Treatment Center (CTC), which is a motel-type Alternate Care Site (ACS) for mild and asymptomatic patients, was opened. This is a case study of the first Community treatment center prepared to respond to COVID-19. One of the researchers worked as a medical doctor in one of the CTCs operated by the Korean government. The CTC's eight medical staff members were interviewed in-depth one-on-one. Then the data obtained from observation, collection, and interview were triangulated. In this study, it was identified based on the
