Topical Tranexamic Acid to Reduce Postoperative Blood Loss in Total Knee Arthroplasty.

INTRODUCTION: Total knee arthroplasty alleviates pain and restores function in patients with osteoarthritis of the knee, but it is associated with postoperative blood loss resulting in anemia and allogeneic blood transfusion in 10% to 38% of patients. STEP 1 PREPARE SOLUTION: Prepare tranexamic acid solution using aseptic technique. STEP 2 APPLY SOLUTION: Apply tranexamic acid solution to the open joint and soft-tissue surfaces. STEP 3 REMOVE SOLUTION AND CLOSE: Remove tranexamic acid, keeping the tourniquet inflated until the wound is closed and the dressing is applied. RESULTS & PREOP/PO…

Endobronchial administration of tranexamic Acid for controlling pulmonary bleeding: a pilot study.

BACKGROUND: Tranexamic acid (TA) is a synthetic antifibrinolytic substance used for short-term control of bleeding. However, control of pulmonary bleeding with endobronchial administration of TA in more than 2 cases has not been attempted earlier. OBJECTIVE: We tested the feasibility of endobronchial administration of TA for controlling pulmonary bleeding. METHODS: This was a 2-year prospective observational trial of endobronchial TA in consecutive patients presenting with hemoptysis to our bronchoscopy unit. Patients were initially treated according to the recommendations of the Spanish Nat…

Polymyoclonus seizure resulting from accidental injection of tranexamic acid in spinal anesthesia.

We present a case of accidental injection of tranexamic acid instead of bupivacaine during spinal anesthesia. One minute after intrathecal injection of 3.5 mL of solution, the patient developed myoclonus of his lower extremities. Accidental intrathecal injection of the wrong drug was suspected and a used ampule of tranexamic acid discovered in the trash can. The ampules of tranexamic acid (500 mg/5 mL) and bupivacaine (5 mg/mL, Merck, Darmstadt, Germany) were similar in appearance. General anesthesia was induced. Ten hours later, the patient developed myoclonus of his upper extremities and fac…

Local antifibrinolytic treatment with tranexamic acid in hemophilic children undergoing dental extractions.

The incidence of post-extraction bleeding and the amount of replacement therapy needed to control bleeding in 24 hemophilic children following the local use of tranexamic acid mouthwash have been investigated. The results of the study showed that 91.6% of patients who used tranexamic acid mouthwash as a supplement to systemic therapy, did not develop post-extraction bleeding; while in 25% of the control patients who received only systemic tranexamic acid, postoperative bleeding was not observed.

Local delivery of tranexamic acid for hemorrhage control in trauma: a narrative review.

BACKGROUND: Uncontrolled hemorrhage, often accompanied by trauma-induced coagulopathy is a leading cause of preventable death, accounting for 30%-40% of trauma fatalities. Tranexamic acid (TXA), an antifibrinolytic agent, has been extensively studied and proven effective when administered systemically early in severe trauma. However, intravenous (IV) administration poses logistical challenges in prehospital and combat settings, and potential risk of thrombosis. Emerging strategies aim to enhance TXA delivery through localized systems and alternative routes. OBJECTIVE: The review explores non-…

The Demonstration of the Net Effect of Tranexamic Acid on Capsule Contracture in a Rat Model.

BACKGROUND: This study investigates capsule contracture, the most common and distressing complication following breast implant surgery, characterized by excessive fibrosis due to an exaggerated inflammatory response to the implant surface. Despite the increasing number of studies on capsule contracture in recent years, limited research has been aimed at reducing it by preventing hematoma, whereas there is not any study demonstrating the direct effect of tranexamic acid on capsule contracture. METHODS: A total of 30 rats were divided into 3 groups of 10 rats each: a control group, a group rece…

Tranexamic acid for management of acute hemorrhage in pediatric patients in the emergency department.

Tranexamic acid (TXA) is an antifibrinolytic medication with multiple routes of topical and systemic administration that can be used to treat acute hemorrhage across a broad range of clinical scenarios. Pediatric data are more limited than the adult literature, which benefits from larger, higher-quality studies; however, TXA remains a potentially effective, safe, and versatile option for temporizing bleeding and limiting blood loss in children. This review discusses the evidence on TXA use in traumatic hemorrhage, traumatic brain injury, posttonsillectomy bleeding, epistaxis, oral and gingival…

The use of tranexamic acid in paediatric adenotonsillectomy - A systematic review and meta-analysis.

BACKGROUND: Tonsillectomy and adenoidectomy (AT) are two of the most commonly performed ENT procedures in children, with over 500,000 cases performed annually in the United States. Whilst generally considered a safe and well-tolerated operation, it is not without its risks and complications including pain, nausea, anorexia and most importantly bleeding and post-tonsillectomy haemorrhage (PTH). Whilst tranexamic acid (TXA) has a proven benefit in reducing bleeding and transfusion requirements in patients undergoing trauma, spinal and cardiac surgery; the effectiveness, timing and safety of its …

Right Drug in the Wrong Place: The Effects of Inadvertent Intrathecal Tranexamic Acid.

Tranexamic acid (TXA) is an antifibrinolytic drug widely used to reduce blood loss in major surgeries and trauma patients, thus reducing morbimortality. In recent years, clinical indications for TXA have expanded, including many off-label uses. This broad use has led to an increased incidence of reported side effects and administration errors with serious neurological and cardiovascular outcomes, such as seizures, myoclonus, and arrhythmias. This case report describes a 75-year-old female, American Society of Anesthesiologists (ASA) physical status III, who underwent lumbar spine surgery and r…

Intramuscular administration of tranexamic acid in a large swine model of hemorrhage with hyperfibrinolysis.

BACKGROUND: Traumatic injury with subsequent hemorrhage is one of the leading causes of mortality among military personnel and civilians alike. Posttraumatic hemorrhage accounts for 40% to 50% of deaths in severe trauma patients occurring secondary to direct vessel injury or the development of trauma-induced coagulopathy (TIC). Hyperfibrinolysis plays a major role in TIC and its presence increases a patient's risk of mortality. Early therapeutic intervention with intravenous (IV) tranexamic acid (TXA) prevents development of hyperfibrinolysis and subsequent TIC leading to decreased mortality. …

Efficacy of topical tranexamic acid in epistaxis: A systematic review and meta-analysis.

INTRODUCTION: Epistaxis is a very common presentation in the emergency department (ED), accounting for approximately 1 in 200 ED visits in the United States. Currently, standard practice includes the initial use of topical anesthetics and vasoconstrictors, followed by more invasive treatments such as nasal packing, cauterization or surgical ligation for refractory cases. Over the years several studies have investigated the potential use of topical Tranexamic Acid (TXA) in the management of epistaxis. We have conducted a meta-analysis to assess the efficacy of topical TXA versus other standard …
