Halo gravity traction (HGT) is a preoperative modality for children with severe spinal deformity used to optimize spine flexibility and balance while decreasing the likelihood of neurologic injury. HGT is a dependable solution for these challenging spinal deformities. Aligning treatment guidelines and providing resources, education, and training for staff are key components for a successful HGT program. The HGT program at Children's Mercy Hospital was then developed with a systems approach with the holistic goal of multidisciplinary collaboration with physical/occupational therapy, hospital me

BACKGROUND: The choroidal vasculature supplies the outer retina and is altered in many retinal diseases, including myopic traction maculopathy (MTM). Choroid health is typically assessed by measuring the choroidal thickness; however, this method has substantial limitations. The choroidal vascularity index (CVI) was recently introduced to provide quantitative information on the vascular flow in the choroid. This index has been evaluated in a wide range of diseases but has not been extensively used to characterize MTM.
AIM: This study aimed to investigate the CVI across different stages of MTM

BACKGROUND: Endoscopic submucosal dissection (ESD) is recognized as a minimally invasive and curative treatment for superficial gastrointestinal (GI) cancers. However, ESD is still challenging and time-consuming with a high risk of adverse events such as bleeding and perforation. Various traction methods have been explored for maintaining good visualization of the submucosal layer during ESD. We developed a novel traction device (the EndoTrac) which can easily tie the thread and has the ability to change the towing direction. The aim of this study is to evaluate safety and feasibility of ESD u

Syphilis is often referred to as "the great masquerader," as it may present with a wide array of clinical symptoms and may mimic a variety of other diseases making diagnosis challenging. We report the case of a young, married woman who presented with a one-month history of significant hair loss, unintentional weight loss, blurred vision, and intermittent headaches. In addition, she endorsed positional dizziness and intermittent arthralgias. Physical exam was only remarkable for a non-scarring alopecia over the frontal marginal hairline and optic disc edema on fundoscopic exam. Laboratory tests

Orthodontic traction of impacted canines stands as a major challenge for Orthodontics. It is a relatively frequent clinical complaint which management, more often than not, requires a multidisciplinary approach. Surgical exposure of the impacted canine, and the complex orthodontic mechanics applied to align the tooth back into the arch, may frequently lead to complications involving supporting tissues, not to mention the long treatment time and high costs imposed to patients. In face of that, it is worth highlighting the relevance of early diagnosis as to intervene efficiently and as soon as p

Since its introduction by Charles L. Schepens, macular buckle (MB) surgery has evolved over the past 60 years. Optical coherence tomography (OCT) has given a paradigm shift to the understanding of myopic macula, thereby helping in objective evaluation of the various manifestation of traction maculopathy. Staphyloma evaluation by ultrasound, wide-field fundus photography, and MRI scans along with OCT has led to the resurgence of MB surgery in the treatment of myopic traction maculopathy (MTM). Various surgical techniques with different buckle materials are being performed with encouraging anato

Traction pins are an essential tool in the orthopedic surgeon's armamentarium. Historically a definitive treatment for some fractures, they are mainly used as a temporizing measure today. Despite their frequent use and relative simplicity, traction pins can have complications, many of which can be subtle and easily overlooked. Here we report on an unusual complication that was difficult to diagnose but caused significant morbidity before being diagnosed and treated. Pseudoaneurysms can cause a range of symptoms and usually present as a painful, tender, pulsatile mass, but in this instance the

An articulating tactical traction splint (REEL™ Tactical Traction Splint), commonly issued to U.S. military personnel, was used to maintain traction in a pulseless forearm fracture during a confined space rescue, with good peripheral perfusion maintained through transport. This enabled a single rescuer to focus attention and provide care for other critical aspects of a multisystem trauma patient.

BACKGROUND: Femoral fracture is a common battlefield injury with grave complications if not properly treated. Traction splinting has been proved to decrease morbidity and mortality in battlefield femur fractures. However, little standardization of equipment and training exists within the United States Armed Forces. Currently, four traction splints that have been awarded NATO Stock Numbers are in use: the CT-6 Leg Splint, the Kendrick Traction Device (KTD), the REEL Splint (RS), and the Slishman Traction Splint (STS).
OBJECTIVE: The purpose of this study was to determine the differences betwee

The authors report four cases of vitreomacular traction syndrome with splitting of the posterior vitreous cortex. Spectral-domain optical coherence tomography (SD-OCT) was performed consecutively in 35 patients with vitreomacular traction syndrome between June 2007 and February 2011. SD-OCT showed perifoveal posterior vitreous detachment with splitting of the posterior vitreous cortex in 4 of 35 eyes (at the fovea in 1 eye and the perifovea in 3 eyes). The separated cortex of the posterior lamella adhered to the retina in all 4 eyes. The posterior vitreous cortex may split at the macular area

Laparoscopic splenectomy is now the gold standard for patients with idiopathic thrombocytopenic purpura (ITP) undergoing splenectomy. There are a few reports in literature on single-incision laparoscopic (SIL) splenectomy. Herein, we describe a patient undergoing SIL splenectomy for ITP without the use of a disposable port device. We report a 20-year-old female patient with steroid-refractory ITP having a platelet count of 14,000/cmm who underwent a SIL splenectomy. Dissection was facilitated by the use of a single articulating grasper and a gastric traction suture and splenic vessels were sec

A 71-year-old man was admitted to Teikyo University Hospital with paraplegia, subileus, back pain and high fever. He gave a history of having received treatment for spinal caries 20 years previously. Chest computed tomography and esophageal endoscopy demonstrated evidence of spinal caries and traction mid-esophageal diverticulum associated with kyphosis of the spine. The present report describes the extremely rare case of diverticula associated with kyphosis of the spine caused by spinal caries (Pott's spinal caries).
