Histologic evaluation of root response to intrusion in mandibular teeth in beagle dogs.

INTRODUCTION: The purpose of this article was to histologically evaluate root resorption and repair after orthodontic intrusion with different force magnitudes and fixed anchorage. METHODS: A randomized split-mouth repeated-measure design was used. Intrusive forces were applied for 98 days to the mandibular second, third, and fourth premolars of 8 mature beagle dogs. Two miniscrew implants were used as anchorage to apply constant intrusive forces of 50, 100, or 200 g per tooth. Demineralized sections of each tooth were stained and histologically studied for root resorption. Multilevel statist

Management of cervical external root resorption following connective tissue grafting.

BACKGROUND: Cervical external root resorption (CERR) is not a common occurrence, but can result in a hopeless tooth prognosis. Its etiology is poorly understood and its management can be challenging. The present case report describes the late presentation and management of CERR at both maxillary first premolar teeth following connective tissue grafting (CTG) procedures including use of citric acid as a chemical root surface conditioner. METHODS AND RESULTS: A 55-year-old female was diagnosed with bilateral external cervical root resorption of both maxillary first premolar teeth 28 years after

Effects of cervical restorations on the periodontal tissues: 5-year follow-up results of a randomized clinical trial.

OBJECTIVE: This study evaluated the effect of resin composite restorations of non-carious cervical lesions (NCCLs) on the occurrence/progression of gingival recession (GR), considering different tooth isolation techniques. METHODS: A randomized controlled split-mouth and blinded trial was carried out. Patients (n = 38) with at least two NCCLs were included. Before the cervical restoration placement, the NCCLs (181 teeth) were randomly allocated into two treatment groups according to the tooth isolation techniques: cotton roll or rubber dam. Experienced, trained, blinded, and calibrated examin

Dens invaginatus and talon cusp co-occurring in a mandibular central incisor: a case report.

Talon cusp and dens invaginatus are tooth shape anomalies that rarely affect mandibular teeth. Their co-occurrence in a single tooth is uncommon. The purpose of this report was to present a rare case of both talon cusp and dens invaginatus in a permanent mandibular central incisor. The diagnosis of dens invaginatus was made from routine radiographs taken to investigate a possible pulpal extension within the taloned cusp. Since the invagination did not communicate with the oral cavity, no treatment was indicated. The talon cusp was slightly grinded and smoothened to eliminate tongue irritation.

Multiple teeth showing invasive cervical resorption--an entity with little known histologic features.

Invasive cervical resorption is a relatively uncommon form of external root resorption, characterized by resorption of the cervical region of the root. There is progressive loss of cementum and dentine with replacement by fibrovascular tissue derived from the periodontal ligament, with deposition of cementum-like hard tissue. In most cases, a single tooth is involved. We report a case of invasive cervical resorption that resulted in loss of multiple teeth from two different quadrants. The case highlights the diagnostic difficulty that may arise in this uncommon lesion, the pathologic features

Immunodetection of enamel- and cementum-related (bone) proteins at the enamel-free area and cervical portion of the tooth in rat molars.

Enamel and dentin at the cervical portion of the tooth are frequently covered by a collagen-free matrix referred to as acellular afibrillar cementum (AAC). It is believed that AAC deposition occurs when the enamel organ is displaced or disrupted, and mesenchymal cells from the dental follicle gain access to the tooth surface, differentiate into cementoblasts, and secrete noncollagenous proteins typically found in collagen-based mineralized tissues. A similar thin layer of mineralized matrix is found at the enamel-free area (EFA) of rodent molars, but in this case the matrix is covered by inner

Association between 3D characteristics of single unilaterally impacted mesiodens and root development of adjacent maxillary central incisors: a retrospective study with cone-beam computed tomography.

OBJECTIVES: This study aimed to evaluate the detrimental effects of single unilaterally impacted mesiodens on the root development of adjacent maxillary central incisors and to explore the key influencing factors. METHODS: A total of 552 patients aged 5-12 years with single unilaterally impacted mesiodens were included in the study. The maxillary central incisor with greater overlap with the mesiodens in the coronal view was identified as the affected tooth, while the contralateral central incisor served as the control one. Cone-beam computed tomography (CBCT) data for these patients were ret

Pulp size in molars: underestimation on radiographs.

The aim was to determine whether radiographs provide a clinically useful indication of pulp size in diseased/restored human first molar teeth, and to investigate accessibility of pulp tissue for diagnostic testing using laser Doppler flowmetry (LDF). Extracted teeth of known age were collected. Restorative materials were removed and teeth with evidence of pulp exposures excluded. Fifty-six teeth were radiographed from buccal and mesial aspects, and then their crowns were sectioned axiobuccolingually and photographed. Images were digitally scanned and measurements made of the total pulp area (a

Endodontic pathogens: propagation of infection through patent dentinal tubules in traumatized monkey teeth.

Periapical pathology indicating endodontic infection, when present in marginal periodontitis-affected teeth, has recently been shown to be an aggravating factor in progression of marginal destruction. This has been associated with patency of dentinal tubules in the tooth cervix, an area normally devoid of cementum following periodontal therapy. These studies are, however hampered by that only circumstantial evidence such as presence of periapical destruction have been applied as criteria of endodontic infection. The purpose of the present investigation was to study to what extent a predefined

External Cervical Resorption Treatment: Case Report and Outcome-based Strategies.

External cervical resorption (ECR) is a complex dental condition that is traditionally treated with external surgical repair and nonsurgical root canal therapy as needed. This case report describes a patient with 8 teeth treated for ECR. Follow-up examination and management strategies are described, using microscopic techniques, three-dimensional imaging, and bioactive root filling materials. The chosen techniques were based on the severity of ECR, root location, defect geometry, and management of the portals of entry. Access to the external resorption defect was achieved through the root cana

Automatic methods for alveolar bone loss degree measurement in periodontitis periapical radiographs.

BACKGROUND AND OBJECTIVE: Periodontitis involves progressive loss of alveolar bone around the teeth. Hence, automatic alveolar bone loss measurement in periapical radiographs can assist dentists in diagnosing such disease. In this paper, we propose an automatic length-based alveolar bone loss measurement system with emphasis on a cementoenamel junction (CEJ) localization method: CEJ_LG. METHOD: The bone loss measurement system first adopts the methods TSLS and ABLifBm, which we presented previously, to extract teeth contours and bone loss areas from periodontitis radiograph images. It then ap

Dental Management of a Patient with Multiple Idiopathic Cervical Root Resorption.

Multiple Idiopathic Cervical Root Resorption (MICRR) is a rare condition. It initiates at the cemento-enamel junction of multiple teeth. The lesions continue to grow until they unite, thereby undermining the entire coronal structure of affected teeth. Its distribution can vary from a single region to the entire dentition and the number of teeth affected by resorption tends to increase as the condition is followed over time. The teeth themselves appear clinically normal. The aetiology of MICRR is unknown and it is considered to be a diagnosis of exclusion. The condition tends to be progressive.

Interdisciplinary treatment of cervical lesions.

Soft tissue grafting is an integral part of treatment of cervical lesions due to the common lack of adequate attached gingiva and root exposure associated with these lesions. Complete root coverage is a predictable outcome for Miller Class I and II recession defects, and partial root coverage can be achieved in Miller Class III defects. In the esthetic zone, it is desirable to cover as much of the root as possible, and all sites require an adequate zone of attached gingiva, especially adjacent to a restoration. Restorations are required for cervical lesions with excessive depth and significant

Apical root resorption of upper first molars as related to anchorage system.

Night-time use of extra-oral traction for anchorage may cause jiggling, rotational and extrusional forces. The purpose of the study was to test the hypothesis that headgear forces in an anchorage system may increase the risk of radiographically detectable root resorptions on molar teeth. Twenty-one patients were selected among patients planned for orthodontic treatment, in which there was a need of anchorage in a full-bond appliance during a period of at least 6 months. An experimental group of 11 patients was given reinforcement anchorage in the maxilla with an extra-oral traction (cervical-p

Root surface characteristics of primary teeth from children with prepubertal periodontitis.

This study describes the histologic characteristics of root surfaces of primary teeth from children with prepubertal periodontitis (PP). Fifteen primary teeth from 4 children with PP, and 2 control primary teeth from 2 healthy children were examined. Light microscopy revealed normal root surfaces in the control teeth. In contrast, the PP specimens revealed bacteria inside dentin tubules or covering cementum, a cuticle, or resorbed dentin; normal, wider than normal, or hypoplastic cementum; resorption lacunae with various depths; aplastic root resorption; alternate resorption and repair; and ac

Management of bilateral invasive cervical resorption lesions in maxillary incisors using a novel calcium silicate-based cement: A case report.

Invasive cervical resorption is a pathologic process leading to progressive and usually destructive loss of tooth structure. The pathogenic mechanism is not completely understood and the diagnosis may be challenging. The aim of this article is to present an unusual case of bilateral presence of invasive cervical resorption lesions in maxillary central incisors and to discuss the treatment procedures using a novel repair material. The management of the present case was carried out in three phases. The first stage of the treatment aimed at curetting the active tissue from the resorption cavity a

Is there a relation between local bone quality as assessed on panoramic radiographs and alveolar bone level?

The aim was to explore the relation between radiographic bone quality on panoramic radiographs and relative alveolar bone level. Digital panoramic radiographs of 94 female patients were analysed (mean age, 44.5; range, 35-74). Radiographic density of the alveolar bone in the premolar region was determined using Agfa Musica software. Alveolar bone level and bone quality index (BQI) were also assessed. Relationships between bone density and BQI on one hand and the relative loss of alveolar bone level on the other were assessed. Mandibular bone density and loss of alveolar bone level were weakly

Periodontal surgery and glass ionomer restoration in the treatment of gingival recession associated with a non-carious cervical lesion: report of three cases.

BACKGROUND: Buccal gingival recession is a prevalent problem in populations with a high standard of oral hygiene and is very often associated with a non-carious cervical lesion, complicating treatment. The purpose of this report is to show three cases treated by an integrated periodontal and restorative dentistry approach. METHODS: Three patients with Miller Class I gingival recessions associated with non-carious cervical lesions were enrolled for treatment. One patient received a coronally positioned flap and a resin-modified glass ionomer restoration, and two patients were treated with a co

Connective tissue grafting employing the tunnel technique: a case report of complete root coverage in the anterior maxilla.

Techniques for surgical root coverage have been continuously revised over the past few decades. With increased knowledge on the etiopathogenesis of gingival recessions and on the repair/regeneration mechanisms of deep and superficial periodontal tissues, procedure simplification has been possible, and more predictable and stable results have been obtained. The maintenance of maximal blood supply has brought major changes in flap design. The coverage of contiguous recessions on the maxillary central incisors using a conservative technique for the incision of the recipient site is presented, alo
